Intramuscle buttock filling 4,725 clinical cases

Objectives: The objective of this paper is to demonstrate the effectiveness of applying intramuscular fillers, in
4725 cases, to correct deformities, volume losses, asymmetry and irregularities.
With the demonstration of the correct application plans, adequate volume, correct technique for the desired results with 2,1% of complications

Introduction: Between 2009 and 2022 we performed a total of 4725 buttocks filling procedures with filler intramuscularly.
Almost 1 million ml of filler was used in these 4,725 procedures, administered through an atraumatic cannula, under local anesthesia with the patient monitoring the procedure in real time.

Materials / method: Intramuscular filler was used, with a cannula, between the years 2009 and 2024, almost 5 thousand filling
procedures with almost 1 million filler used.

Results: We show the complications observed in this sample, 2.1% of complications, no serious complications observed in this
sample. Nodules and seroma were the most common complications, both with a rate of 0.5%, followed by hemtama and pain,
both treated and eliminated in less than 30 days.

Conclusion: By applying the correct technique, adequate volume and manual skill, it is possible to successfully perform body
corrections in the gluteal area. With this procedure that I have been carrying out daily for over 10 years, I can restore
self-esteem, promote quality of life and make women feel more confident and happy with themselves.

Tranexamic Acid Mesotherapy in Melasma: “Spot on Strategy”

Objectives: • Present a case series of successful treatment of melasma with the use of tranexamic acid mesotherapy in Latin-American women.
• Literature review regarding tranexamic acid mesotherapy in the treatment of melasma and other hyperpigmentation disorders.

Introduction: Melasma is a type of hyperpigmentation that most frequently affects the face and rarely extends to the chest and forearms. It is disproportionally prevalent in women of Latin-American descent making it a common concern in the dermatological practice, also representing a big therapeutic hurdle in many cases. The following case series aims to highlight the use of tranexamic acid mesotherapy as an alternate strategy for melasma treatment, as well as evaluating its use in other formulations and the evidence that supports newer therapeutic strategies.

Materials / method: Patients recruited in private practice after being evaluated by the dermatologist who oversaw the procedure and calculated the MASI score. A liposomal solution with tranexamic acid, along with IL-10 and human growth hormone was used along with a standard anti-pigment regimen without hydroquinone. Sessions are done monthly with 2.0cc in each session and following a 0,5cm grid for each injection site with 0,05cc per point. Photos were taken before each mesotherapy appointment.
Short literature review was done in PubMed database to evaluate evidence behind tranexamic mesotherapy use in melasma.

Results: 8 female patients recruited for mesotherapy sessions with varying degrees of melasma, mean age of 42 years, different number of monthly sessions (one to three). All achieved improvement based on photographic follow-up and MASI score.
Data base evaluation with “tranexamic acid” and “mesotherapy” had 9 studies that evaluated hyperpigmentation disorders, mostly melasma. Pharmacological evidence behind its use indicates that by inhibiting plasmin activation it indirectly inhibits melanogenesis. Treatment regimens vary regarding frequency and concentration but coincide in volume and point interval.

Conclusion: Patients submitted to monthly tranexamic acid mesotherapy show important improvement both in photographic follow-up and MASI score without significant adverse events. These results, along with evidence found through database review highlight the importance of this local spot-on therapy that may avoid limitations associated with systemic tranexamic treatment. However, more evidence is needed to standardize treatment regimens.

CLINICAL CASE AWARD - TREATMENT OF INFLAMMATORY ACNE USING NON-ABLATIVE FRACTIONAL LASER

Objectives: - Evaluate the effectiveness of non-ablative fractional laser in the treatment of inflammatory acne.
- Analyze the impact of laser treatment on life quality of patients participating through the application of the Dermatology Quality of Life Index (DLQI).
- Quantify the improvement in the severity of acne before and after treatment.
- Evaluate satisfaction, tolerability and adverse effects of the fractional laser with a questionnaire applied to patients.

Introduction: Acne is a chronic inflammatory disease of the pilosebaceous unit whose pathogenesis is multifactorial and it presents a pattern of relapse difficult therapeutic management, which has an important psychosocial impact associated. The use of lasers and technologies in the treatment of acne has increased recently due to their safety and effectiveness. This study aims to evaluate the effectiveness of using a non-ablative Nd:YAP 1340 fractional laser in the treatment of inflammatory acne.

Materials / method: 8 patients with facial inflammatory acne underwent 4 Nd:YAP 1340nm laser sessions with an interval of 30 days between them. Data were collected by evaluating pre and post-procedure photographs, pre and post-procedure active acne severity index scale and Dermatology Life Quality Index (DLQI) questionnaire before and after treatment, in addition to the satisfaction and tolerability answered by patients at the end of treatment.

Results: All of the 8 participants who completed the study showed a positive outcome after applying laser therapy in relation to the severity of their acne, with no worsening in any of the cases. Regarding the patient's subjective assessment, all of them showed a drop in their DLQI score, from 22.62 points (pre-treatment average) to 4.37 (post-treatment average). No side effects were observed during the treatment.

Conclusion: The treatment with non-ablative fractional Nd:YAP 1340nm laser proved to be effective and safe in the treatment of inflammatory acne, being a promising tool in this condition. The method can be considered as an adjuvant to conventional therapy and should be considered as an alternative in patients who are resistant to systemic medications and in those with intolerance to systemic or topical medication. Further studies with more patients and longer follow-up time are necessary in order to consolidate the author's conclusion.

Platelet rich plasma in androgenetic alopecia treatment

Objectives: The objective is to show how PRP can improve and boost even more the gold standard treatment in male and female patients with androgenetic alopecia

Introduction: Platelet rich plasma is rich em growth factors that can boost the treatment of androgenetic alopecia, one of the principal causes of dermatologic visits. The gold standard treatment is the use of minoxidil, but in some cases it is not sufficient for the treatment and good results.

Materials / method: For this trial was selected patients with androgenetic alopecia Hamilton I - III or Ludwing I - II
We use 11 ml of blood centrifuged for 10 minutes and infuse at the alopecia area 3 ml of PRP monthly for 3 months.
Padronize pictures were taken and analyse by a system (HairMetrix) to improve the analysis.
One group received only topic minoxidil and saline injections monthly and the other group topic minoxidil and PRP injection during the same period.

Results: We are still on going with the study and waiting to see the final results.
The parcial results are promising and all patients that did the PRP were satisfied with the visual results

Conclusion: We are still working to see all the final results.
As a partial result it is possible to see clinical improval at the androgenetic alopecia, but we are waiting for the end of all patients treatments to have the final results

Use of ablative fractionated laser with 5-fluorouracil drug delivery in the treatment of genital diseases.

Objectives: GENERAL OBJECTIVE:
To determine the efficacy of combined ablative fractionated laser and 5FU treatment as a single or adjunct therapeutic option for inflammatory and non-inflammatory lesions present in the genital area.
SPECIFIC OBJECTIVES OF THIS STUDY:
1.To describe the sociodemographic and clinical characteristics. 2.To assess the effectiveness of treatment using ablative fractionated laser combined with 5FU by analyzing improvements in clinical characteristics, and histopathological aspects. 3.To evaluate the impact on patients' quality of life using the DLQI scale.

Introduction: Inflammatory and non-inflammatory diseases affecting the genital area can have a
significant impact on patients' quality of life and intimate relationships. Despite being
common in dermatology outpatient clinics, these conditions are still considered taboo within
the field of dermatology. There is a lack of literature describing the clinical and pathological
correlation of these diseases, along with limited treatment options available.
Surgery is considered the gold standard treatment for the sequelae of these diseases;
however, it is invasive and can negatively affect patients' self-esteem. A

Materials / method: A quasi-experimental, open-label, pre-and-post, single-center, non-randomized study was conducted using a convenience sample of patients from the hospital outpatient clinic. Patients meeting the inclusion and exclusion criteria and expressing willingness to participate were enrolled between September 2023 and October 2023
Inclusion Criteria:
Age over 18 years
Lesions located in the genital area
Clinically and histopathologically confirmed diagnosis of LSIL, LS, BP
Exclusion Criteria
Nonspecific initial biopsy
Previous laser treatment for the same condition
Previous treatments 30 days prior

Results: SYMPTOMS: At the beginning of the study, 84.6% of patients (n = 11) reported itching, and 61.50% (n = 8) reported burning. However, by the end of the study, only one patient continued to experience mild itching, indicating substantial improvement in symptoms for the majority of participants.
HISTOPATHOLOGY
The presence of acanthosis, dermal fibrosis, increased fibroblasts in the papillary and reticular dermis, as well as decreased skin hyalinization, epidermal atrophy, and inflammatory infiltrate indicate a consistent and beneficial therapeutic response.

Conclusion: CONCLUSIONS
The 2940 nm fractional ablative laser + 5-fluorouracil has shown to be an effective alternative in the treatment of genital lesions, including lichen sclerosus, low-grade squamous intraepithelial lesion, and Bowenoid papulosis. The treatment contributed to the improvement of symptoms and clinical signs, such as color, brightness, thickness, and extension, although it did not show efficacy in managing extensive LS adhesions
The most common findings after intervention were changes in dermoscopy and histopathology in all study patients. These changes indicate a positive response.
