Objectives: The objective of this paper is to evaluate the Technique that will help in calculating the dimensions of donor area strip for harvest, with safety and minimal or no donor area scarring. It is very important in cosmetic surgery to give the naturalness in reconstructive area, but equally important to preserve the integrity and avoid the un-naturalness. Donor area preservation is very important, especially with recent hair styles. This technique gives minimal visible scarring in over 85% of cases
Materials / method: Few important steps need to be taken to make your scar look less visible and to avoid problems in second surgery.
1) tricophytic lower edge closure, was done in order to produce what is called invisible scarring of the donor area. Simple 1 mm wedge excision of the lower edge of the wound was done. The scar was then evaluated for the visibility..
2) While excising the donor area strip some loose tissue is left behind, in order to reduce the morbidity of this donor area scar healing that tissue needs to be removed as well.
3)Donor area mobility was taken into account to determine the width
Results: All Surgeries were performed by same surgeon and same evaluation technique was used in patients. The initial patients did experience some tight closure and tellogen effluvium effect on the donor area. All results were evaluated by another independent surgeon. Some learning difficulties were also met.
over 85% of the cases the scar was less than a mm. In most cases the direction of the hair, above and below the scar were also aligned.
Conclusion: This technique of donor area closure has proved to be very simple to understand, can be reproducible and dependable. It has shown a significant decrease in the post surgical donor area scarring. This simple, safe, reproducible technique will help hair restorative surgeon especially the lesser experience surgeons to execute their surgeries with minimal donor area problems. This simple technique along with few points if followed will allow the new surgeons to harvest the donor area with out making bad looking scars.
