Combination of filler and toxins for upper face rejuvenation

Objectives: Forehead rejuvenation with fillers was neglected for many years and due to being high risk most of the physicians focus on pure toxins.
As aging is a complicated process and affecting all layers of the face, a combination therapy is always more effective and injection of the fillers is getting more popular.

Introduction: In this lecture I will describe the general concepts of beauty and aging in the upper face with comparisons of the Asian and caucasians. I will also briefly talk about the differences of male and female.
A quick review of the anatomy and vascular complications of the forehead and temple.
Some of the injection techniques with needle and cannula and I will show 1-2 videos of my techniques

Materials / method: All materials are power points slides and videos
And I have uses some photos from different references as well

Results: In this lecture I will review my land marking , injection techniques and results of my patients

Conclusion: Upper face is a high risk are for injection of the filler due to vascular supply and anastomosis between internal and external carotid artery .
Having good understanding of the anatomy and the planes of injection make the risks minimal
Aging is a multi factorial process and combination of toxins and filler can be more effective

A novel combined triple-wavelength (755nm, 810nm and 1064nm) laser device for hair removal: efficacy and safety study

Objectives: To evaluate safety and efficacy of hair reduction using a novel simultaneous multi-wavelength laser device

Introduction: : Photoepilation by a single-wavelength laser device is being commonly used for hair removal. Whether emitting multiple laser wavelengths to induce hair unit damage would be beneficial is yet to be established

Materials / method: A retrospective cohort study was carried out among adult males. Hairy sites were treated by a simultaneous triple-wavelength (755nm, 810nm and 1064nm) laser device. Treatment parameters were: fluence of 7-9J/cm2, pulse frequency of 9-10Hz and spot sizes of 2cm2 or 4cm2. Participants underwent up to 6 treatments at 6-8 weeks intervals and were followed for 4-6 months. Clinical photography before and after treatment was documented. Treatment efficacy was assessed using Global Aesthetic Improvement Scale by two independent dermatologists. Pain perception and adverse events were recorded as well.

Results: A total of 27 treatment sites among 11 participants, with Fitzpatrick skin type III - V, were included. Mean hair reduction was 3.4 out of 4 in the GAIS. Participants' satisfaction rate was high (mean 4.8). No adverse events were recorded

Conclusion: Simultaneous triple-wavelength laser device is a safe and effective hair removal modality, and may be superior to the traditional one wavelength devices. A large (4cm) handpiece can reduce treatment time in half-and severs as an additional advantage.

Noninvasive periorbital rejuvenation using ablative and non-ablative combination of multisource RF technology

Objectives: To investigate the safety and efficacy of multisource phase control focused RF energy treatment combined with ablative fractional RF for periorbital areas.

Introduction: The periorbital area is one of the first areas to show the signs of ageing such as laxity, fine lines and wrinkles. Treatment options are limited, due to the delicate nature of the skin in this area and safety concerns related to proximity to the eyeball.
RF has been found to be effective in the safe delivery of energy into the skin, independent of skin color. This study utilizes a sophisticated Multisource phase-controlled RF technology which allows the creation of multiple heating fields for deep penetration together with ablative fractional RF.

Materials / method: 10 patients completed a course of 3 combined sessions using a multisource RF designed for periorbital areas and an ablative fractional RF handpieces. 4 weeks interval was set for the treatments. Each session included 30 minutes of RF energy delivery followed by ablative fractional RF pulses around the eyes. Power range for multisource RF was between 3W-5W, surface temperature was maintained at least 40°C. Ablative fractional RF parameters were 5W and pulse width of 30msec. All patients were photographed before treatment, and at follow-up visits, side effects were monitored

Results: All the subjects recruited for study completed the treatment protocol.
Patients reported the treatment was comfortable and painless. Mild edema and erythema were observed up to 24 hours post treatment. Most patients reacted with crusts to the ablative treatments due to high intensity of parameters. No severe adverse events were reported during and after the treatments. Patients’ before and after photos, were graded according to Quantitative Comprehensive Grading Scale of Rhytids, Laxity, and Photoaging and according to it over 75% of patients had significant clinical improvement.

Conclusion: Effective non-surgical periorbital skin rejuvenation and wrinkle reduction is a big concern of many patients, who are looking for a short treatment sessions with minimal downtime. Combined protocol of multisource RF with ablative fractional RF was safe and effective treatment option for the periorbital area.

Emerging challenges for aesthetic manufacturers: what is the state of the art? Ensure compliance with the new Medical Device Regulation

Objectives: understanding of the upcoming changes for entering the european market

Introduction: The growing use of devices that do not have a specific purpose in the medical field but that are used both in the professional and private sectors, has pushed the European legislator to insert them in the EU Reg. 745/2017 (MDR) to protect not only the public health but also manufacturers and end users. These devices are specifically identified in Annex XVI of the MDR Regulation

Materials / method: among these are predominant those used in the aesthetic field, such as coloured contact lenses, fillers or other skin injectables (except those used for tattoos), equipment used for liposuction, lipolysis or lipoplasty.
Even in the absence of clear information, the manufacturers of devices for aesthetic use, having to fulfil these new obligations, shall start planning an adequate Quality Management System and prepare the Technical Documentation required by the MDR Regulation.

Results: The goal of aesthetic devices inclusion within MDR is a demonstration of constant caring for improving the safety in the interest of public heath. Common Specifications delay is leading to a climate of pressure and negativity from stakeholdes. Continuous alignment on the timeline and new requirements is essential to keep updated manufacturers and ensuring optimal harmonization and adaptation.

Conclusion: This session will describe the current understanding of requirements that aesthetic manufacturers shall fulffill based on MDR analysis and Medical device coordination group document, providing a general overview of the state of the art applicable to these devices, that will be newly impacted by the Regulation.

Enzymatic proteins for cellulite

Objectives: My mere objective is to share and teach my 5 year experience regarding biological enzymes

Introduction: Enzymes can treat the different components of the cellulite. Their combination is carefully calculated depending on the level and type of cellulite that the doctor is eager to treat.

Materials / method: The combination of different enzymes, including lypase, hyloronidase and collagenese.

Results: Prominent results are expected after 4 to 8 sessions, one per month.

Conclusion: All in all, cellulite is a Chronic and evolutionary pathology of women. Nevertheless, with this novel treatment with biological enzymes, applied on dipplings and nodules of the PEFE, the estetical imperfections of cellulite can be treated. Likewise, these biological enzymes can serve to remodel localized fat.

Surgical vs non-surgical facial rejuvenation: the patients’ choice

Objectives: Patients seeking pan-facial rejuvenation often present to clinicians for their desired service once they have decided on either surgical or non-surgical options, but without actually first seeking proper initial medical advice as to which is best for them. Among our patient cohort, we sought to understand the factors that influence the choice of surgical versus non-surgical options. An understanding of these factors assists in counselling patients adequately, planning their treatments, and guiding them realistically as to the outcomes that can be achieved.

Introduction: Surgery achieves superior anatomical corrections of age-related secondary changes and a more youthful appearance that nonsurgical rejuvenation only can approximate. Nonsurgical rejuvenation procedures can be staged and tailored to the limits of a patient’s budget and required downtime. However, patients influenced by marketing, social media and peer influences may have unrealistic expectations. Nonsurgical interventions can disrupt tissue planes and lead to scarring, potentially increasing complication rates of any future surgical interventions, and patients are often unaware of this fact.

Materials / method: Ninety-two patients who had consulted the practice about pan-facial rejuvenation in the last 12 months were surveyed. A questionnaire was electronically distributed to this group, and the results were collated for analysis. This survey was devised to gain an insight into the factors that influence a patients choice.

Results: Forty-seven percent of respondents had considered facelift surgery and 14% of them had sought further information or plastic surgery consultations. Interestingly, 44% of respondents would still consider a facelift at a later stage. The most common reason for selecting nonsurgical treatments was the desire for natural and subtle results. Other reasons included cost, the flexibility and choice of treatments, concerns about risks and downtime from surgery and anaesthesia, the ability to divide treatments over time, less scarring and pain.

Conclusion: Nonsurgical interventions are perceived as delaying rather than preventing surgery in 44% of our study group, yet surgery is potentially made more difficult when there has been prior intervention or trauma. We advise patients to wait at least 3 years after nonsurgical or biostimulatory procedures before embarking on facelift surgery to allow any deep scarring to mature and soften. All patients considering nonsurgical rejuvenation treatments should be asked if they would ever consider surgery and where appropriate, receive surgical consultations prior to embarking on nonsurgical treatments.

The effect of neuromodulators on proprioception

Objectives: A retrospective review of patients who switched from onabotulinumtoxinA and/or obabotulinumtoxinA to incobotulinumtoxinA detected a difference in ‘feel’ not described previously. We wished to understand these patient-reported symptoms as they may impact toxin selection and treatment satisfaction.

Introduction: The four FDA-approved botulinumtoxinA preparations derive from the same Hall strain but are manufactured by different proprietary means. Despite similar clinical efficacies in aesthetic indications, physicians have reported some clinical differences between the preparations. These include a difference in spread, immunogenicity and time of onset. Some patients who switched from onabotulinumtoxinA and/or obabotulinumtoxinA to incobotulinumtoxinA reported that incobotulinumtoxinA felt ‘lighter’ while other toxins led to treatment-associated headaches or tightness.

Materials / method: Patients who had prior treatments with more than one botulinumtoxinA for aesthetic facial indications completed a questionnaire. The questions covered the botulinumtoxinA formulations and incidence with which the following symptoms occurred: muscular weakness, tightness, stiffness, headache, heaviness, and ‘frozen’ sensation in the targeted area. The results were collated in a spreadsheet for tabulation.

Results: The questionnaire was completed by 79 patients. Symptoms were reported by 69.6% of patients, 54% of whom observed differences in symptoms between formulations. The commonest symptom was tightness (33%) followed by headache (29%), heaviness (27%), ‘frozen’ sensation (20%), stiffness (14%) and muscular weakening in the targeted area (14%). Symptoms were experienced by 68% of onabotulinumtoxinA recipients, 38% of abobotulinumtoxinA recipients, and 11% of incobotulinumtoxinA recipients.

Conclusion: Our observations of differences in sensory symptoms with the different preparations could be linked with previously unknown mechanisms affecting the trigeminal branches. The occurrence and incidence of the observed symptoms occurred differently between the formulations, confirming that some patients can detect sensory symptoms and that there is a perceptible difference between formulations. This difference in symptoms must be considered when switching patients from one formulation to another as patients may assume that a toxin is ineffective or non-durable even if a motor effect is present.

“Anterior” chemabrasion for acne scars treatment – Scientific review

Objectives: Scarring is a common result of acne and may take the form of atrophic or hypertrophic scars. Acne scarring often occurs in highly visible areas such as the face, thus resulting not only in an un-desirable cosmetic appearance but also potential impairment of mental health, social functioning, and overall well-being. There is a wide variety of medical and surgical therapies available for treatment of acne scarring.

Introduction: The most commonly used cosmetic therapies for acne scarring include dermabrasion, laser resurfacing, radiofrequency (RF), subcision, skin needling, punch techniques, chemical peels, soft-tissue augmentation, intralesional therapy, cryotherapy, and silicone dressings.

Materials / method: The recommended concentration of TCA is 15% w/w and the recommended medical device is EASY TCA . Soft abrasion performs by using a rolled sandpaper. The pressure to be applied on the skin is a “scratching". The use of little circular movements is recommended for increasing uniformity of abrasion. The end point of abrasion is reached when confluent pinpoints of bleeding appear on the abraded area, it's correspond to the depth of Grenz zone.

Conclusion: The performance of the skin abrasion prior to the application of a chemical peeling (Easy TCA) is a good alternative for treating moderate and severe acne

Objectives: Hair loss( nonscarring alopecia) with a multi-factorial etiology is characterized by a reduction in hair density and hair miniaturisation. The affected areas are mid-frontal scalp, frontal hairline without any signs of inflammation. Alopecia poses only a cosmetic concern and may have a significant negative psychological impact. Based on post market clinical study, regular use of XL Hair ( injectable dermal implant) could stop hair-loss in 4 weeks and stimulates new hair coming in 6 weeks, injections recommend to be combine with special hair care.

Introduction: Buccal fat pad reduction surgery (BFPR) while limited in its therapeutic effect is a treatment with high patient satisfaction. It is possible for early social recovery and minimal down time due to the treatment’s intraoral approach. However, because the approach involves incision in the oral cavity, it is important to maintain proper understanding of the surgical field and anatomical structures such as the Stenon(parotid) duct etc., to perform the surgery. We are examining preoperative diagnosis, surgical techniques, postoperative complications its preventative measures.

Materials / method: We are examining surgical adaptation, excision volumes and complications based on the experience of 1,212 BFPR cases.

Results: Although the surgical procedure approach involves mandibular branch anterior border incision, it was effective in removing sufficient amount of BFP and is preventative of complications.

Conclusion: Faceline can be classified into bone and soft tissue. Regarding soft tissue, we should consider sagging cheeks, amount of fat and BFP, how prominent BFP is, and hypertrophic masseter muscle. As BFPR is possible for early social recovery and minimal down time, it is popular among a wide range of patients. MRI findings of pre and post treatment shows that BFPR is effective in reducing volume of lower face. However, we should regard its effect, such as possible change in soft tissue in the long term. As for mandibular branch anterior border incision, peeling region will be minimal and it is easy

Evaluation of rejuvenating effect of the treatment with high molecular weight HA in the perioral area: open-label clinical study

Objectives: The aim of the study was to assess the safety and rejuvenating effect of the treatment with high molecular weight HA and a cluster of amino acids in the perioral area.

Introduction: The lips and perioral area play a crucial role in facial aesthetics. The ageing signs (wrinkles, lack of tone, thinning of the skin, etc.) frequently appear first in this anatomical area and then in other areas. This is the reason for the numerous requests to correct this area, possibly without changing natural features. The author presents an open-label uncontrolled observational study of the safety and rejuvenation effect of an injectable product containing native HA with high molecule weight (HMW) and a cluster of amino acids (AA).

Materials / method: 18 female patients underwent 3 sessions of intradermal injections with HMW HA gel with a cluster of AA. Follow up after 30 days, 60 days and 180 days. The outcome was evaluated subjectively by the patient’s questionnaire, by the GAIS questionnaire and objectively by the 3D photosystem

Results: All patients concluded the study. The subjective evaluation made by patients and by the investigator during follow up and at the end of the study was very positive, all the patient found improvement of the perioral area. The objective evaluation showed a statistically significant improvement in wrinkles condition

Conclusion: The protocol of the rejuvenation of the perioral area with injections of HMW HA gel and a cluster of AA is safe and effective.

Combination therapy for facial rejuvenation - An analysis of how Asian faces age & an algorithm driven approach for planning anti-aging treatments

Objectives: To describe How Asian Faces Age. To Describe how that knowledge (derived from our published paper on analyzing 300 faces) allowed us to develop An Algorithm Driven Approach for Planning Anti Aging Treatments.

Introduction: Aging is the norm from the day we are born. Therefore, understanding specifically how faces age is critical in every ethnicity to plan anti aging treatments better.

Materials / method: We describe How Asian Faces Age. We also Describe how that knowledge (derived from our published paper on analyzing 300 faces) allowed us to develop An Algorithm Driven Approach for Planning Anti Aging Treatments. We demonstrate combination therapies to specifically retard aging in Asian faces.

Results: Combination therapies for facial shape changing and anti aging are described.

Conclusion: Non surgical and surgical therapies for anti aging are described using an algorithmic approach, specifically designed per the aging patterns in the Asian population.

A patented growth factor peptide vs PRP - A randomised, comparative, prospective study

Objectives: To discuss a randomized controlled trial between two groups of patients, comparing a patented growth factor formulation with Platelet Rich Plasma, for causing hair growth. If we can produce growth factors effectively outside the human body, using the latest technology, is it time for a tedious process like Platelet Rich Plasma (PRP) to become obsolete? After all, we do not necessarily extract Vitamin C from oranges to make high quality skin serum in our clinics.

Introduction: Hair plays a significant role in shaping the appearance of an individual. Loss of hair can lead to serious effects on social esteem of an individual. The most common cause of hair loss is Androgenetic Alopecia (AGA).This hereditary disorder followed a specific pattern causing progressive thinning of hair in both Men and Women.

Aims: The aim of the current study is to compare and evaluate the efficacy of a novel, patented, growth factor formulation versus PRP in the treatment of Male Androgenetic Alopecia.

Materials / method: A prospective, comparative, single-blind study was carried out with 2 groups of 25 patients each. Intradermal administration of the novel formulation and PRP were performed in group A and B respectively. Hair pull test, Video microscopic assessment, Global Photographic assessment was done and patient's subjective assessment was done through questionnaire at the end of the study. Results were evaluated after 6 months and follow up was done till 1 year.

Results: 100% reduction in hair fall was noted at the end of 6 months in the novel formulation group, which was maintained for 1 year. Video microscopic evaluation showed that the hair density, terminal hair density, vellus hair density and shaft diameter were significantly better in this group (P < .005) compared to the PRP group.

Conclusion: The bioengineered novel formulation proved to be more beneficial for Male Androgenetic Alopecia (Male pattern hair loss) compared to PRP.

The Importance of hormonal balance in aging and health

Objectives: The purpose of this clinical trial was to delineate some of the negative consequences of high BMI on health and explore the possibility of a solution. We analysed the blood test results of nine overweight adults with sedentary lifestyles, and an average BMI of 32.23. Results revealed a statistically significant reduction of visceral adipose tissue, very-low density lipoprotein (VLDL), and triglycerides. Testosterone, leptin, IGF-1 and Free T3 increased within the normal range, juxtaposed by cortisol and ghrelin that declined, but without dipping into abnormality.

Introduction: Optimal health appears to be the best defence against all disease and aging. Physical training is decidedly the most acclaimed solution, yet, the least implemented one, due to procrastination, or demoralization after investing constant exhaustive effort with no immediately visible physical change. Excess adiposity, elevated lipids, and hormonal imbalance instigate systemic disintegration, stemming from accumulated inflammation, dyslipidemia, metabolic dysfunction, and insulin resistance, starting from hormonal and appetite dysregulation & leading to accelerate aging and disease.

Materials / method: Nine adults, five females and four males, ages 35 - 49 years, with an average BMI of 32.23, participated and completed this fifteen treatments clinical trial that took place over a period of five weeks. Inclusion criteria were: 1) Overweight or obese. 2) Sedentary lifestyle. 3) Had previously received laser and radiofrequency treatments with modest outcome and rebound results. 4) Not currently following a particular diet plan. We adopted a novel technology from London University designed after 27 years of empirical research to reduce visceral fat and increase muscle tissue.

Results: All subjects indicated a BMI decrease after the fifteen treatments. There was evidence that visceral adiposity, VLDL and triglycerides were reduced, accompanied by increased metabolism, and an optimal leptin/ghrelin inverse relationship, indicating that this method can serve as a proactive/preventive and possibly corrective measure to counteract the adverse effects of obesity. The substantial IGF-1 and skeletal muscle mass enhancement implied enhanced fitness. Conclusion: The primary goal of this randomized double blind clinical trial was to bring attention to the systemic balance perspective of health maintenance. Deterioration into illness is gradual. Technological advances in accelerated wound repair, stretch marks, keloid scars, and facial regeneration

Objectives: We reviewed a number of wound repair, keloid and hypertrophic scar research methods that included lasers, microcurrent and ultra-low energy technologies. Laser research reports short-term improvement in wounds, keloid and hypertrophic scars, but without follow up to control for reoccurrence of keloids or diabetic lesions which generally reoccur following laser treatments. The microcurrent and ultra-low energy studies demonstrate significant healing where age is not a factor with no reoccurrence of diabetic wounds and other skin lesions. Introduction: 8 wound repair clinical cases with an age range of 28 - 86, followed for one year, evidenced accelerated healing and no reoccurrence. The number of treatments required for substantial healing depended on the chronicity and severity of the lesion, with chronic severe lesions requiring more treatments, rather than age, a conclusion supported by ultra-low microcurrent research. These results on age-independent wound healing directly contradict a large body of literature postulating that healing is much slower with age.

Materials / method: Eight subjects, one male and 7 females, 28 - 84 years of age, one Caucasian, 4 Hispanic and 3 Chinese were included in this clinical trial. Subjects were randomly selected out of 15 cases of wound healing cases treated by 4 independent clinicians in their private clinics. We utilized an ultra-low energy nanotechnology, originally invented in London University in 1992 and subsequently modified over a period of 20 years on the basis of unpublished in vitro, clinical and electronic research.

Results: The results of this clinical trial indicated age-independent wound repair in our eight 28 - 86 years old adult subjects of different ethnicities. The number of treatments required for skin repair was related to the chronicity and severity of the lesion, with more chronic severe lesions requiring more treatments irrespective of the subject’s age. Subjects were monitored for at least one year after treatment, confirming the absence of reoccurrence.

Conclusion: Our results were consistent with previous research using ultra-low microcurrents demonstrating accelerated wound healing, including diabetic foot lesions, where age was not a factor.

Age-independent wound repair seems to defy a number of aging theories postulating inevitable decline and irreversible inflammation. It directly contradicts the concept of “inflammaging” of a slower healing with age due to immune insufficiency, accumulative oxidative stress & inflammation.

How to get rid of visceral fat

Objectives: Inflammation and oxidative damage are immanent in visceral adiposity that is characterized by excess lipids and lipoproteins, viewed as the core components of arterial plaques, ultimately obstructing blood flow and lymphatic drainage. Accumulated toxicity dysregulates the orexigenic hormone ghrelin, and anorexic hormone leptin that are part of a reciprocal network controlling appetite. Weight gain promotes hormonal imbalance, expressed in disturbances in free T3, and an inverse low testosterone / high cortisol incongruity that provokes stress eating behaviors.

Introduction: We explored a number of interventions designed to reduce visceral adipose tissue (VAT), including RF, laser plus working out, and exercise alone. Short term gymnastics evidenced a modest advantage in VAT decrease, but no changes in BMI or physical appearance. Overtraining appeared to negate the benefits of exercise by increasing inflammation and cortisol while suppressing testosterone and leptin, inevitably instigating hunger and weight gain.

Materials / method: We examined the blood samples of 30 overweight, healthy adults that underwent twelve treatments during the course of one month. Results demonstrated a statistically significant decline in VLDL, triglycerides and VAT, accompanied by a substantial increase in BMR and skeletal muscle mass. Importantly, Free T3, IGF-1, leptin, and testosterone were elevated towards the top of the normal range, while cortisol and ghrelin gravitated towards the low end of the normal range, without ever spiking outside the limits of hormonal balance.

Results: Subjects reported reduced cravings for sweets and fatty foods, yet, regular appetite. Physical activity has been traditionally the reliable in reducing visceral fat; however, short-term workout produced negligible gains in weight loss and physical appearance. Furthermore, overtraining evidenced negative effects, including increased inflammation, leptin suppression, and an inverse negative cortisol / testosterone incongruity bound to negate the benefits of exercise by increasing food consumption.

Conclusion: In the current trial, the blood samples of 10 healthy adults demonstrated a significant increase in BMR and skeletal muscle mass, contrasted by a decrease in VAT, VLDL, and triglycerides. Free T3, IGF-1, leptin, and testosterone were elevated towards the top of the normal range, while cortisol and ghrelin gravitated towards the low end of the normal range. These results have important implications for weight loss plus speedy fitness that balances hormones and safeguards health.

Female social empowerment - Hidden emotional issues

Objectives: Sexual satisfaction questionnaires reporting female contentment after vaginal rejuvenation are reviewed with respect to females’ tendency to focus on satisfying their partners rather than themselves. Our double-blind longitudinal clinical psychological research included 14 menopausal women with high FSFI satisfaction scores following laser or RF vaginal interventions.

Introduction: Undergoing a surgical vaginal rejuvenation procedure can have serious side effects, including bleeding and perpetual discomfort, which is why a number of Federal agencies, including the FDA, have recently issued warnings against such cosmetic interventions. A recent study on 46 patients revealed sustained injuries following laser and RF vaginal rejuvenation including vulvar, bladder or urethral pain, vaginal numbness or burning, increased dyspareunia, and loss of sexual sensation. Around 72% of these patients reported long‐term infections, bladder disturbances, scarring, lichen sclerosis,

Materials / method: and disfigurement. We analysed the testing records and psychotherapy notes of 14 postmenopausal women who consented to participate to the clinical trial and completed it without any subject attrition. Tests included the FSFI [64-65], Izard’s Differential Emotions Scale(DES) [66-68] and the Minnesota Multiphasic Personality Inventory (MMPI-2) a 567-item standardized psychometric test of adult personality and psychopathology based on a large number of reliability and validity studies [69-74]. Test results and clinical notes were collected from three independent clinics who approved the research.

Results: Statistical analysis using both the Pearson’s Correlation Coefficient and the Spearman Pho tests indicated a statistically significant correlation between the FSFI and the Lie-scale (L) of the MMPI at p<0.01, suggesting that these women were untruthful in their FSFI responses. Both statistical tests yielded a strong correlation between the FSFI and the Depression-scale (D) of the MMPI at p<0.05, indicating that these women’s smiling persona concealed a general dissatisfaction with their lives, brooding subjective depression, mental dullness and perpetual worrying. Materials / method: Twenty obese and overweight adults, 15 - 82 years, with an average BMI of 35.41, eleven females and nine males, with either a diabetic or prediabetic condition, participated and completed the study, after they signed a consent form. Fifteen of the subjects were of Indian descent and five of them were Caucasian. Selection was made by randomly selecting medical history files from two different clinics and making sure that the participants fulfilled both the inclusion and exclusion criteria:

Results: Results were analysed with the t test for dependent means (Table 6). Results for overall fat loss were statistically significant at p < 0.00001; with a mean of −7.25, a square deviation of 245.69, a T value of t = −9.01646. Results for visceral fat reduction were also highly statistically significant at p < 0.00001; with a mean of −4.88, a square deviation of 94.11, a T value of t= −9.805937. The results of skeletal muscle mass (SMM) increase were statistically significant at p < 0.001; with a mean of +5.18, a square deviation of 7.63, a T value of t = +8.387633 and a p value of p < 0.00055.

Conclusion: Results demonstrated a significant improvement in blood glucose regulation and insulin resistance, along with reduced dyslipidemia, reflecting an optimal reverse relationship of elevated HDL versus triglycerides descending towards the normal range. The substantial reduction of visceral fat was supported by the cm loss around the umbilicus and lower abdomen which represent the body areas where visceral adiposity accumulates; it was also confirmed by the sonography reports of seven subjects diagnosed with hepatic steatosis who presented evidence of no fatty liver after the twelve treatments.

Plasma exeresis therapy in combination with NBUVB phototherapy: a novel approach towards repigmentation in vitiligo

Objectives: To determine the safety and efficacy of plasma exeresis as a treatment of vitiligo.

Introduction: Plasma exeresis is an effective non-surgical treatment used for multiple dermatological and cosmetological procedures. Its various applications include removal of moles, skin tags, correction of rhytides, marionette lines and for upper eyelid dermatochalasis [1] as well. Plasma exeresis devices converts the air into the ionised gas in the space between the instruments tip and the target tissue being treated [2].
Recent studies have reported development of hyperpigmentation after treatment through plasma exeresis. This was particularly reported in Fitzpatrick Skin types from IV to VI [3]. The

Materials / method: This case series was conducted at Madina Teaching Hospital Faisalabad after approval from the ethical review committee. 5 patients were selected on the basis of pre-determined inclusion criteria. They were receiving NBUVB phototherapy for stable vitiligo along with topical calcineurin inhibitors application. Selected patches of vitiligo were subjected to sessions of plasma exeresis under application of topical anaesthesia after three weeks. They were called on follow up on a weekly basis and results were noted with the help of serial photographs, VASI scoring and physician assessment. Patient
