A randomized study evaluating pain and safety of hyaluronic acid with lidocaine for correcting moderate to severe nasolabial folds (unrestricted educational grant from Galderma)

Objectives: To evaluate pain associated with injection of hyaluronic acid filler, HAR with addition of lidocaine.
The aim of the study was to show that subjects experienced less pain from injection of HAR with lidocaine compared to without.
The proportion of subjects that had a within-subject difference of ≥10 mm on an Visual Analogue Scale (VAS) at the time of injection was calculated with a 2-sided 95% confidence interval (CI). The objective was to show that the CI was above 50%.

Introduction: Addition of lidocaine to hyaluronic acid fillers brings a better patient experience of the treatment and is convenient for injectors that otherwise usually would apply local or topical anesthesia prior to injection.

Materials / method: This was a randomized, multi-center, subject, and evaluator-blinded, 2-week study. Chinese subjects with moderate to severe nasolabial folds were injected with HAR with 0.3% lidocaine in one nasolabial fold and HAR without lidocaine in the other.
Maximum recommended volume to achieve optimal correction was 1.5 mL per side
Injection site pain was assessed for each side on a 100 mm VAS at the time of injection and at 15, 30, 45, and 60 minutes after treatment.
Wrinkle severity and aesthetic improvement of nasolabial folds was assessed.
Subject diary entries and adverse events were collected.

Results: Demographics and treatments
Effectiveness on Visual Analogue Scale, Wrinkle Severity Rating Scale, Global Aesthetic Improvement Scale
Safety on Subject diary entries of pre-defined, post-treatment symptoms and adverse events

Conclusion: The addition of lidocaine to HAR had a substantial effect on pain relief during injection, and a similar safety profile as HAR without lidocaine in a Chinese population.
Nasolabial fold wrinkle severity was reduced, and aesthetic improvement was comparably high after both treatments.

Effectiveness in pain reduction with addition of lidocaine to hyaluronic acid for correction of nasolabial folds in Asian subjects (unrestricted educational grant from Galderma)

Objectives: The aim of the study was to evaluate the pain associated with injection of hyaluronic acid filler, HARL with lidocaine compared to HARL without lidocaine, for correction of moderate to severe nasolabial folds.
The proportion of subjects that had a within-subject difference in visual analogue scale (VAS) score of at least 10 mm at injection was calculated together with a two-sided 95% confidence interval. The objective was to show that the confidence interval was above 50%.
Secondary objectives were evaluation of effectiveness and safety of the study products.

Introduction: Based on previous clinical experience in EU and US, the addition of lidocaine hydrochloride to hyaluronic acid filler HARL, used for e.g. correction of nasolabial folds, provides substantial pain relief for patients during treatment. It is important to provide clinical data on the pain-relieving effect of adding lidocaine to HARL in Asian subjects as well.

Materials / method: This was a randomized, multi-center, evaluator-blinded, 2-week study conducted in Taiwan
Each subject received HARL with 3% lidocaine in one nasolabial fold and HARL without lidocaine in the other. Recommended maximum volume to achieve optimal correction was 2.0 mL for each side
Subjects assessed the pain experienced on a 100 mm VAS at the end of each injection and at 15, 30, 45, and 60 minutes after injection
Aesthetic improvement and nasolabial fold wrinkle severity was assessed
Pre-defined symptoms were recorded during 14 days after treatment
using a diary, and adverse events were collected

Results: Demographics and treatments
Effectiveness on Visual Analogue Scale, Wrinkle Severity Rating Scale, Global Aesthetic Improvement Scale
Safety on subject dairy entries of pre-defined, post-treatment symptoms and adverse events

Conclusion: HARL with lidocaine had a substantial effect on pain relief in Asian subjects undergoing correction of nasolabial folds, without affecting the safety profile, and with maintained improvement in nasolabial fold wrinkle severity and aesthetic appearance.

A novel full face rejuvenation strategy using Polycaprolactone (PCL)-based collagen stimulator to prevent facial overfilled syndrome(unrestricted educational grant from Sinclair)

Objectives: - The objective of this evaluation is to analyze a novel injection strategy of PCL-based collagen stimulator and assess the aesthetic improvement.
- The Less Is More (LIM) Full Face Rejuvenation Injection Strategy has been designed to provide natural, enhanced look and avoid high volume overcorrection (FOS)
- Clinical results of the LIM Full Face Rejuvenation Injection Strategy using the PCL-based collagen stimulator are demonstrated in three patients

Introduction: The significant demand for minimally facial rejuvenation procedures has led to an increasing number of patients with facial overfilled syndrome, a condition associated with incorrect placement and/or overzealous use of hyaluronic acid volume-based injection dermal fillers. The author (Dr Lim Ting Song) has developed a novel injection technique and strategy using PCL-based collagen stimulator to avoid FOS in patients seeking facial rejuvenation.

Materials / method: - PCL-based collagen stimulator is available in a range of three products, with effects lasting for 1, 2 or 3 years* allowing for treatment selection according to specific cosmetic or reconstructive requirements
- The filler is presented in a 1 ml sterile syringe (2 syringes per package) and is injected in subdermal or supraperiosteal planes using a 0.1 to 0.3 ml volume per injection area
- Each patient is injected with 1-2 ml per procedure and reassessed every 3 to 6 months
- Patient selection criteria include age, gender, and level of facial aging (mild/moderate/severe)

Results: 3 cases study on before and immediately/one year after treatment.

Conclusion: - Dermal fillers are useful tools for facial rejuvenation as they are minimally invasive, effective, and well tolerated while providing immediate and sustained effects
- Importantly, choice of product and expert injection technique are crucial to avoid FOS, often caused by multiple treatments with VBI techniques using dermal fillers
- The LIM Full Face Rejuvenation Injection Strategy using the PCL-based collagen stimulator provides effective facial rejuvenation with a natural and subtle enhancement through a careful injection strategy and sustained collagen stimulation

Pustular penile pyoderma gangrenosum: uncommon presentation case report

Objectives: Pyoderma gangrenosum (PG) is a rare, chronic, often destructive, inflammatory skin disease. It is characterized by a painful nodule or pustule which breaks down to form a progressively enlarging ulcer with a raised, tender, undermined border. It can occur by itself or with an underlying systemic disorder. PG most commonly occurs on the lower extremities and trunk of adults aged 40-60 years. Penile PG and pustular presentation are more uncommon and can mislead the diagnosis. By this case we want to remind this clinical presentation to doctors.

Introduction: Penile pyoderma gangrenosum (PPG) is non-infectious and idiopathic skin disease commonly associated with systemic diseases such as inflammatory bowel disease, rheumatoid arthritis and hematological malignancies Therapeutic options include topical or systemic corticosteroids or other systemically administered immunomodulatory drugs. We described a pustular PPG that was difficult to diagnose.

Materials / method: A healthy 60 year-old morrocan man was referred with an erythematous vesiculo-pustular and erosive, sharply demarcated lesions , located on the glans penis .The lesions have developed 2 years ago. The patient denied extramarital sex during the preceding years. Further physical examination was unremarkable. The lesions were unresponsive to topical antibiotics. The biopsy showed features of pyoderma gangrenosum and tissue cultures were negative for bacteria, mycobacteria and fungus. Full blood screening revealed normal.

Results: Four clinical variants of pyoderma gangrenosum have been described: ulcerative, bullous,vegetative and pustular.The last is a relatively uncommon variant showing vesiculo-pustular lesions that do not ulcerate and it is most commonly observed in patients with active colitis ulcerosa . Only 15 pustular PPG cases were reported some, were associated with hyperglycaemia and hyperlipidaemia.Topical tacrolimus therapy is effective and it is recommended as first-line modality. Surgical debridement is not recommended in the acute stage due to risk of tissue progression.

Conclusion: This case is reported for its extreme rarity. There are only very few reports of penile pustular PG. Highlighting this clinical presentation may help to early diagnosis.
