Objectives: The Dermal Orbicular Pennant (DOP) flap, first described by Harold Edgerton as an alternative to canthoplasty,nvolved using a de-epithelialized dermal pennant of lateral canthal tissue, offers lower eyelid support without the need for commissuroplasty. In cases where the DOP alone does not provide sufficient correction, it can be converted into a lateral tarsal strip, thereby ensuring adequate stabilization of the lid.” Glenn W. Jelks, together with Paul M. Glat, Elizabeth B. Jelks, etc., contributed to a paper in 1997 titled “Evolution of the lateral canthoplasty: techniques and indications” Conclusion: The Dermal Orbicular Pennant (DOP) is an effective and anatomically respectful technique for lateral lower-eyelid support, avoiding commissuroplasty while preserving natural canthal anatomy. It provides stable, vascularized support and allows a stepwise approach, as it can be converted into a lateral tarsal strip when additional correction is needed.
