Introduction: Blindness is a rare complication of dermal filler injections; however, it represents the most feared adverse event and is likely underreported, often resulting in devastating and irreversible outcomes. With the worldwide increase in aesthetic procedures performed daily, clinicians must remain vigilant and adequately prepared to recognize and manage this complication. Materials / method: I am going to present a case of filler-associated blindness encountered in an office-based setting. The case is analyzed to identify contributing factors, procedural errors, and missed opportunities for prevention. The discussion focuses on recommended filler volumes, pre-procedural risk assessment, patient counseling, informed consent requirements, early diagnosis, and evidence-based management and treatment protocols
