Objectives: Learn how blindness may present in the outpatient clinical setting through a case-based example and key learing points.
Introduction: Blindness is a rare complication of dermal filler injections; however, it represents the most feared adverse event and is likely underreported, often resulting in devastating and irreversible outcomes. With the worldwide increase in aesthetic procedures performed daily, clinicians must remain vigilant and adequately prepared to recognize and manage this complication. Materials / method: I am going to present a case of filler-associated blindness encountered in an office-based setting. The case is analyzed to identify contributing factors, procedural errors, and missed opportunities for prevention. The discussion focuses on recommended filler volumes, pre-procedural risk assessment, patient counseling, informed consent requirements, early diagnosis, and evidence-based management and treatment protocols
Results: Analysis of the case highlights critical failures that contributed to the adverse outcome, including procedural decision-making and preventive measures. The case demonstrates how prompt recognition, appropriate management, and adherence to established safety principles may influence outcomes. Key learning points are identified to improve clinician preparedness and reduce the risk of future events.
Conclusion: Although uncommon, filler-induced blindness carries profound and life-altering consequences. This case underscores the importance of thorough anatomical knowledge, meticulous technique, informed consent, and preparedness for emergency management. Learning from adverse outcomes is essential to enhance patient safety and minimize the risk of this catastrophic complication in future practice
