Objectives: After reminding the pathophysiological and histological definition of keloid scar, its predominance and its contributing factors, this presentation focused on surgical treatment and the prevention of keloid scars.
Introduction: Severe and extensive keloids often have only a surgical indication
Materials / method: Surgical indications are presented. Surgical techniques to excise and reduce the risk of relapse are described. Clinical cases, on African and Asian skin are presented. Several medical treatments and their indications are also remained.
Results: Treatments for thick or extensive keloids are often surgical. The prevalence of keloids in dark, African or Asian skin is higher than for Caucasian skin. Skin excision, local and loco-regional flaps, skin graft and dermal substitutes are used for keloid treatments. Occlusive and compressive dressing can help decrease the risk of hypertrophic scar formation and technique such as minimal tension scar, compression silicone sheet, silicone gel application, or injections. intra-incisional corticosteroids are explained.
Conclusion: The place of surgery in the treatment and prevention of keloids is essential. Severe and extensive keloids often have only a surgical indication.
