Objectives: This is a short course introducing the principles of ultrasound imaging of the skin, specifically the use of duplex mode for vascular structures in the face. This is very relevant for the safety of filler injections.
Introduction: Ultrasound imaging of the skin is relatively new. For this, high frequency probes (12-17 MHz) have to be used. With the duplex mode, vascular flow can be detected in relation to other structures, e.g. bone, muscle and fillers. Although the overall incidence of intravascular filler injections is very low, the impact of this complication is high. Knowledge on the use of duplex / ultrasound to identify the course of the major facial arteries to avoid these events, seems very logical. Since these devices are getting smaller and more affordable, barriers against every day use are being resolved.
Materials / method: At Erasmus University Medical Center we tend to do vascular mapping on all our patients before injecting fillers in a certain area.
Results: There are variations in the course of facial arteries. After facial surgery blood flow may be quite different and unexpected.
Conclusion: Duplex / ultrasound is a good addition to use for your patient's safety.
