Objectives: All these while, we have been practising volume based injection that is looking at improvements that based on volume of fillers used. However, these approach is not only unsafe, but can lead to many unwanted complications.

Introduction: Volume based injection is a primitive approach in using fillers based on the volume injected. The huge volume use is not only dangerous but also leads to many unwanted complications and outcome in the long run. We look at other alternatives that work.

Materials / method: We look at the possible complications of volume based injections and how we can avoid it.

Results: Volume based injection should be the practice of the past, and we should move on to better and safer techniques for natural outcome.

Conclusion: We should stop using big volume and stay away from volume based injection, and start to practice rheology based injection.

Objectives: I want to show the safe filler injection technique considering the vascular anatomy, filler Rheology, and other injection related dynamics.

and want to make a real guidelines based on the evidences over the experience and consensus.

Introduction: The knowledge about facial vascular anatomy is essential for safe filler injection procedure.
The knowledge is increasing, but the filler complication do not disappear.
Knowledge of the filler properties beyond the anatomical knowledge and knowledge of the filler dynamics according to the procedure of the practitioner is needed.

Materials / method: I tested various filler dynamics like injection pressure, ejection pressure, dynamics of filler when ejected.
I also checked the vascular anatomy and vascular dynamics as to the various drugs and our procedure

Results: viscoelasticity of the filler makes various unique dynamics of filler

and vessel dynamics are changed using the drugs like lidocaine, epinephrine etc.

Conclusion: for safe filler injection procedure,
An understanding of the vascular anatomy and it's variation, the understanding of the filler rheology, and practitioner's safe technique should be combined.
