Objectives: To learn how to treat cases of vascular complications with dermal fillers presenting with less characteristic clinical features
Introduction: Vascular complications with dermal fillers need to be promptly diagnosed and treated. In the era of "whatsup- and selfie-based" communication between patients and physicians, unpredicted bias can occur, delaying the diagnosis and treatment.
Materials / method: We present a case of ischemic event diagnosed 5 days after HA filler injection.
A day by day course including the treatment and the conclusions drawn, are presented and discussed, based on the published literature.
Conclusion: Conclusions drawn from the case presented:
1: Believe your patient. “Terrible pain” always means something is wrong, even if you do not see anything on the selfie
2: Always ask the patient to remove make up before taking selfie
3: Previous surgical interventions can increase the predisposition for arterial cannulation
4: Whatever anatomical explanation is, when you see ischemic event, treat it as ischemic event
5. Cannula is safer than a needle, but can penetrate artery especially in the scarred tissue
6. It is never too late for generous infiltration of hyaluronidase
