Objectives: Facial pores are the result of high sebum excretion, decreased elasticity
around pores, and increased hair follicle volume. Conventional treatment options such as topical retinoids, chemical peels, oral anti-androgens, and laser devices have shown mixed results, especially when treating large pores. In this study, the use of exosomes and Botulinum Toxin type A (BTXA) was explored to treat large facial pores. 
 

Materials / method: Subjects with large facial pores were selected for this study and treated in the following sequence. On day 1, the subjects were treated using air dissection and micro-droplet Botulinum Toxin type A. After one week, the exosome was applied to the treatment area using a meso roller. Two weeks later, another round of air dissection and micro-droplet BTXA injection was followed by the exosome application the following week. The subjects were followed up 3 weeks and 6 weeks after day 1 to assess the pore size, depth, and skin roughness using QuantifiCare 3D Lifeviz.

Results: Improvements in pore size and depth, especially for small and medium-sized
pores were recorded during the first follow-up on week 3. After 6 weeks, significant
improvements to pore size and depth were recorded, including large pores. Assessment using QuantifiCare 3D Lifeviz showed improvements in the subject’s skin roughness throughout the follow-up sessions.

Conclusion: Conventional treatments for facial pores have shown mixed results when
used for the treatment of large facial pores. In this study, the use of exosomes and
Botulinum Toxin type A has been demonstrated to be effective in the treatment of large facial pores.

Objectives: 1: Roles of botulinum toxin A for trapezius contouring and trapezius myalgia.
2: An updating injection land mark for trapezius contouring with Botulinum toxin A.
3.Different therapeutic outcome between Incobotulinumtoxin A and Onabotulinumtoxin a for trapezius contouring and trapezius myalgia.

Introduction: Trapezius myalgia and trapezius hypertrophy are conditions usually occurring together among middle-aged women.
Trapezius myalgia is characterized by symptoms such as stiffness, pain, and tightness of the upper trapezius muscle.
In addition, trapezius myalgia could be definitely diagnosed when muscle tightness, neck pain, and trigger point are presented .This clinical study aimed at evaluating the therapeutic equivalence in the efficacy of Incobotulinum toxin A compared with Onabotulinum toxin A in the treatment of trapezius hypertrophy contouring and trapezius myalgia.

Materials / method: Twenty volunteers with trapezius hypertrophy and trapezius myalgia were randomly injected with 30 units of Incobotulinum toxin A and Onabotulinum toxin A in each trapezius muscle guided by ultrasound with a randomized, double-blind, split-shoulder manner. All patients were treated with Botulinum toxin A in one session, with two-month and one-year follow-up period. And the trapezius thickness were record with ultrasonography for further comparison as well as pain score for trapezius myalgia.

Results: The trapezius thickness among those receiving treatment with Onabotulinum toxin A and Incobotulinum toxin A , Compared with the muscle size from day 60 to one year, the size of the trapezius muscle that had been treated by Onabotulinum toxin type A regained a significantly larger size compared with that treated by Incobotulinum toxin A (P = 0.027).On comparing the pain score from day 60 to day 0, it was observed that the pain scores of trapezius myalgia treated by Onabotulinum toxin A and Incobotulinum toxin A significantly differed (P = 0.003).

Conclusion: Incobotulinum toxin A had the same efficacy but a longer lasting effect for the trapezius size contouring and a higher efficacy for trapezius myalgia treatment compared with Onabotulinum toxin A.
