Objectives: Melasma is one of the common pigmentary disorder in Asian skin. They are difficult to treat and manage. Some patients are responding very well some are not. There are many conventional treatment such as bleaching cream, chemical peeling and Energy based devices (EDBs).

Introduction: Concept of EDBs whitening was developed from laser resurfacing to laser toning to fractional mode of Q-switched Laser , Picosecond laser, Fractional RF microneedle and vascular laser. Melasma primarily was treated for hyperfunctioning melanocytes. There is vascular involvement in some cases called vascular melasma.

Materials / method: We performed 12 cases of recalcitrant melasma with Pico 1064 at 0.6-0.8 J at 7 mm spot size then use FractionalRF microneedle at 1.0mm depth. After 3 treatment with 2-4 weeks apart we found 40-60% improvement of MASI score.

Results: The etiology of melasma caused by hyperfunctioning of melanocytes. There are three main characteristics of the dermal involvement in melasma patients include impairment of dermal ECM, increased vascularity, and the increased activity of mast cell. Combination treatment of various EDBs can help improvement of melasma. Pulsed dye laser with 585 nm wavelength has the effect on vascularity component of the melasma. We can treat the melasma with redness by using 585 nm together with 755 or 1064 nm wavelength.

Conclusion: Various kinds of Energy based devices can be a good alternative for the combination treatment for the melasma.

Objectives: Efficacy of a plant based extract (Fernblock®) as a photoprotective adjuvant in the oral and topical treatment of melasma

Introduction: Melasma is common pigmentary disorder that occurs in sun exposure skin areas. Although its etiology is multifactorial, solar radiation is the most important trigger factor, making your treatment a challenge. All of melasma treatments are based on the use of topical depigmenting compounds or different procedures and broad spectrum sunscreens.

Materials / method: Polypodium leucotomos (PL) is a fern native from Central and South America. Fernblock® an aqueous extract of PL, is a potent antioxidant ingredient with studies that demonstrate its photo and immunoprotective activities against Visible light (VL), Ultraviolet (UVR) and Infrared radiations (IR). Several studies have found that Fernblock® can significantly reduce the severity of sunburn, decrease the risk of UVR induced skin cancer, prevent skin aging and have beneficial effects in pigmentary disorders, like melasma.

Results: Our results demonstrate that Fernblock® is a safe adjuvant treatment that significantly improves and accelerates the outcome reached with hydroquinone and sunscreen.
