Microneedling of irradiated human amniotic collagenous matrix for skin rejuvenation

Objectives: Assess the efficacy of irradiated amniotic collagen matrix (IACM) in comparison with PRP in facial rejuvenation.

Introduction: Many modalities are used for the treatment of facial wrinkles, such as micro-needling that enhances new collagen production, and platelet-rich plasma (PRP) which contains concentrated levels of platelets and growth factors. The human amniotic membrane isolated from the placentae of donors (during elective cesarean sections) has high levels of growth factors that help in skin rejuvenation by improving the proliferation and migration of dermal fibroblasts and epidermal keratinocytes as well as increased collagen synthesis.

Materials / method: The present study included 20 patients with facial wrinkles divided into 2 groups using the split-face technique: Group A subjected to micro-needling with topical IACM on the right side of the face. Group B was subjected to micro-needling with topical PRP on the left side of the face. Patients received 6 sessions 2 weeks apart. Photos by an Antera camera and skin biopsy were taken to evaluate the clinical results.

Results: There was a statistically significant improvement in patients treated with IACM more than those treated with PRP using micro-needling in both sides as proved clinically, pathologically, and by an Antera camera.

Conclusion: Micro-needling using IACM could be considered an effective and safe method for facial rejuvenation.

Skin exposomes: can cosmeceuticals counteract their effect?

Objectives: To shed light on definition of exposomes .
How they affect the skin ?
How and if cosmeceuticals can help to counteract their effect .

Introduction: Skin exposomes as UVR
Visible light and pollution contribute in skin ageing.
Topical cosmeceuticals are usually the first lind sought for by both the patient and the dermatologist .

Materials / method: Skin exposomes and how they can cause skin ageing is explained .
Anti-aging cosmeceuticals their active ingredients and vehicles and how they can counteract the effect of these on extrinsic ageing will be elaborated.

Results: Anti aging cosmeceuticals do have a Role in protecting the skin from exposomes
They can help in improving signs of ageing

Conclusion: Future studies of the micro environments around the skin can help reach a skin care which help reduce and delay the appearance of signs of external aging of the skin

Vitamin serums as a skincare routine - Truth or myth?

Objectives: 1. List the various types of topical vitamin serums used as skincare routine.
2. Find out the benefit of applying such products on our skin, if there's any.
3. Giving scientific proofs on benefits or harmful use of these serums.
4. Discuss the role of us dermatologists in educating our patients and sharing our knowledge.
5. Encourage doctors to be more scientific in prescribing skincare products to patients.

Introduction: Nowadays, topical vitamin serums are trending hugely and became a must have in any skincare routine.
But do they really benefit our skin?

Materials / method: I used an inductive approach, by searching the literature for published, evidence based studies that researched the use of various types of topical vitamin serums. To find out the "proved" medical benefits of these serums on our skin.

Results: -Vitamin serums are better used pure forms, not esterified.
-Better in combination.
-Vitamin A & C are most effective in specific forms.
-Other topical serums are promising like Vitamin B (niacinamide), hyaluronic acid & Q 10, but more studies are needed.

Conclusion: Doctors should familiarize themselves with the best forms of topical vitamin serums for the optimum benefit of patients and satisfactory results. They have a moral role not to follow trends and market advertisements that are not based on real scientific proof.

Preliminary results from an investigational post-shave cream

Objectives: This research is a prospective case study which was conducted to assess consumer acceptance and tolerability of a post-shave cream when used by healthy women and men immediately after shaving a diversity of body parts and over the course of four weeks.

Introduction: Pubic and hair removal has often been described as the “new norm”. Surveys estimate that approximately 80% of men and women reported that they have removed some or all of their pubic hair. Multiple methods of removal of unwanted hair but may affect skin health and barrier integrity. Little is known on post-shave skin recovery and health. A novel post-shave cream product was developed to address skin health for all body parts. The product was designed and formulated with specific ingredients that are compatible with the sensitive skin in the perineum and genital area.

Materials / method: A post-shave cream was formulated as a reverse emulsion containing emollients, skin protectants, moisturizing ingredients, and contains technology to allow for immediate and prolonged skin benefits (soothing, moisturization). A consumer in-use case study was performed. 20 subjects, 17-65 years old, with no active untreated medical conditions were recruited. Subjects applied the post-shave cream to the body area immediately after shaving and once daily for 4 weeks. Self-assessment questions were completed. A Pearson’s chi-squared test was performed. All subjects responded to all questions.

Results: Males (n=10) applied the post-shave cream to their face, penile shaft, chest, scrotum, buttocks, legs, and head. Females (n=10) subject applied the post-shave cream to their legs/arms, underarms, mons pubis, and vulva. Self-assessment results indicated highly statistically (***p < 0.001) favorable responses for each category: product characteristics, skin feel, and overall satisfaction. The post-shave cream helped soothe the skin, reduce the appearance of redness and improve skin health. The tolerability evaluation showed that the post-shave cream yielded no adverse events.

Conclusion: This single-center consumer in-use case study supports the hypothesis that the post-shave cream when used immediately after post-shaving and for continuous use up to four weeks achieved consumer acceptance and was tolerable. Consumer acceptance was demonstrated in all self-assessment categories, product characteristics, skin perception, and overall satisfaction.

Glutathione for skin lightening: a regnant myth or fact?

Objectives: The Skin lightening in this part of the world is an absolute reality and the market is flooded with several compositions containing so called ‘super skin lightening’ glutathione. But is this just a fad or can we clinically back this molecule? Through this talk, I would like to share ‘facts’ about this antioxidant and also deliberate on its properties, formulations and effectiveness.

Introduction: I do use it in my practice and would like to share the techniques to determine the dosage for a given patient. Will also discuss the safety protocol and clinical results.

Materials / method: Will be discussing about various formulations and cofactors that go about with this treatment and discuss various case studies of my patient population to bring out the nuances that I believe one should take note of in one's practice.

Results: Though Glutathione still continues to be a controversial therapy, we cannot ignore the demand amongst our patients, especially in Asia.

Conclusion: I would say that Glutathione if administered in controlled manner and dosage can provide the results our patients want. It is important to not get influenced by patients demand, one has to be clinically aware of its abuse and administer appropriately.

Restoring healthy beautiful skin from the inside, a functional and preventative medicine perspective

Results: -There is plenty of evidence that an anti-inflammatory diet helps with ageing, skin restoration and the reduction of inflammation in inflammatory conditions like Eczema, hence treating it from the inside.
-There is evidence that allergic skin conditions can be controlled form the inside using preventative lifestyle modifications as discussed in the 6 R protocol

Conclusion: It’s important that all Aesthetic doctors or anyone treating skin is aware that it can be optimised from the inside. Challenging conditions such as Eczema not getting better warrant a trial of lifestyle modifications.

All that glitters : A youthful glowing skin - Beyond just lifting !

Objectives: Bio Revitalization & Bio Rejuvenation of skin is the most sought after skin aesthetic treatment even after one has gone through a face lift surgery. The simple transdermal drug delivery system introduced by Michel Pistor stays as safest & effective tool to achieve a glowing skin. Data has shown a dramatic increase in the demand for noninvasive rejuvenation procedures(a 228% increase in non invasive procedures seen from 2000-2018).Though EBD like Hifu has gained a great deal of popularity for skin lift. The synergistic effect of cosmeceuticals cant be denied to get the optimal aesthetic outcome

Introduction: Aging of face is a complex, multifactorial process involving all layers of skin,sub/cut layer, SMAS,fat pads & bone.These days using threads,EBD,Injectables etc we try to achieve optimal aesthetic outcome. But we neglect the importance of a supple, soft, spotless &well hydrated skin. The Magnificent-7,as explained by Arthur Swift includes a clear well hydrated skin as an essential beauty parameter. By using peptides, vit B3, vit C,Hyaluronic acid & Transxemic acid with Microneedles/ mesotherapy we work at cellular level to improve hydration,texture &quality of skin to achieve luminosity.

Materials / method: 40 female Asian patients aged between 30-55 years were divided into group 1 & 2 as HIFU alone & HIFU combined with cosmeceuticals respectively. Both were assessed on Glogau & Fitzpatrick wrinkle scale before/after session. Both groups were first treated with HIFU using 1.5,3.0 & 4.5mm transducers. After15 days , Group 2 started a series of 4 mesotherapy sessions with a cocktail of non-cross linked HA,vit B3, vit C, biopeptides & retinol every 15 days.All were photographed before/after at day 0,15,30,60& 90. Everyone completed sessions with no side effects except mild erythema.

Results: All females in both groups completed their session.13 out of 20 females in group1 were satisfied and showed good response in before /after photos.10 showed improvement on Glogau & Fitzwrinkle assessment scale with satisfaction of 60 % .All of them wished to take another session.Others showed 35% improvement on wrinkle scale . Majority of the females in group 2 were satisfied and showed remarkable improvement on Before/after photos .16 showed a great result on Glogau & Fitzpatrick wrinkle assessement scale with 75 -80 % satisfaction.

Conclusion: Synergistic combinations of multiple aesthetic modalities enhance the efficacy of individual ones. Cosmeceuticals & biostimulators optimise the effects of EBD/ HIFU & other aesthetic procedures.We can achieve more natural results when these cosmeceutics are used on regular basis as an adjunct therapy to avoid over treatment. HIFU can tighten and strengthen retaining ligaments &SMAS by giving thermal injury. The cosmeceuticals delivered to epidermis& dermis work at cellular level to undo the effect of free radical & solar damage to elastic fibers.They bring the luminous, age defying skin back.
