Objectives: this study aims to assess the outcomes and safety of the Guided Superficial Enhanced Fluid Fat Injection (SEFFI) technique for facial rejuvenation. Through a retrospective analysis of cases from multiple Italian centers, the study seeks to evaluate the efficacy of guided SEFFI in enhancing facial aesthetics, improving skin quality, and ensuring patient satisfaction. This investigation contributes valuable insights into the applicability and benefits of SEFFI

Introduction: Clinically, aging manifests as fine lines and wrinkles, a loss of elasticity, dyschromia, epidermal thinning, and increased coarseness . The volumetric loss of contours and angularity in the face are also visible due to tissue atrophy within the deep layers of the skin. Regenerative therapy exploits the properties of the cells of the stromal vascular fraction (SVF) of the adipose tissue to counteract these aging-associated changes. In this study, we present data retrospectively collected by ten physicians who carried out more than 2300 guided SEFFI procedures in various anatomic face area

Materials / method: This study is a retrospective analysis of data obtained from 2365 healthy patients, whom 2123 were females aged, on average, 49.2 years old (range: 38–65 years), and 242 were males aged, on average, 47.0 years old (range: 41–53 years). Patients were treated in 10 different professional centers for aesthetic physicians and surgeons, with the guided SEFFI technique. The follow-up of the cases was between 4 and 6 months depending on the center.

Results: guided SEFFI was used alone in more than 60% of the patients and in all facial areas. In about one-tenth of the patients, guided SEFFI was combined with a botulin toxin treatment or hyaluronic acid filling. Other procedures were used more rarely. Ecchymosis in the donor or injection sites was the most frequent adverse event but was only observed in 14.2% and 38.6% of the patients, respectively.

Conclusion: The guided SEFFI technique is standardized and minimally invasive, leading to very few complications. It constitutes a promising antiaging medical treatment that combines effectiveness, safety, and simplicity.

Objectives: The objective of this study was to identify the secretory proteins of human dermal fibroblasts using proteomic analysis. We also aimed to bioinformatically analyze the functions and pathways of identified secretomes.

Introduction: Human dermal fibroblasts secrete numerous growth factors and proteins that have been suggested to promote wound repair and hair regeneration.

Materials / method: Human dermal fibroblast-conditioned medium (DFCM) was prepared, and proteomic analysis was performed. Secretory proteins in DFCM were identified using 1-dimensional sodium dodecyl sulphate-polyacrylamide gel electrophoresis, in-gel trypsin protein digestion, and quantitative liquid chromatography tandem mass spectrometry (LC-MS/MS). Identified proteins were analyzed using bioinformatic methods for the classification and evaluation of protein-protein interactions.

Results: Using LC-MS/MS, 337 proteins were identified in DFCM. Among them, 160 proteins were associated with wound repair, and 57 proteins were associated with hair regeneration. Protein-protein interaction network analysis of 160 DFCM proteins for wound repair at the highest confidence score (0.9) revealed that 110 proteins were grouped into seven distinctive interaction networks. Additionally, protein-protein interaction network analysis of 57 proteins for hair regeneration at the highest confidence score revealed that 29 proteins were grouped into five distinctive interaction networks.

Conclusion: DFCM contains numerous secretory proteins that comprise groups of protein-protein interaction networks that regulate wound repair and hair regeneration.

Objectives: Oxidative stress-induced cellular senescence and mitochondrial dysfunction result in skin aging by increasing ECM levels-degrading proteins such as MMPs, and decreasing collagen synthesis. MMPs also destroy the basement membrane, which is involved in skin elasticity. We evaluated whether skin booster(contains various antioxidant) decreases oxidative stress and mitochondrial dysfunction, eventually increasing skin elasticity in aged animals. Skin booster(AA, NA, coenzymes, glutathione) and sodium hyaluronate improves skin rejuvenation by decreasing oxidative stress and mitochondrial dysfunction

Introduction: During the skin aging process, oxidative stress leads to the upregulation of nuclear
factor kappa-light-chain-enhancer of activated B cells (NF-kB) and its downstream signal pathway of activator protein 1 (AP-1), which eventually increases the levels of extracellular matrix (ECM)-degrading proteins such as collagenase and matrix metalloproteinases (MMPs). We hypothesized that Skin booster(AA,NA) injection could decrease oxidative stress and cellular senescence, which eventually resulted in a reduction in NF-kB/AP-1 and mitochondrial dysfunction in the skin.

Materials / method: To determine whether aged animal skin was affected by Skin booster(AA.NA) treatment, 12-month-old aging mice were injected intradermally with RA (100 L/cm2/day) twice every two weeks using a microneedle therapy system (MTS; Derma-Q Gold 0.5 mm, DONGBANG medicare, Seongnam, Republic of Korea). The control was injected with distilled water under the same conditions.To confirm whether the skin elasticity of aged animal skin was changed by skin booster treatment, the skin elasticity of the animal before skin booster(AA,NA) treatment and after 4 weeks was measured.

Results: 1.Skin booster decreased oxidative stress in H2O2-Treated Keratinocytes and Aged Animal Skin.
2. Skin booster decreased Mitochondrial Dysfunction and Cellular Senescence in H2O2-Treated Keratinocytes and Aged Skin.
3. Skin booster decreased NF-Kb/AP-1 and MMP1/2/3/9 Expression in Aged Skin.
4. Skin booster increased the Expression of Laminin and Nidogen and the BM Density.
5. Skin booster upregulated the Expression of TGF-B, CTGF, and a-Smooth Muscle Actin (A-SMA) and Collagen Fiber Accumulation in Aged Skin.

Conclusion: Our study showed that skin booster(AA,NA) decreased oxidative stress and mtDNA injury, which eventually decreased mitochondrial dysfunction in aged skin. RA also decreased ECM destruction by decreasing the levels of NF-B/AP-1/MMPs and increasing the levels of BM proteins such as nidogen and laminin. RA enhanced the collagen synthesis-related signaling pathway of TGF- and CTGF. These modulations associated with RA treatment led to increased collagen fiber accumulation and skin elasticity in aged skin.

Objectives: Since the birth of regenerative medicine, different types of cell transplantation techniques have become important treatments in medicine. Micrograft treatments have become more popular in the past 5 years because of their ease of use and effectiveness. Micrograft treatments are mostly accomplished by processing skin tissues to get progenitor fibroblast and melanocyte cells. Currently, many groups have switched to the mechanical isolation methods of fibroblast instead of lab-grown fibroblast cells.

Introduction: Various types of skin rejuvenation techniques have been introduced in the past 10-15 years and the success of these techniques depends on their capacity to stimulate cellular regeneration. Fibroblast cells are presented as the building block of the skin. Our main focus is to separate progenitor fibroblast cells mostly placed in the bulge area of the hair follicle. The methods used to require GMP Laboratories to get Fibroblast cells. Due to practical and cellular restrictions, many groups have turned to the use of mechanical/bedside methods to isolate fibroblast treatments.

Materials / method: Skin tissue harvesting was done by using the biopsy punch in this technique. For suspending the cells, PRP was used. Emulsification of skin tissue is a hard process without damaging the cells which is why a liquid is needed to suspend cells. Harvested samples were manually processed with special blades along with the PRP. Unprocessable skin remnants with Extra Cellular Matrix were filtrated (Filtered) through 150-micron filter for the purpose of safe injection. The end product was injected in order to provide dermal and superficial regeneration of the skin.

Results: The final injectable cell suspension was injected into the patients. Most of the patients received scalp and skin injections. Within 3 months of follow-up, high rates of rejuvenation and hair growth were observed in all patients significantly. The micrograft portion was analyzed using fluorescence microscope in order to check for the nucleated cell counts along with their viability data. The viability of the final solution was around 95% on experimental points. In this study, no side effects or adverse reactions were observed/reported.

Conclusion: Micrograft treatments are great alternatives to many conventional techniques. Also, the preparation of progenitor fibroblast-rich cell solution without sending samples to the GMP laboratories gives rapid operation and ensures cells to not lose their capacity for regeneration. Further studies are needed for long-lasting effects shall be followed up to conclude the superiority of this technique. Yet, these findings and clinical outcomes have shown Micrografting technique is a great and promising tool to prepare a fibroblast-enriched solution in safe conditions.

Objectives: To assess the effectiveness of intradermal injection of exosomes for the correction of static rhytides.

Introduction: Treatment of static rhytides mainly relies on the improvement of age-related loss of skin elasticity. Exosome treatment is an emerging intervention that mitigates human dermal fibroblast senescence, directly involved in the process of skin regeneration as transporter of bioactive molecules to the recipient cells.

Materials / method: A 40-year-old patient with static glabellar rhytides was administrated with a single dose of exosomes intradermally under topical anesthesia on 2 points of the bilateral glabellar furrows.

Results: Patient was seen with clinically appreciable improvement one month post-procedure, with decreased depth of dermal atrophy and improvement in skin elasticity.

Conclusion: Exosome therapy holds much promise as a valuable intervention for biostimulation and skin rejuvenation. Its bioactive and physiologic properties deserve to be studied more extensively clinical trials.

Objectives: Polydeoxyribonucleotides (PDRNs) are naturally occurring polymers comprised of deoxyribonucleotides linked by phosphodiester bonds. PDRNs derived from various sources, including salmon, have gained significant attention in aesthetics due to their potential regenerative and anti-aging properties.
This paper aims to explore the utilization of salmon-derived PDRNs in aesthetic medicine, focusing on their mechanisms of action and specific applications in skin rejuvenation.

Introduction: PDRNs are a type of nucleic acid composed of deoxyribonucleotide monomers, which are the building blocks of DNA.
PDRNs extracted from salmon generally consist of 10 to 50 nucleotides and their the specific functional properties are still being studied, but they have been found to have various biological activities, such as immune modulation, anti-inflammatory effects, and promotion of cell regeneration. These properties make PDRNs potential candidates for an effective therapeutic applications in medicine and skincare.

Materials / method: PDRNs promote the growth and activity of fibroblasts. Approximately 54,55 fibroblasts are located in the dermal layer, suggesting that PDRNs must be administered directly into this layer to obtain maximum benefits. In this case, some applications are:
a)Facial rejuvenation: Use of salmon PDRNs in dermal fillers, mesotherapy, and microneedling
b)Hair restoration: To promote hair growth and preventing hair loss
c)Wound healing: Acceleration of tissue repair and scar reduction

Results: PDRNs derived from salmon DNA have been shown to have various biological properties such as anti-inflammatory, wound healing, and tissue regeneration effects and may continue to be explored for their potential applications in aesthetic medicine.
Some side effects have been observed like allergic reactions, hypersensitivity, as swelling, redness, pain, or bruising at the injection site. However, these side effects are generally temporary and resolve within a few days.
It is important to note that the side effects mentioned above are not exhaustive, and individual responses may vary.

Conclusion: Salmon-derived PDRNs have demonstrated a generally good safety profile in aesthetic use.
However, it is crucial for healthcare professionals to conduct thorough evaluations and follow established guidelines to ensure patient safety and minimize potential risks and get good results.

Objectives: This case studies not only has proven there is much more tolerable treatment available in the market particularly for this rare autoimmune condition. Not only it's effective, with no adverse effects reported up to date, and most importantly easy to use and showed favourable response within such a short period of time. With the presence of this treatment, patients would not need to be on topical steroid and cytotoxic chemotherapy based treatments for long period of time which could potentially caused secondary inflammation as steroid induced inflammatory skin reactions.

Introduction: Exosomes have gained significant attention in the field of medicine in recent years, and they hold great promise for various applications in cosmetic and medicine. However, the effectiveness in treating severe inflammatory skin conditions in Dermatomyositis has not been reported yet.

Materials / method: In this case study, we are looking at the response of one of the rare autoimmune diseases Dermatomyositis towards PSCD (Plant Stem Cells Derived) Exosome as a stand alone treatment for inflammatory skin conditions. Even more interesting given that the patient found to be Positive with Mi-2 Beta antibody on myositis 11 blood panel antibody testing. Which means the risk of developing malignancy almost 5 folds compared to general populations.

Results: Therapeutically the patient was treated with topical cream high in exosome concentrated for 6 weeks twice a day despite marked improvement seen in just after 2 weeks. This is to avoid and ensure no relapses. Patient continues to improved over the 6 weeks course and subsequently maintained with preparation with lower exosome concentration.

Conclusion: This is the first case demonstrating the effectiveness of topical exosome preparation in treating severe inflammatory skin conditions in a rare autoimmune disease, Dermatomyositis.
Exosome based treatment should be considered as 1st line treatment of choice based on results, tolerability and nil side effects reported up top to date compared to existing treatments. Hence, increases compliance and patient's QoL.

Objectives: Skin Pigmentation or hyperpigmentation is a common skin concern that affects patients of all skin types. Apart from a frequent aesthetic concern, facial hyperpigmentation negatively impacts self-perceptions and social/emotional functioning, resulting in increased social anxiety and disability.
Exosomes are extracellular nanovesicles produced by cells, that are known to own anti-inflammatory and anti-pigmentary properties. Rose stem cell derived exosomes are a novel therapeutic option for facial pigmentation with significant clinical evidence to improv pigmentation and skin health

Introduction: Facial pigmentation is multifactorial and is related to a variety of factors including sun exposure, hormonal imbalances, genetics, skin inflammation, medications, and systemic disease. Rose Stem cells derived exosomes (RSCE) are a novel cosmeceutical treatment approved for topical application on the skin, with significant in vitro data to show improvement of skin thickness, pigmentation and texture. We hypothesized that RSCE can potentially improve skin pigmentary disorders, and therefore ameliorate

Materials / method: 12 female participants were recruited, with skin type ranging between 2-4 (mean age 46.64+\-13,05). Patients were diagnosed with facial pigmentation (5 patients with melasma, 3 with age spots and 4 with post-inflammatory hyperpigmentation). The treatment protocol included 3 sessions of microneedling, with topical application of 5ml of RSCE, spaced 3 weeks apart. Clinical photography and imaging analysis, for skin pigmentation, erythema and wrinkles were documented at baseline (V0) and at the end of the study(4 weeks after the last treatment-V4). DLQI was completed by the patients at V0 and V4.

Results: There was a significant decreased in the % of photodamaged skinat V4(mean UV pigmentation% at V4=38,2+/-11,5) compared to V0 (mean UV pigmentation% at V0= 45,3+/-10,8; repeated ANOVA mean difference=8,901, P=0,12). The % of superficial dark spots significantly decreased at V4 (mean dark spots% at V4=33,4+/-7,4), compared to V0 (mean dark spots% at V0=39,8+/-8,9; P=0,000). The %of sensitive red skin markedly improved by 7.4%+/-0.05 (P=0,000). The %of wrinkles also significantly improved (P=0.13). A significant improvement in DLQI was noted at V4 (P=0,006)

Conclusion: According to our data, microneedling with a topical RSCE- based blend can improve skin pigmentation, inflammation, and texture while it's a safe and well-tolerated treatment. This is the first clinical study to demonstrate the efficacy of RSCE on aesthetic skin concerns and the associated psychosocial impact. Future studies are required to further elucidate the therapeutic role of RSCE on human skin.

Scars on the face not only result in aesthetic deformities but also impose functional limitations, affecting essential activities such as facial expressions, eating, and speaking. Severe cases may induce pain, itching, and substantially diminish quality of life. Only with the synthesis of proper examinations and imaging the plastic surgeon should start fat grafting the face or treating scars.

Introduction: Techniques like autologous fat grafting and injectable platelet-rich plasma (I-PRF) offer substantial contributions to facial scar remodeling, positively impacting both the structure of scar tissue and associated conditions. These approaches not only enhance aesthetics but also improve functionality. The most effective strategy for managing facial scars and tissue irregularities involves a comprehensive approach. Utilizing architectural lipofilling alongside specialized techniques such as the use of dissector cannulas, undermining, and liposhifting are paramount.

Materials / method: Microstructural fat grafting resolves volume loss in scar tissue, while nanofat grafting and I-PRF contribute to refining skin quality, texture, elasticity, and color.
To achieve state-of-the-art results, regenerative medicine should be integrated with other modalities known to have beneficial effects on facial scarring. Our protocol encompasses the use of silicone sheets or ointments, compression garments, laser therapy, and 5-Fluorouracil.

Results: In our clinical practice, we have successfully treated 176 patients spanning a wide age range from 1 to 85 years. Results, as assessed through both subjective and objective measures, including the Vancouver Scar Scale, have consistently demonstrated good to excellent outcomes. Patient satisfaction with cosmetic appearance has notably increased postoperatively. Regarding patient satisfaction with cosmetic appearance, our cases were evaluated. The patient satisfaction index regarding the face and their whole body increased considerably postoperatively.

Conclusion: Regenerative medicine combined with other modalities offers a safe, reliable way to achieve significant improvement in scar appearance (e.g., color), skin characteristics (e.g., texture, elasticity). Three-dimensional contour through the restoration of volume can be achieved with fat grafting.
The treatment combination has limited side effects.

Objectives: This presentation aims at providing a comprehensive overview of the evolving landscape of Plant-Derived Exosomes-like Nanoparticles and their transition to Plant Stem Cells-derived Exosomes. The audience will gain insights into the historical context of extracellular vesicles research, the current understanding of the unique properties of these nanoparticles, and potential future directions of their indications in the realm of aesthetic and regenerative medicine.

Introduction: In the context of the growing significance of exosomes in regenerative medicine, ethical and sustainability questioning from the public and the physicians’ community has raised, providing rationale for exploring plant-derived exosomes-like nanoparticles, their potential advantages over other sources and the need for a comprehensive understanding of their evolution to plant stem cells-derived exosomes. The relevance and implications of this evolutionary process in the field of aesthetic and regenerative medicine are also highlighted.

Materials / method: By conducting a comprehensive and systematic review of current research in the field of plant-derived exosomes, with criteria including good quality study designs, peer-reviewed publications and clinical trials, the evolution in this field has been traced and contemporary advancements compiled independently of any industry.
By reviewing extraction and characterization of plant-derived exosomes-like nanoparticles, as well as the cultivation and isolation of exosomes from plant stem cells, the properties and efficacy of the different methods were outlined.

Results: Plant-derived extracellular vesicles are evidenced to successfully mediate intercellular communication through naturally occurring bioactive components and exogenous bioengineered cargo, customised with biomimetic actives, proteins, peptides, amino-acids, growth factors, liposomes addressing specific indications. Efficacy on inflammation, pigmentation, skin regenerative functions, collagen and elastin synthesis, angiogenesis, wound healing, cutaneous immune responses to external environment are found across studies. These findings elucidate current state of knowledge on plant-derived exosomes

Conclusion: Bioactive nanovesicles released from plants show remarkable anti-inflammatory and regenerative properties. They comprise a variety of bioactive molecules, potential to deliver their nano-cargo to human skin inducing anti-inflammatory, antioxidant, and regenerative activity. Clinical challenges ahead offer perspective on evolution of plant stem cells-derived exosomes, their promising broader implications as alternative cell-free approach in regenerative medicine, aesthetic procedures but also in cutting-edge cosmeceuticals. Future research will surely propel the field of plant exosomes forward.
