Treatment of dimpling from cellulite. (Sponsored by LOVE COSMEDICAL)

Objectives: Cellulite can be seen on the skin in widespread alterations of the skin surface and dimpling. The purpose of this study was to assess the effectiveness and safety of the manual subcision technique to treat dimpling from cellulite, using a specific class IIA medical device (Celluerase)

Materials / method: The multi-center observational study assessed 200 women treated in a single session for different dimpling, using manual subcision administered by Celluerase. Aesthetic outcomes were evaluated by the authors, and the patients as- sessed satisfaction levels.

Results: wo hundred women between 20 and 55 years were treated. The medical evaluation of patients saw improvements with an average score of 8.1, whereas the subjective evaluation by patients gave an average improvement score of 7.8. Adverse events were reported.

Conclusion: The study has shown the effectiveness and safety of the manual subci- sion in the treatment of dimpling. The device used, designed specifically
for this technique, has shown itself to be very helpful and effective in terms of practical use, aesthetic outcome and safety, with various advantages compared with other commonly used devices.

Supraperiosteal Filling Technique for Full Face Rejuvenation. (Sponsored by LOVE COSMEDICAL)

Objectives: Modern theories of facial aging are based on the concept that bone structures play an important role in inducing the typical aged face. This study evaluated the efficacy and tolerance of a new gel, which was injected over the periosteum in a standardized pattern in order to reduce the typical aging defects due to absorption and distortion of the bony facial tissue, replacing the deep support of the overlying soft tissue.

Introduction: The study was a multicentric, non-controlled clinical trial. Patients were treated in a single session with a standardized infiltrative technique where small boluses was released over the periosteum in 6 specific sites of each half-face. One hundred-78 subjects completed the study. The mean total amount injected was 5.4 ml (3.6 - 6.8 ml) per face per session. Treatment was judged by the subjects as well tolerated and not very painful. Recovery time was immediate, with a minimum disconfort during the first 24 hours (edema, pain, numbness) and it did not limit social life or other activities.

Conclusion: The study demonstrated therapeutic success in 98.32% (evaluated by subjects) and 99.44% (evaluated by doctors) of the patients demonstrating the effectiveness of injection therapy. The medical device, employed with our specific standardized technique and with our range of dosage, was proved to be valid and safe.

Safety and Effectiveness of Poly-L-lactic acid (PLLA-SCA) for the Improvement in Appearance of Cellulite – A Pilot Study (Sponsored by GALDERMA)

Objectives: To evaluate effectiveness and safety of Sculptra poly L-lactic acid injectable implant (PLLA-SCA) for improvement in appearance of cellulite in posterior thighs.

Introduction: PLLA-SCA stimulates collagen production and is mainly used in the face, but there is increased interest to also treat non-facial indications. This abstract presents data from a 12-month, pilot study in women ≥18 years with Grade 2-4 cellulite severity on the Galderma Posterior Thigh Cellulite Scale (GTCS), i.e. superficial to moderately deep depressions with undulations, who were treated with PLLA-SCA.

Materials / method: PLLA-SCA, reconstituted in 17 mL sterile water+1 mL 2% lidocaine, was injected immediately after preparation in the posterior thighs (max. 54 mL/thigh) at 1-3 treatment sessions, 4-6 weeks apart. Assessments included aesthetic improvement in cellulite appearance on the Global Aesthetic Improvement Scale (GAIS), GTCS, skin laxity, subject satisfaction and adverse events. Primary endpoint was GAIS at Month 9 after first treatment, assessed by investigators. Time to return to daily activities after each treatment, and pre-defined injection-site responses were collected in a 28-day subject diary.

Results: At Month 9, 28/29 subjects (97%) had aesthetic improvement of cellulite appearance on both thighs (GAIS), and 93% improvement on the GTCS. Improvement rates were overall high on GAIS (≥93% Months 2-12) and GTCS (79-100% Months 6-12), with improved skin laxity in all subjects at Months 6-12 (investigator-assessed). Most subjects were satisfied with thigh appearance (97%) and reported improved skin sagginess (93%) and firmness (97%) at Month 12. No treatment-related adverse events occurred and diary-events were mainly tolerable and transient, but remained 2-4 weeks for 17% of subjects.

Conclusion: Treatment with PLLA-SCA safely improved the appearance of cellulite at Month 9 (primary endpoint) in this pilot study, and aesthetic improvement remained through Month 12. This was further supported by high GTCS improvement rates and subject satisfaction. Study limitations include a small sample size and no control group.

ReBorn 940nm IR Power LED Technological Breaktrough - A new source of energy for fat cells apoptosis (Sponsored by LIGHTFECTIVE)

Objectives: Body contouring, essential for aesthetics, traditionally relies on surgery, posing risks and recovery challenges. Non-invasive methods, including lasers, ultrasound, and radiofrequency, offer safer alternatives. Power LED 940 nm IR light using hyperthermic treatment, induces adipose apoptosis at 42-47°C, making it a promising technique for fat reduction.

Introduction: The Lightfective ReBorn system, utilizing 940 nm Power LED, delivers controlled energy for non-invasive fat reduction and body contouring. This case series aims to validate its ability to reach apoptosis temperatures safely.

Materials / method: Study Participants: Thirteen subjects (27-60 years,
females and males) with BMI ≤ 30 underwent single
ReBorn treatment. Exclusion criteria included large
visceral fat, skin laxity, and severe cellulite.
Study Procedure: Hyperthermic treatment lasted 35
minutes to all subjects, with LED energy gradually
increased for the first 10 minutes, the rest of 25
minutes were maintained with same cosistent energy.
Temperatures were recorded using a in-vivo
hypodermic thermocouple needle at a depth of 15 mm -
20 mm immediately after the treatment.

Results: Of the 35 applicator sites with
temperatures recorded, 82.86% (n=29)
reaching the desired temperature range
(mean: 44.91°C, median: 43.9°C). The
remaining 6 (17.14%) applicator sites did
not reach the desired temperature ranges.
It is expected that a delay in reading the
temperatures at these sites is responsible
for the reduced temperatures. No adverse
events were noted.

Conclusion: The ReBorn system, similar to 1060 nm lasers is able to reach the desired temperature range of 42-47oC, necessary to illicit adipose apoptosis during a 35- minute treatment session, with no recorded adverse events and within the patient’s comfort level.

CLINICAL EFFICACY OF A NOVEL DAILY FACE CREAM, WITH A PATENT-BASED ACTIVE, ON THE KEY SIGNS OF AGING (Sponsored by LABORATOIRES FILORGA PARIS)

Objectives: Skin is a protective and dynamic organ subject to different internal and external factors, including but not limited to chronobiological, hormonal variations,
lifestyle-related factors (environmental exposure, diet, etc.).

Introduction: These different factors are responsible for the accelerated deterioration of collagen and elastic
fibers, as well as an impact on skin barrier. These changes in the biomechanical properties appear on the skin's surface over time, leading to visible signs of
aging, such as wrinkles, dull complexion and loss of elasticity. Applying an anti-aging daily face cream can limit and reduce the aging induced damages. A
novel daily face cream has been developed which contains a patented repairing factor1. The efficacy was assessed in an in-vivo clinical study.

Materials / method: In the IRB-approved in-vivo clinical study², 36 Caucasian and Asian women aged from 43 to 65 of phototypes II-IV, were enrolled following the informed
consent process (U.S. Investigational Review Board, Inc, Miami, Florida). All subjects had face-wrinkles and mild to moderate skin slackness present. The daily
study face cream was applied twice a day to the entire face for 56 days. Efficacy was measured using bio-instrumentation (Cutometer®, Tewameter®,
Spectrophotopmeter-CM 700®) and image analysis (profilometry) after 7 days, 28 days and 56 days of application.

Results: Skin surface was quantitatively assessed by Primos 3D (Fig.1): a non
contact in vivo skin measurement device based on structured light
projection. In conjunction with a comprehensive 3-D measurement
and evaluation software, the sensor allows to evaluate skin
properties (i.e. wrinkle depth, skin roughness, etc.).
Compared to baseline in the clinical study, profilometry results show
a significant decrease of 9.6% on forehead (Fig.1A) and -13.3% on
crow’s feet (Fig.1B) wrinkles depth after 56 days of use.

Conclusion: The data from this clinical study shows global benefits of the daily
use of face cream on skin brightening/radiance, wrinkles,
smoothness, barrier, firmness and skin elasticity. This study
formulation presented a good skin tolerability on a variety of skin
types (normal, dry, oily, combination, sensitive). Furthermore, after
56 days of applications, 91.7% of subjects declared that they
preferred this cream to their usual cream.

A multi-center, retrospective, chart review to evaluate the safety of poly-L-lactic acid injectable implant when used in non-facial areas (Sponsored by GALDERMA)

Objectives: This retrospective chart review study evaluated real-world data on safety of PLLA-SCA when used in non-facial areas.

Introduction: PLLA-SCA is an injectable poly-L-lactic acid implant, which gradually stimulates collagen formation. PLLA-SCA received US-approval for facial aesthetic use in 2009.

Materials / method: Subjects treated in a non-facial area with ≥2 vials of PLLA-SCA in ≥2 treatment sessions were eligible. Medical charts from 9 US sites were reviewed to collect demographics, treatment details and treatment-related adverse events (AEs) based on treatment sessions performed in 2012-2022.

Results: Among the 498 subjects included, most subjects had 2 (45%) or 3 (27%) treatment sessions, using on average 4.0 vials/treatment session. Reconstitution volumes were generally ≥10 mL for all treatment areas. Most common treatment areas were buttocks (61% of subjects), hip dip (17%), and decolletage (9%). Most treatment-related AEs were injection-related and mild or moderate. The most common AE was injection site bruising (5.0%), and AEs were mainly reported in the buttocks (4.0%) or posterior thigh (1.0%). Nodule occurrence was low (1 subject, 0.2%).

Conclusion: Based on this study, PLLA-SCA was well tolerated in real-world use for treatment of non-facial areas.

This study revealed the 1 to 1 dilution but it is necessary to evaluate the effect of other dilutions as well. Animal Research Center of the Fourth Military Medical University (Xi’an, China) in
strict accordance with the NIH guidelines for the care and use of laboratory animals

20 subjects from the AF4 clinical trial were selected to be evaluated with high-resolution imaging techniques. All subjects were injected on the nasolabial fold with ART FILLER® LIPS (FILLMED, France).

The keratinocytes show an irrigular honeycomb pattern at baseline. However by
day 90 (3 months) after the injection of crosslinked HA even in immediate hypodermis, the structure of the keratinocytes becomes reggular and their borders are clearer with higher contrats. This may be due to CD44 receptors which are the specific receptors for HA and they are in commun with both
fibroblasts and keratinocytes. This result could explain the modification of the skin surface quality after the injection of HA fillers.

Conclusion: HA fillers based on TRI-HYAL® Technology have demonstrated proved effects on different layers of the skin from epidermis to hypodermis. Early injection of this HA filler could enhance skin quality by supporting all skin cells across different layers. This data should be supported with larger clinical trials.

Objectives: The objective of this study was to evaluate the anti-pigmentation efficacy of 2-MNG in preventing UV-induced pigmentation in phototype III skin

Introduction: Hyperpigmentation disorders are a frequent reason for dermatology consultations as they can impair quality of life.
The most frequent disorders are melasma, post-inflammatory hyperpigmentation and solar lentigos with increased
melanin content on top of other pathophysiologic features. The development of a key and safe ingredient to
manage facial skin tone heterogeneities is of great importance. The objective of this study was to evaluate the antipigmentation
efficacy of 2-MNG in preventing UV-induced pigmentation in phototype III skin.

Materials / method: Fourteen studies were conducted in women (between
20-50 years old) in Shanghai (China), Paris (France) and
Bucharest (Romania). For each study, 30 volunteers
phototype III were recruited. Several products were
applied (4 mg/cm2) on mini zones of the back, in a
randomized and blinded protocol, five days a week for
six weeks. Assessments were performed using Chromameter and
scales for pigmentation and erythema. All studies were
conducted in exactly same conditions and validated
with Ascorbic acid 7% standard pivot. A Bayesian

Results: (i) 2-MNG 0.5% showed a significant antipigmentation
efficacy versus vehicle.

Ascorbic acid 7% when compared to its vehicle.
(iiii) Meta-analysis showed that 2-MNG 0.5% provide
statistically superior anti-pigmentation performance
compared to all selected standards.

Conclusion: This new ingredient delivers a very high efficacy versus recognized active ingredients from the market. It becomes a
promising approach to develop dermo-cosmetic products targeting main hyperpigmentation concerns of women

Preclinical and Clinical Evaluation of ACTV8SKN Actuator on Enhancing Skin Renovation: Impact on Collagen and Elastin Expression and Skin Quality Parameters (Sponsored by CONTRAD SWISS SA)

Objectives: A double evaluation, in vitro and in vivo, of a hydrogel-dispersed mixture of human synthetic biomimetic peptides (ACTV8SKN™ Actuator) to modulate COL1A1 and ELN fibroblast gene expression and to modify skin appearance and texture, in comparison with a similar GFs-enriched product.

Introduction: Skin appearance and texture are strictly dependent to a proper biosynthesis of biomolecules by fibroblasts. Fibroblasts continuously renovate the matrix
surrounding the dermal cell population by orchestrating the synthesis of collagen and elastin. Hence, they also support structural integrity and elasticity to the skin. The biosynthetic machinery is activated mainly by some proteins, among which growth factors. Materials / method: In vivo
- Human skin fibroblasts (ATCC CRL-2522) were maintained in MEM supplemented with 10% FBS and antibiotics.
- Cells were seeded, incubated for 24h and serum-starved for 24h. Treatment groups: negative control (untreated), positive control (TGF- ), Actuator/competitor treatments (same concentrations tested).
- Post-treatment, RNA was extracted and subjected to RT-PCR for COL1A1 and ELN expression quantification.

In vivo
40 healthy volunteers were enrolled for the study. 20 used the Actuator twice a day for 7 days, and 20 used a competitive peptide-base product, twice a day for 7 days.

Results: In vitro
- Actuator showed a 4.78-fold increase in COL1A1 after 24h (p<0.05), exceeding the positive control, whereas the CP did not significantly up-modulate the COL1A1 expression (fig.1).
- Actuator stimulated a 2.94-fold increase in ELN at 24h (p<0.05). The CP did not significantly up-modulate the ELN expression (fig.2).

In vivo
- Hydration: increase by 16.36% with Actuator (p=0.0002) Vs 14.53% with CP (p<0.0001) (fig.3).

Conclusion: This study shows the ability of the Actuator to significantly enhance the expression of collagen and elastin in human fibroblasts, with a superior efficacy at 24h when juxtaposed with another GFs-enriched product, and in a not concentration-dependent manner (data on files). These pre-clinical data paired with the study on volunteers, where the elasticity, hydration and firmness significantly improved after one week more than with the use of the CP, with a remarkable reduction of wrinkles, particularly around the periorbital area. Skin Tissue Modification with two different types of needling and radiofrequency (Sponsored by CAMPOMATS)

Efficacious with minimal side effects : A novel treatment regimen for acne vulgaris using multifunctional ingredients. (Sponsored by ALUMIERMD)

Objectives: The primary objective was to evaluate the efficacy and tolerability of a non-prescription, active acne regimen in adult patients of all Fitzpatrick skin types with mild to moderate acne using both objective and subjective parameters. The secondary objective was to assess if this regimen would also mitigate post-acne sequelae and improve skin’s overall appearance.

Introduction: Acne is the most common skin condition and dermatologists are seeing a rise in the number of adult acne patients seeking treatment. There are numerous options available over the counter, although many formulations have tolerability issues and lack the cosmetic elegance desired by adult patients.

Materials / method: 35 male and female subjects ages 24-55 years with all Fitzpatrick skin types were enrolled in this single-site 8-week study to evaluate the effect of an acne treatment regimen that included a clarifying cleanser, balancing serum, and broad-spectrum sunscreen, which were formulated with unique and synergistic ingredients to mitigate active acne, restore barrier function, and reduce sequelae including PIH (post-inflammatory hyperpigmentation) and PIE (post-inflammatory erythema). Ph III efficacy and safety data on relabotulinumtoxinA, an investigational liquid botulinum toxin for treatment of glabellar or lateral canthal lines (Sponsored by GALDERMA)

Objectives: To present results of two 6-month, Phase III clinical studies investigating relabotulinumtoxinA (relaBoNT-A) treatment of moderate-to-severe lines versus placebo. The READY-1 study investigated glabellar lines (GLs) (NCT04249583) and the READY-2 study lateral canthal lines (LCLs) (NCT04249687). Both studies evaluated efficacy and safety of treatment, and the primary endpoint for the EU analysis was responder rate at Month 1, defined as achievement of none-or-mild line severity on the investigator live assessment (ILA) scale for GLs at maximum frown or LCLs at maximum smile, respectively.

Introduction: RelabotulinumtoxinA (relaBoNT-A) is an investigational, innovative, complex-free, ready-to-use liquid botulinum toxin A that is manufactured from a proprietary C. botulinum strain and is free from human- or animal-derived components. The PEARL™ manufacturing process has no freeze-drying or precipitation steps, which protects and preserves the core neurotoxin in its natural conformation in liquid state throughout manufacturing, to maintain a high activity.

Materials / method: The studies were double-blind and placebo-controlled, with subjects randomized 3:1 to relaBoNT-A or placebo treatment. Assessments included line severity of GLs or LCLs by investigators and subject live assessment on separate scales (ILA, SLA), subject-reported onset of effect, subject satisfaction and adverse events (AEs). Responder rates of subjects achieving none-or-mild GL severity were calculated for the ILA and SLA scales separately. Time to return to baseline GL severity was calculated as a composite of both ILA and SLA scales.

Results: In both studies, none-or-mild ILA responder rates were significantly higher for relaBoNT-A vs. placebo at Month-1 (GLs: 96.3% vs 4.5%, p<0.001; LCLs: 87.2% vs 11.9%, p<0.001), remaining higher through Month 6 (GLs: 24% vs 2%; LCLs: 23% vs 7%, p≤0.002). 39% reported onset within 1 day in GLs, 34% in LCLs. Median time to return to baseline ILA/SLA was >24 weeks (end of study) in GLs, 24.7 weeks in LCLs. Both studies had high subject satisfaction (94% GLs; 87% LCLs, Month 1) and low rates of treatment-related AEs with relaBoNT-A vs. placebo (GL study: 3.6% vs. 0%; LCL study: 6.1% vs. 5.5%).

Conclusion: RelaBoNT-A treatment of GLs or LCLs was effective and safe, with high subject satisfaction and rapid onset. For both indications, the none-or-mild severity response was significantly higher after RelaBoNT-A treatment than placebo both at Month-1 (primary endpoint) and at Month 6. A long duration of effect was supported by Kaplan-Meier analyses showing median times to return to baseline of 24 weeks or longer for both indications.

An Evaluation of VYC-17.5L for Forehead Contouring: A Prospective, Open-Label, Post-Marketing Study (Sponsored by ALLERGAN AESTHETICS)

Objectives: This prospective, open-label, post-marketing study conducted in France evaluated the effectiveness and safety of the hyaluronic acid (HA) injectable filler VYC-17.5L for the improvement of forehead contouring.

Introduction: Forehead contouring, particularly in the supraorbital area and lower forehead, can be altered using injectable fillers. VYC-17.5L is a filler containing 17.5 mg/mL of HA with lidocaine (3 mg/mL) for treatment of skin depressions due to premature aging. Prior studies have demonstrated the safety and efficacy of VYC-17.5L in several facial areas including nasolabial folds and radial cheek lines, but little has been published on the effectiveness of VYC17.5L on the forehead. The purpose of this study was to evaluate the efficacy and safety of VYC-17.5L for forehead contouring.

Materials / method: Adult (≥18 years) subjects with irregularities of forehead contouring received VYC-17.5L in the forehead on Day 1 with optional touch-up on Day 14. Effectiveness was assessed by the proportion of subjects with an investigator-assessed Global Aesthetic Improvement Scale (GAIS) of “improved” or “much improved” from baseline at month 1 (M1). Secondary endpoints were subject-rated GAIS and FACE-Q Satisfaction with Facial Appearance. Injection site reactions (ISRs) and adverse device effects (ADE) were assessed throughout the study.

Results: 80 subjects completed the study. The primary endpoint was met: 98% of subjects showed improvement on the investigator GAIS. Subject GAIS scores were also improved. FACE-Q scores significantly improved from baseline (p<0.0001) and were sustained through month 12. Most ISRs were mild or moderate and resolved in ~1 week. 9 subjects (11%) reported ADEs with the majority being mild or moderate; the most common ADE was pain which resolved in <15 days.

Conclusion: This prospective, open-label study showed that VYC-17.5L was effective for improving forehead contouring, as demonstrated by improvements in investigator- and subject-rated measures. In addition, VYC-17.5L was well tolerated.

A Prospective, Open-Label Post-Marketing Study of Safety and Effectiveness of Ha/CaHA Filler for Midface Soft Tissue Augmentation: Interim Analysis. (Sponsored by ALLERGAN AESTHETICS)

Objectives: This open-label, post-marketing study of HA/CaHA conducted in France collected prospective data to support ongoing registration in Europe and other countries. These interim data include primary endpoint results.

Introduction: HA/CaHA is a soft-tissue hybrid filler intended for facial soft tissue augmentation. It consists of synthetic calcium hydroxyapatite (CaHA) microspheres (55.7% w/w) embedded in a cross-linked sodium hyaluronate gel (20 mg/mL HA). The Lidocaine version includes 0.3% (w/v) lidocaine HCl. Limited published prospective data on its safety and effectiveness are available.

Materials / method: Adults with moderate to severe midface volume deficit on Allergan Midface Volume Deficit Scale (MFVDS) received HA/CaHA in midface (main injection site), lateral face or jawline (optional) at initial treatment and optional touch-up at day 14. Primary endpoint was responder rate: ≥1-grade improvement from baseline on MFVDS at month 1 (1M). Secondary endpoints were investigator- & subject-rated Global Aesthetic Improvement Scale (GAIS) & FACE-Q Satisfaction with Cheeks & Satisfaction with Facial Appearance. Safety (injection site reactions [ISRs], adverse events [AEs]) was assessed throughout.

Results: 134 subjects completed the 1M visit; most had moderate midface volume deficit. The primary endpoint was met: 82.8% of subjects had ≥1-grade MFVDS improvement and responder rates remained high through month 9. Investigator and subject GAIS were improved. Both FACE-Q scale scores were improved. Most subjects (95.7%) had ISRs as expected of a HA and CaHA filler (eg, tenderness, pain); most were mild/moderate and resolved in <1 week. 52 subjects (37.1%) had treatment-related AEs (eg, injection site pain); most were mild in severity and resolved in <1 week. No treatment-related SAEs were reported.

Conclusion: Overall, most subjects treated with HA/CaHA achieved clinically meaningful improvement in midface volume deficit, GAIS, and subject-reported outcome measures. In addition, the safety results showed that HA/CaHA was well tolerated.

Utilizing the AART methodology to maximize aesthetic treatment outcomes across different patient profiles (Sponsored by GALDERMA)

Objectives: To utilize the novel, standardized, systematic methodology of Assessment, Anatomy, Range and Treatment (AART™) for facial aesthetic treatments across different patient profiles using poly-L-lactic acid (PLLA-SCA; Sculptra®).

Introduction: Patients seek aesthetic treatments to address dissatisfaction with specific areas of their face. A full understanding of the patient’s reasons for seeking treatment is necessary to fully address their concerns and optimize treatment. AART™ is a novel tool for aesthetic clinicians that provides guidance for practitioners to perform a standardized, full facial assessment and allows an individualized treatment approach to holistically address a patient’s aesthetic concerns. Its use for facial aesthetic treatments across four different patient profiles using PLLA-SCA will be evaluated.

Materials / method: The use of AART™ in clinical practice was demonstrated across different patient profiles using four case studies. Following assessment, PLLA-SCA was the most appropriate treatment solution to address identified issues. PLLA-SCA was reconstituted with sterile water for injection and 2% lidocaine hydrochloride was added before injection. PLLA-SCA was injected subdermally into the appropriate facial region for the desired clinical outcome. The treatment outcomes evaluated were skin quality, glow, renewed volume and lifting. Adverse events were recorded throughout the treatment journeys.

Results: Each subject received PLLA-SCA at 2–3 injection sessions administered at 4–6-week intervals. In case 1, extensive acne scarring was considerably improved after 6 months and glow restored. In case 2, PLLA-SCA provided natural full face volumization with increased fullness and skin thickness/density in a post-bariatric patient. In case 3, PLLA-SCA helped correct volume deficiencies, restore firmness and address facial sagging. In case 4, PLLA-SCA improved structure and firmness to provide a lifting effect to slim a heavy face. No adverse events were reported in any of the patients presented.

Conclusion: The AART™ methodology applies a comprehensive and global approach to skin health and patient well-being while allowing a focused approach to different treatment outcomes and patient goals. In the four patients evaluated, PLLA-SCA delivered improvement in skin firmness plus it renewed facial volume, improved skin quality and thickness, restored skin glow, and provided a natural lifting effect. The AART™ methodology permits an individualized treatment approach in facial aesthetics and may be updated to include new advances in aesthetic medicine and address patients’ evolving needs over time.

Trichloroacetic acid based chemical peeling treatment of Fitzpatrick IV-VI skin phototypes: specifics and outcomes (Sponsored by SKIN TECH PHARMA GROUP)

Objectives: Targeted evaluation of the efficacy and safety profile of the Trichloroacetic acid-based chemical peeling - Skin Tech® Easy TCA® (CE-marked Medical Device Class IIa containing 15% TCA in Base Solution, Ex-tempora mixture) in patients with facial skin photoaging, pigmentation disorders and high skin phototypes was conducted in UK and South Africa. The study was part of the post-market surveillance of the product.

Introduction: Approximately 25% of the global population have Fitzpatrick IV-VI skin phototypes(1), characterised by increased amount of melanin content in the epidermis due to structural differences of melanosomes comparing with low skin phototypes. There are important considerations to take when performing chemical peels for medical and aesthetic indications on individuals with high phototypes.

Materials / method: Multicenter, open, single-arm, before-after study was performed in July-November 2023 in 3 study centers in United Kingdom and South Africa. Patients received a single full-face treatment with 15% TCA-based chemical peeling - Skin Tech® Easy TCA® - according to the Instructions for Use. The solution was applied in 2-3 coats to reach the peelings depth 3-4 according to Deprez’s classification(2). Sun avoidance during the study was strictly recommended. Skin photoaging clinical signs were tracked before (Day0) and after the treatment (Day7) using validated Alexiades-Armenakas scale(3). The asses

Results: Patients (n=16; 75% women, 25% men; aged 36-55 years; Fitzpatrick skin phototype III till VI) presenting photoaging of predominantly mild grade showed improvement in wrinkles, skin laxity, elastosis, dyschromia, erythema, telangiectasia, keratoses and texture. The photoaging grade demonstrated an average improvement of 23% compared to baseline measurements, achieving a 100% success rate.

Objectives: Investigation of a novel monophasic dermal implant containing crosslinked hyaluronic acid associated with amino acids (CE-marked Medical Device Class III, RRS® HA Long Lasting, Skin Tech Pharma Group, Spain) for its moisturising and volumising effects on the skin of the back of the hand in the treatment of photoaging. This study, which is part of the post-marketing clinical follow-up activities, aimed to evaluate the efficacy of the product in the treatment of the signs of ageing on the hands, with an additional focus on the evaluation aspects of safety profile.

Introduction: Following the face, the hands represent the second most prominent indicator of an individual’s age. Typically, subcutaneous fat loss accentuates the visibility of veins, tendons, and bony contours on the dorsum of the hand(1,2). Additionally, prolonged exposure to UV light throughout one’s lifetime renders the hands especially susceptible to cutaneous pigmentation alterations, commonly referred to as photo-damage(3). The most commonly conducted interventions involve filler injections, laser treatments, and fat grafting(4).

Materials / method: A multinational, multicenter, open-label, single-arm, before-after study conducted from July to November 2023 involved 10 study centers across Albania, Bulgaria, Georgia, Macedonia, Serbia, and the United Kingdom. The study included 60 clinical cases, with participants predominantly female (93%) and male (7%), aged between 56 and 65 years. The participants exhibited primarily moderate wrinkles and dull skin radiance.

Conclusion: The clinical study on two new HA dermal fillers showed an aesthetic improvement with an average of at least one improvement grade for each treated area after injection. The increase in lip fullness as well as the reduction in the severity of perioral fine lines and nasolabial folds were maintained over time.
For each injected area, notable improvement effects were still observed at 12 months for more than half of the subjects.

The bidirectional cogged thread loaded into the specially designed flex-form cannula (Sponsored by AYOON MEDI)

Objectives: The drooped nasolabial fold is corrected using the cogged thread loaded on the flex-form cannula, and its usefulness is verified.

Introduction: Recently, various attempts have been made to correct the drooping of the nasolabial fold with threads. However, it is difficult to insert a sufficiently long cogged thread due to the natural convex curvature of the anterior mala area. Using a bi-directional cogged thread loaded on a flexed cannula designed in a special shape devised by the author, the nasolabial fold was corrected and its usefulness verified.

Retrospective case series following patients treated with a combination of CPM-HA and CaHA fillers (Sponsored by MERZ AESTHETICS)

Objectives: Clear guidelines for practitioners who want to implement such methods are lacking. In this case series, we provide examples showcasing real-world applications of combination use of CPM-HA and CaHA.

•A retrospective case study examined 4 patients who received treatments of both fillers composed of cohesive poly-densified matrix hyaluronic acid (CPM-HA; Belotero®, Merz Aesthetics GmbH, Frankfurt am Main, Germany) and calcium hydroxyapatite, (CaHA; Radiesse®, Merz North America, Raleigh, NC) either in the same session or series of sessions.

Materials / method: •Data were gathered from the charts of 4 international physicians that utilized treatment plans including both CaHAand 1 or more of 3 types of CPM-HA fillers (CPM-HA/V, Belotero® Volume; CPM-HA/I, Belotero® Intense; CPM-HA/B, Belotero® Balance). The study included adults over 30 years of age who provided informed consent for treatment, data collection and photo release for publication.

The use of magnetic stimulation for the treatment of female urgency urinary incontinence (Sponsored by ISKRA MEDICAL)

Objectives: The aim of our study was to assess the efficacy of MS in the treatment of urgency urinary incontinence (UUI). The efficacy of MS was assessed with the score of ICIQ-SF (International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form), which is the only validated questionary in Slovene language, and we compared our treatment group with the sham treatment group to assess the placebo effect in the treatment of UUI with MS.

Introduction: UI is differentiated according to the underlying pathophysiological mechanisms and is divided into the most common types: stress (SUI), urgency (UUI), mixed (MUI), and “overflow” UI. Deciding on a conservative or surgical treatment approach, which vary according to the type of UI, depends predominantly on the type and severity of UI and on comorbidities. Materials / method: In the first part of our research, we conducted a systematic review of the available literature on the efficacy of MS in the treatment of UI. In the second part we conducted a prospective randomized, sham-controlled trial (RCT) that took place at the Division of Gynaecology and Obstetrics at the University Medical Center of Ljubljana. We included 40 female patients with UUI who were randomized in active and sham-controlled group in a ratio 2:1 in favour of the

Results: Based on the findings from the systematic review, it can be concluded that MS is a successful non-invasive conservative method for treating UI. Future studies are necessary, all of which should include a large sample size, a control group, an optimal research protocol, pretreatment analyses, standardization, and longer follow-ups. Attraction of fibroblasts by Micro-Focused Ultrasound (MFU-V) (Sponsored by MERZ AESTHETICS)

Objectives: Micro-Focused Ultrasound with real-time Visualization (MFU-V, Ulthera®, Merz North America, Inc.,Raleigh, NC, USA) is the first FDA-approved device for non-invasive skin lifting treatment. The MFU-V uses transducers with different frequencies to target specific depths in the skin ranging of 1.5, 3.0 and 4.5 mm, inducing thermal coagulation points (TCPs) at precise intervals in the respective area. Thereby, stimulating the tissue for efficient neocollagenesis and ECM-remodeling resulting in subsequent skin lifting effects.

Introduction: The current study investigates the underlying mode of action of MFU-V, focusing specifically on the attraction of fibroblasts to the treatment area.

Materials / method: MFU-V treatment was used in an in-vivo study with transducers targeting 1.5 mm and 3.0 mm depths following histomorphometric analysis of tissue sections 14 and 90 days post treatment.
Sections were paraffin embedded and stained for immunohistochemistry (IHC) with Hsp47, a marker of activated and ECM-producing fibroblasts and a collagen-specific chaperone that plays an essential role during collagen biosynthesis1.
Subsequent analysis of Hsp47-positive fibroblasts within TCPs and within a 300 µm expansion around TCPs.

Results: • The effects of MFU-V in the skin can be visualized at 1.5 and 3.0 mm depth by IHC using Hsp47 antibody (A, B).
• After 90d, the total number of cells per mm² increased by 2.4 fold compared to the surrounding tissue (C, ANOVA Šídák‘s multiple comparison ****p<0,0001).
• At 90d post TCP, the number of Hsp47- positive fibroblasts within the TCP is greatly increased compared to the expansion area and compared to 14d (D, ANOVA Šídák‘s multiple comparison ****p<0,0001 ).

Conclusion: • The current study shows that MFU-V generates TCPs that strongly attract ECM-producing fibroblasts within 90d after treatment.
• Attracted fibroblasts were Hsp47 positive, indicating a TCP-mediated tissue lifting via neocollagenesis.

Evidence-based science behind the diffusion and spread of abobotulinumtoxinA (Sponsored by GALDERMA)

Objectives: This presentation aims to summarize the scientific evidence of diffusion or spread of abobotulinumtoxinA (aboBoNT-A) as compared to other commercially available botulinum toxin A (BoNT-A) formulations.

Introduction: There is a myth that different formulations of BoNT-A approved for aesthetic use may have different diffusion and spread characteristics. Diffusion is a passive movement toward a lower concentration gradient (Fick’s law) and is dependent on number of injected molecules (dose), while spread is mainly affected by the injector (e.g. volume/point, injection speed, injection technique/angle).

Materials / method: Publications exploring diffusion/spread of aboBoNT-A and other BoNT-A products were reviewed. These included in vitro experiments on how pH affects the BoNT-A complex; studies to measure diffusion/spread into adjacent muscles of BoNT-A products (in mice) and of aboBoNT-A in escalating doses (injected into foot muscles, extensor digitorum brevis (EDB), in human volunteers). Safety results from clinical studies of glabellar line (GL) treatment were reviewed for evidence of spread, e.g. ptosis events, on-label and when using high doses up to 125U of aboBoNT-A and at different injection volumes.

Results: At physiological pH in vitro, complex proteins dissociate from the core toxin within minutes. In mice, BoNT-As of various complex sizes and formulations showed no difference in diffusion/spread into adjacent leg muscles. In healthy subjects, a randomized GL study using 50-125U aboBoNT-A or placebo showed a low, not dose-related ptosis rate, 0-2.5%. In healthy subjects, when injecting EDB at different doses no spread of aboBoNT-A was observed in nearby muscles. Another GL study revealed similar safety profiles with no distant spread when doubling the aboBoNT-A volume from 0.05mL to 0.1mL/point.

Conclusion: Evidence from in vitro, animal experiments and human trials show that diffusion of BoNT-A is not related to complex size, and that the risk of spreading of aboBoNT-A to other muscles is comparable to other BoNT-A products. AboBoNT-A GL treatment is well tolerated at a large range of doses (50 to 125 U) or volumes per injection site, with a low incidence of ptosis comparable to that of other BoNT-A products.

Effectiveness and Safety of a New NASHA-HD Hyaluronic Acid Injectable, HA-SHA, for Augmentation and Correction of Chin Retrusion (Sponsored by GALDERMA)

Objectives: To assess effectiveness and safety of HA-SHA vs. a no-treatment control for augmentation and correction of chin retrusion.

Introduction: A new hyaluronic acid (HA) injectable, Restylane® ShaypeTM (HA-SHA; Galderma), has been developed using the new NASHA-HD™ technology, with improved efficiency to the cross-linking, creating a gel with high HA concentration (25 mg/mL), high G’ (916 Pa), and high gel content and stability. The product is to be injected on bone, aiming for an aesthetic improvement of chin retrusion that mimics the natural shape of the bony chin. A pivotal clinical investigation was conducted to assess effectiveness and safety of HA-SHA vs. a no-treatment control for augmentation and correction of chin retrusion.

Materials / method: Adults >18 years with mild or moderate chin retrusion on the Galderma Chin Retrusion Scale (GCRS) were randomized 3:1 to HA-SHA (n=103) or no-treatment control (n=37). The HA-SHA group received up to 4 mL HA-SHA on Day 1, and optional touch-up (up to 2 mL) at Month 1 if required for optimal results. Primary endpoint was the responder rate (≥1-grade improvement on the GCRS) at Month 3 as per the Blinded Evaluator live assessment. Other assessments included GCRS, aesthetic improvement on the Global Aesthetic Improvement Scale (GAIS), subject satisfaction, and adverse events (AEs) up to Month 12.

Results: After injection of a mean volume of 2.8 mL (≥95% supraperiosteally), the HA-SHA group had a significantly higher responder rate (83.3%) vs. controls (10.8%) at Month 3 (p<0.001). A high GCRS response was sustained throughout the study (74% and 66% in Months 9 & 12). Aesthetic improvement (GAIS) was achieved in ≥97% (by investigator) and ≥89% (by subject) from Month 1-12. The HA-SHA group subjects were satisfied with their chin shape (≥87%), projection (≥87%), and profile (≥82%) from Month 3-12. Product-related AEs were mostly mild to moderate, and non-serious (pain, tenderness, redness, etc.).

Conclusion: HA-SHA, a new NASHA-HD injectable, with high G’ and high HA concentration, significantly improves chin retrusion, compared with a no-treatment control. HA-SHA was effective for chin augmentation and correction of mild to moderate chin retrusion, with high aesthetic improvement and subject satisfaction sustained through Month 12. HA-SHA treatment had a good safety profile and was well tolerated throughout the 12-month study period.

A Snapshot in Time – Experts’ Main Concerns on Adverse Event Prevention and Management in Aesthetic Treatments with Hyaluronic Acid Fillers (Sponsored by GALDERMA)

Objectives: To identify and prioritize topics or concerns related to adverse event management and prevention, for injectable fillers.

Introduction: Aesthetic practitioners prioritize preventing and managing complications associated with hyaluronic acid (HA) fillers and biostimulators; however, numerous aspects within the safety field remain controversial and despite guidance in the literature conflicting data exist. Galderma convened a safety task force with globally recognized experts in filler safety. The first meeting in a planned series of meetings was held on June 17th 2023.

Materials / method: The meeting gathered 15 experts, each with a background in scientific research, extensive clinical experience, and membership in diverse safety associations or committees. The experts represented eight countries including Austria, Brazil, Canada, China, France, Netherlands, South Korea, and US. The group identified and prioritized eight critical concerns related to filler adverse event management and prevention.

Results: The top concerns were: vascular adverse event management, hyaluronidase protocols, ultrasound (preventive and complication management), cannula versus needle, endpoints (landmarks to identify vascular occlusion), aspiration, treatment options beyond enzymes, and intraluminal versus extraluminal origins of vascular occlusion. Furthermore, a majority supported use of aspiration and cannulas for safer injections although the topic remains controversial. Skin color, condition, and capillary refill were deemed the best endpoints for addressing vascular adverse events.

Conclusion: Top priorities were identified for future review, including management of vascular adverse events, hyaluronidase protocols, and ultrasound use, with delayed complications also a key concern. The experts provided valuable insights, identifying concerns within adverse event prevention and management. A need remains to assess the current state-of-the-art for each strategy related to adverse event prevention and management. Furthermore, the frequently overlooked delayed-onset complications were also deemed a high-priority topic for the next meeting.

SAMiRNA spontaneously forms spherical nanoparticles without cationic lipids in an aqueous solution. Therefore, SAMiRNA may be an ideal solution to overcome the stability and an undesirable innate immune response of current siRNA in the product development for Androgenic Alopecia.

Materials / method: Clinical studies were performed in a randomized and double-blind manner with two different doses and frequencies. In the low-dose (0.5 mg/ml) clinical study, AR68 was applied three times per week for 24 weeks, and through quantitative analysis using a phototrichogram, we confirmed increases in total hair counts.

Results: In the 24-week long high-dose (5 mg/ml) clinical study, AR68 showed average additional hair growth of 1.3-1.9 hairs/cm2 per month. No side effects were observed. Therefore, SAMiRNA targeting AR mRNA is a potential novel topical material for AGA.

Conclusion: This study demonstrates the efficacy and safety of
SAMiRNA-AR68 through an in vitro analysis of SAMiRNA-AR68 and two clinical studies. In comparison to the -9.3 hairs/cm2 average hair growth seen with daily oral administration of 1 mg finasteride over a 24-week period, high-dose AR68 application per week over 16-week period led to average hair growth of 7.5 hairs/cm2, indicating comparable efficacy as finasteride. AR68 has no side effects and is more convenient as a weekly application.
This study has limitations in a number of subjects and a spectrum of races.

7 days after PDLLA(AestheFill) injection (Sponsored by AESTHEFILL® BY CHAEUM PHARMA)

Objectives: To show the collagen stimulation begins after 7 days of application of PDLLA

Introduction: Biostimulators are the latest trends in Cosmetic procedures, substances such as Poly-D, L-Lactic Acid are used to induce Collagen synthesis by a Sub clinical inflammatory reaction, however there are no data available showing the exact timing collagen stimulation begins after the application, in this report we want to show that Collagen production can be seen in biopsies samples after 7 days of the application of PDLLA.

Materials / method: A 34 year old female is our patient, a first biopsy was taken in the inner arm, this first biopsy is the basal sample or biopsy 0.
After the biopsy was taken, PDLLA (AestheFill®)was applied in the inner arm of the patient. PDLLA (AestheFill®)was diluted with 7cc of sterile water and lee of lidocaine without epinephrin, once AestheFill® was prepared and ready to use, PDLLA was applied with fanning technique all over the inner arm area. 7days after the application of AestheFill® another biopsy was taken in the same place it was applied. 2ldays after the application of AestheFill® another biopsy

Results: We chose 2 types of stainning for the biopsies; Hematoxylin and eosin Staining and the Masson Trichrome Stain. We could see twice the amount of fibroblasts compared to the basal biopsy, angiogenesis was also observed on the papillary dermis, lymphocytes, elastic fibers and collagen fibers were twice the amount compared to the basal biopsy. Another biopsy was taken 2ldays after the application and major histological changes were observed with an increasing amount of collagen fibers, elastic fibers as well as angiogenesis.

Conclusion: In this report we can observe that major histological changes in Collagen production are seen as early as 7 days after the application.

An Economic Model for a Recently Introduced Ready-to-Use Liquid Formulation of AbobotulinumtoxinA (Sponsored by GALDERMA)

Objectives: The first ready-to-use (RTU) liquid formulation of abobotulinumtoxinA (aboBoNT-A solution) was introduced in Europe in 2021 for glabellar line treatment. To evaluate the impact on patient satisfaction and revenue for clinics switching from powder BoNT-A to the new RTU aboBoNT-A solution, a dynamic budget impact model was created, investigating clinical, logistic and economic aspects of this transition.

Introduction: Performance data on aboBoNT-A solution was available from 2 Ph3 clinical trials, 1 Ph4 study of usability and patient satisfaction, and 1 real-world study.

Materials / method: The dynamic economic model was built to assess the potential impact of using RTU aboBoNT-A versus powder BoNT-A on business revenue using variables to cover clinical (eg effect duration), safety (eg reconstitution errors), logistic (eg appointment frequency), economic (eg price) and commercial (eg patient acquisition) aspects. Model input included performance data from prior clinical and real-world studies, as well as data from a literature search validated by primary market research with practicing clinicians from UK, to define current practice baseline.

Results: Main clinical benefits of switching to RTU-aboBoNT-A fed into the model were faster onset, longer duration of action (6 vs 4 months) and reduced touch-up appointment rate (2 vs 10%), motivating patients to pay a higher price. Reduced appointment frequency frees capacities in the clinic, while clinical benefits of the product attract additional patients. The economic model considers different potential adoption rates of the product by the clinic, variable pricing scenarios and availability of additional capacities. The expected increase in revenue for the clinic ranged from £21000-195 000/year.

Conclusion: RTU-aboBoNT-A offers several clinical benefits positioning the product as a “Premium Product”, resulting in a greater willingness to pay an above-average price. Sharing of positive experience is expected to attract additional patients, taking up capacities freed by reduced required appointment rates. The model flexibly predicts increases in revenue depending on parameters that can be variably selected to reflect each clinic’s settings and preferences. Depending on selected parameters, increased revenues up to £195 000 per year are expected, for a 1 000 patient annual caseload.

Screw Caps: A Break with The Past (Sponsored by ALTHENA MEDICAL)

Objectives: Our goal was to develop and produce a screw cap easy to unscrew, even for a doctor wearing gloves, a screw cap that is suitable for sterilization in an autoclave and that guarantees a seal even years after ﬁlling the syringe.

Introduction: There is only one type of screw cap at present. This standard screw cap contains a rubber inside that wraps around the tip. In this way the doctor has to apply an initial force for unscrewing, which in some cases can be annoying. Our research comes from the complaints of doctors and specialists who, wearing gloves, find the current systems for unscrewing pre-filled syringes annoying and impractical. Our effort has been to find a different solution to the current standard: a simple, practical and easy to use solution.

Materials / method: The screw cap has been tested during autoclaving many times using various types of drugs and substances from ferroﬂuid materials, to bone cements, from hyaluronic acids with di erent densities to saline solutions. All the syringes, 100 pieces for every substance, were then subjected to hermeticity tests with blue methylene, thus demonstrating the absolute tightness of the cap.

Results: The same syringes have been subjected to fast aging, up to 5 years, proving also in this case the perfect seal of the cap. Tests with a torque wrench have shown that the necessary force to open one of our syringes is only 0.1 Nm, while standard syringes required 0.5 Nm.

Conclusion: Many laboratory tests have conﬁrmed that with a considerable reduction of the force torque when opening the syringe, the sealing power is remarkable and this is conﬁrmed both in the autoclave during sterilization and 3 years after ﬁlling the syringes.

DISCOVERY OF 2-MERCAPTONICOTINOYL GLYCINE (Sponsored by L'ORéAL DERMATOLOGICAL BEAUTY)

Objectives: Melanin production inhibitors currently marketed inhibit tyrosinase, the rate limiting enzyme in melanin synthesis process. Yet only a few have demonstrated clinical efficacy while some present safety issues. Moreover, environmental respect is barely taken into consideration. Research on skin lightening agents considering efficacy. safety but also environmental respect is then of great importance to significantly improve marketed actives.

Introduction: For this purpose, a high throughput screening test (HTS) evaluating melanin production on a large chemical diversity and in silico predictive methodology was then used to design original and performant chemical structures resulting in the discovery of 2-mercapto nicotinoyl glycine (2-MNG). Mechanism studies were conducted in tubo, in vitro, as well as in vivo showing a unique model of action of 2-MNG consisting in conjugating with melanin precursors, avoiding their integration into growing eumelanin and pheomelanin.

Materials / method: In vitro screening and 2-MNG discovery:
HTS assay: Normal human melanocytes (NHM) were cultivated in 384-well plates. Over 100 000 raw materials were screened after dilution in the culture medium at 10 doses and incubation for 72 hours. Optical density was measured at 340 m reflecting melanin content. Data was normalized in comparison with untreated cells. IC 50 was determined.
Environmental profile assessment & in silico prediction: Water footprint index (WFI) was determined. An internal digital tool was used to design and screen the best thiopyridinone profiles, taking into consideration p

Results: Selection of 2-MNG: HTS screening led to the discovery of thiopyridinone chemical family as potent brightening structures. 197 analogs were virtually designed and screened based on in silico prediction of relevant parameters' (safety, environmental respect, skin penetration). 2-MNG (WFI=11) was selected for further investigation as displaying the best compromise between these parameters
and WFI compared to on-market benches: 4-n-BR WFI = 5813, kojic acid WFI= 316 (Figure 1).

Conclusion: 2-MNG is a new molecule displaying a high efficacy in managing melanin production in vitro. Its mode of action, which consists of binding and thus inactivating melanin precursors, leads to a high clinical performance either on prevention of skin darkening or inhibition of delayed tanning with no adverse effects

Objectives: Evaluation of the efficacy and tolerability of a combined therapy: preformed growth factors vehiculated through iontophoresis in patients with androgenetic alopecia and associated telogen efflu- vium, to obtain faster hair regrowth.

Introduction: Hair loss represents a problem for the patient for cos- metic and psychological reasons because hair sym- bolizes an important mirror of our image and a physical attractiveness to self-perception of beauty. The field of hair disorders is constantly growing. The first step to address this issue, is to collect a good historical record and perform a thorough physical examination. Labo- ratory testing is often unnecessary, while trichoscopy is fundamental for all hair diseases. Androgenetic al- opecia and telogen effluvium are common causes of non-scarring alopecia. Many treatments are available,

Materials / method: Treatment was performed between June 2018 and June 2019 on 60 pa- tients with androgenetic alopecia and associated telogen effluvium. Each patient underwent 4 sessions in total, each session was performed every 3 weeks. Global photography and trichoscopy were collected at every session of therapy. All patients filed out a self-assessment questionnaire.

Results: Results were very promising, with improvement of hair density and thickening of the hair shaft diameter in most of patients seen with both global photography and trichoscopy. All patients were satisfied of the clinical result and reported a complete reduction in hair loss. No serious adverse side effects were reported.

Conclusion: The use of growth factors associated with iontophoresis technique is a useful treatment for treating and preventing androgenetic alopecia. In addition, in case of associated telogen effluvium, this technique allows for an early stop of hair shedding, especially when cosmetic procedures do not provide satisfactory results in patients.

Objectives: A scar is a connective tissue formed in the healing process at the site of the traumatic action. Scars can be divided into mature and immature scars, and due to their shape into linear, atrophic, hypertrophic and keloids [l]. Physicochemical properties of two poly L-lactic acid dermal fillers: potential impact on their biological properties (Sponsored by GALDERMA)

Objectives: To evaluate the thermal properties and crystallinity of two commercially available poly L-lactic acid (PLLA) dermal fillers, Sculptra® (PLLA-SCA; Galderma, Sweden) and GANA V® (PLLA-GA; GCS Co., Seoul, South Korea), and how these properties might affect the subclinical inflammatory response that stimulates collagen growth in the skin.

Introduction: Dermal fillers have been used for many years in cosmetic dermatology. Across all PLLA fillers currently available, PLLA-SCA is the most widely studied. PLLA fillers comprise a mixture of PLLA microparticles and other components and, despite similar formulations, differences are often observed in their clinical application in daily practice. Moreover, degradation of PLLA is dependent on factors such as stereochemistry, molecular weight, crystallinity, particle size and microparticle morphology. Hence, the physicochemical characteristics of a PLLA filler can influence its clinical performance.

Materials / method: The thermal properties and the degree of crystallinity of PLLA-SCA and PLLA-GA were determined using differential scanning calorimetry analysis. Samples were scanned from 25 to 300°C in a nitrogen atmosphere with a heating rate of 10°C/min. Additional samples were heated to 120°C, allowed to cool to room temperature and then analysed by heating from 25 to 300°C at a heating rate of 10°C/min. PLLA-SCA and PLLA-GA samples were also analysed using an X-ray powder diffractometer. Measurements were performed at room temperature (approximately 22°C).

Results: The X-ray powder diffraction spectra displayed a sharper and more intense peak for PLLA-GA compared with PLLA-SCA. There were also additional smaller peaks on either side of the main peak in the diffractogram of PLLA-GA that were absent with PLLA-SCA. The differential scanning calorimetry thermograms indicated that there were three thermal events occurring for both the PLLA-SCA and PLLA-GA samples. For PLLA-SCA, the first two thermal events occurred between 65 and 90°C and the third thermal event occurred at 165°C, while for PLLA-GA all three thermal events occurred between 156°C and 169°C.

Conclusion: PLLA-GA has a more crystalline structure than PLLA-SCA, and the three melting points of PLLA-GA suggest it comprises a heterogeneous mix of polymer isoforms. The single melting point observed with PLLA-SCA suggests the presence of a homogenous formulation of PLLA-SCA which, combined with the less crystalline structure, could result in a slower degradation rate and more sustained biological response versus PLLA-GA. Degradation rate may impact treatment duration and could, in part, explain the 24-month duration of effect observed with PLLA-SCA in pivotal clinical trials.

POST-COMBAT BURNS AND SCARS TREATMENT: UKRANIAN EXPERIENCE (Sponsored by BIOFORMULA)

Objectives: Following the Russian invasion of Ukraine on February 24, 2022, a significant number of civilians and military personnel were affected, many experiencing combined thermo-mechanical type injuries.
To address this urgent problem, the 'Un-burned' humanitarian project was initiated in July 2022 (www.neopalymi.in.ua).

A randomized, comparative study describing the gene signatures of Poly-L-Lactic Acid (PLLA-SCA) and Calcium Hydroxylapaptite (CaHA) in the treatment of nasolabial folds (Sponsored by GALDERMA)

Objectives: To identify and compare gene signatures of skin biology-related genes 3 months after initial treatment with injectable poly-L-lactic acid (PLLA-SCA) and calcium hydroxylapatite (CaHA) via punch biopsies of the nasolabial folds (NLF) and to assess volume change in the NLF with 3D photography.

Introduction: PLLA-SCA, a biostimulator, and CaHA, a biostimulatory filler, have been shown to stimulate varying ratios of type 1 and type 3 collagen. The gene signatures related to skin biology for each collagen biostimulatory agents are unknown. 

Materials / method: This was a single-center, randomized, comparative study in which subjects with a minimum of shallow NLF received 2 treatments of on-label PLLA-SCA injections or CaHA injections at a 4-week interval for optimal correction. Study assessments included punch biopsies and 3D photography at Baseline and 3 months post first treatment.

Results: Eleven subjects received PLLA-SCA and ten subjects received CaHA. Both treatments induced gene expression associated with collagen formation, inflammation, and extracellular matrix remodeling. Compared to CaHA, PLLA-SCA quantitatively expressed more genes related to regeneration and morphogenesis (16% vs 10%), and less genes related to inflammation (14% vs 18%) and pro-inflammation (5% vs 11%) at 3 months. 3D photography demonstrated improvement in depth and volume, in line with previously published data on NLF correction. There were no treatment-related adverse events throughout the study.

Conclusion: This study suggests that PLLA-SCA and CaHA have differentially expressed gene signatures and unique mechanisms of action. PLLA-SCA upregulated more genes relating to regeneration and morphogenesis, whereas CaHA was more inflammatory and pro-inflammatory.

Conclusion: This study elucidates that the pairing of PX/NAM benefits post-procedure skin by enhancing the level of barrier renewal and reducing the level of inflammation and melanogenesis. These benefits translated to the creation of a topical formula that augments the anti-ageing benefits of non-ablative laser procedure at a superior level when compared to PBO.

Tissue changes over time after polydioxanone thread insertion: An animal study with pigs (Sponsored by N-FINDERS)

Objectives: We aimed to determine what significant changes occur in the tissue over time when a PDQ suture is inserted.

Introduction: In early 2011, a treatment using monofilament polydioxanone (PDQ) thread for facial rejuvenation was introduced in Korea. Absorbent threads used for lifting are divided into monofilament type threads without teeth and barbed threads with teeth. Clinical studies using PDQ threads have shown positive results. Suh and colleagues demonstrated that monofilament treatment can improve tightening and skin texture, indicating that PDQ thread injection can induce positive changes in facial tissue.

Materials / method: Four white Yucatan breed pygmy pigs with skin most similar to human skin structure were selected. 4-0 PDQ thread was inserted into the subcutaneous fat layer. Tissue samples were collected at 4, 12, 24, and 48 weeks. For histological analysis, H&E staining, Masson's trichrome staining, and anti-smooth muscle actin immunohistochemical staining techniques were used.

Results: Nine histological findings emerged over time, summarized in five tissue changes.

Conclusion: PDQ suture causes specific changes in the surrounding tissue, resulting in nee-collagen production, fiber fusion effect, fat reduction, tissue construction, and improved vascular environment. The results of this study explain the positive changes described in previous clinical studies.

Novel Pathway in Enhancing Patient Comfort Post-Procedure with a Topical Treatment (Sponsored by REVISION SKINCARE)

Objectives: I. To develop a post-procedure cream (PPC) that can immediately be used by patients after receiving an energy-based treatment to ameliorate discomfort and promote healing for enhanced recovery and increased likelihood patients will complete the recommended treatment series.

II. To determine the effect of the PPC in modulating the transient receptor potential vanilloid 1 (TRPV1) and stimulating the production of beta-endorphins after skin explant procedure treatment with thermal and mechanical energy.

Introduction: Skin rejuvenation procedures include those that wound skin through mechanical means, through chemical aggressors, and through the use of energy-based devices to treat aging facial skin concerns. These three common types of skin rejuvenation procedures vary in intensity, depth of penetration into the skin, and patient post-procedural downtime. Even though the end result from these rejuvenation procedures are notable and desirable, the negative experience of these procedures reduces the likelihood that the patient will return to complete the optimal series of treatments.

Materials / method: A semi-occlusive, breathable post-procedure cream (PPC) was formulated with botanical ingredients that directly stimulate communication between the nervous system and the skin. The PPC is a reverse emulsion (water-in-oil) which allows for effective delivery of water-soluble ingredients to a disrupted skin barrier by the action of a lipophilic base. Skin from plastic surgery of 48-year-old Caucasian women with Fitzpatrick Skin Type II was irradiated with UVB at 0.5 J/cm2 followed by microneedling. PPC was applied to the skin post-procedure for 3 hours and TRPV1 and beta endorphin were measured.

Results: Compared to skin explants damaged by UVB radiation and microneedling, there was a progressive decrease in TRPV1 expression and an increase in beta-endorphin expression when skin explants were treated with the PPC post-procedure. After 3 hours of treatment (post procedure), a highly statistically significant increase in beta endorphin of 104.9% was determined (*** p < 0.001). Conversely, a highly statistically significant decrease in TRPV1 by 54.8% was determined (*** p < 0.001).

Conclusion: The developed PPC targets an unmet need in post-skin rejuvenation procedures to improve post-procedure discomfort, enhance recovery time, and increase patient treatment plan adherence. This semi-occlusive, breathable cream formulated with neurocosmetics, microbiome technology, and hydrators and without anesthetics, can be applied to skin immediately after chemical, mechanical, and thermal based treatments. Results from the ex vivo study demonstrated the PPC targets specific biological pathways to reduce discomfort by suppressing TRPV1 while increasing the local production of beta endorphins.
