Introduction: By creating a controlled wound of the skin, the chemical exfoliators destroy the epidermis, a part of the dermis and induce tissue regeneration. They are mostly used in the field of esthetics, In therapy, TCA chemical peels may be of interest in many pathologies, both cutaneous and mucosal. They can also be useful in certain pathologies of the ano-genital sphere.
Materials / method: Chronic vulvar and anal mucosal and semi-mucosal pathologies resistant to classical treatments, in particular lichen sclerosis, lichenification, dyspareunia, vaginal dryness and mucosal fragility due to estrogenic deficiency in the context of menopause and abscesses and scars in hidradenitis suppurativa but also genital hyper pigmentation can benefit from TCA peel treatments.
TCA solutions ranging from 2O% to 35%Can be applied on these areas.
Paint brushes or cotton tips are used to perform the treatment.
Topical anesthesia or pulsed cold air system are welcome to decrease the pain.
Results: Whatever the location is, we obtain a clear objective improvement of the clinical signs and a regression of the functional signs.1 to 3 treatments are sometimes necessary. Maintenance treatments may be indicated to maintain the result. In all these patients the recurrences are less frequent after each treatment. In the context of Verneuil’s disease, TCA treatment can sterilize abscesses after incision drainage, treat fistulas and improve disfiguring scars.
Conclusion: Certain intimate pathologies resistant to conventional treatments seem to benefit from TCA peels. These peels probably have their place in the rehabilitation of the genital area in various diseases as well as in the context of menopause. These treatments improve the cutaneous texture of the ano genital area, and could therefore be used in rejuvenation in complement to other techniques currently being used in this field.
