Objectives: 1- Importance of botulinum toxin on therapeutic and cosmetic use;

2- Highlight the rarity of therapeutic failure;

3- Discuss the failure possibilities with emphasis on discussion of formation of neutralizing antibodies.

Introduction: The impressive results obtained since the late 70's on therapeutic indications and on cosmetic use in early 90's, botulinum toxin has become one of the preferred therapies.
Botulinum toxin has become the most common and first choice injectable used for cosmetic purpose and has an increasing number of indications on Dermatology.
Although rare botulinum toxin therapy failure may occur due to numerous reasons, including due to formation of neutralizing antibodies (NAb). The presence of NAb is estimated to be less than 1% in cosmetic use but should encourage more studies.

Conclusion: Therapeutic failures with the use of botulinum toxin may occur. We need more studies in order to clarify the possible causes. The finding of neutralizing antibodies in cases of cosmetic use failures appears to be less then 1%.
Our role is to seek scientific evidence to get answers to our questions.
