Objectives: Evaluation of the appropriate surgical technique for the treatment of the cervical region
Introduction: The cervical region is a target area for cosmetic defects due to aging, gravity, sun damage and lifestyle. An accurate physical examination is mandatory before planning the appropriate surgical treatment.
Materials / method: Apart from the rare cases of simple skin excess, we generally treat the platysma muscle by submental skin incision, at the level of the cervical angle, and removing the bands, if present, then performing a median suture. If fat is present, a liposuction if performed as well. We treat the SMAS by plication or flaps, performing the traction with 3-4 stiches 3/0 non-reabsorbable suture and a 4/0 resorbable running suture.
In the case of significant tissue relaxation in male, a treatment with skin incisions and « Z-plasty » of the cervical angle is performed.
Results: In our clinical experience, we noted that the treatment of the SMAS is mandatory to obtain a stable and long-lasting result. The direct treatment of platysma by submental approach is useful to achieve a pleasant cervical angle without recurrence of the bands.
Conclusion: In presence of severe skin and tissue laxity the facelift is the main surgical treatment for facial rejuvenation . The SMAS treatment rapresents a key point in order to obtain natural and stable results over time.
