Objectives: To show a step by step video of the procedure of injectable tissue replacement and regeneration for facial fat grafting.

Introduction: The majority of surgeons and surgical dermatologist who performfacial fat grafting use the Coleman technique, grafting the fat as if a natural filler primarily for aesthetic effects. Little attention is paid to the anatomical losses of fat in specific superficial and deep fat compartments.

Materials / method: A step by step video illustrating the anatomic and regenerative fat grafting technique is shown. Deep structures are replaced with microfat for structural fat grafting, but in the specific areas of losses as well as to address pre-existing skeletal deficiency. Superficial fat, dermis and epithelium are regenerated by nanofat injection, microneedling and nanofat biocreme.

Results: We have treated over 300 patients using this anatomic and regenerative approach to fat grafting. Numerous publications have documented both the aesthetic and intermediate term outcomes of our approach.

Conclusion: Anatomic replacement and regeneration of tissues with autologous fat grafting is feasible and simple to learn.
