Melasma in men

Objectives: Melasma in men is a under reported condition in the Indian scenario and the rising cosmetic awareness among the Indian men is highlighting the need for better understanding of the prevalence and possible causative factors of melasma in men.

Introduction: To study the prevalence of melasma in men with history of use of topical medications and cosmetics and identify areas of distribution in male patients

Materials / method: A total of 20 male patients with melasma and age ranging from 21-53 years were screened. Data included duration, sun exposure, application of cosmetics, mustard oil, topical medications and history of sunscreen use, systemic medications followed by general physical, cutaneous and woods lamp examination.

Results: 9 patients were outdoor workers, mean age of patients affected – 35 years. Positive history of use of cosmetic creams in 9 patients. History of taking irregular treatment with triple combinations in 7 patients. Area of face involved were noted as malar in 14 patients, centrofacial and mandibular in 3 patients each respectively. All patients denied daily use of sunscreens and were unaware of the protective role of sunscreens. No history of mustard oil use or intake of systemic medications present. Woods lamp examination revealed 16 patients with dermal and 4 patients with epidermal melasma.

Conclusion: Abuse of topical medications, irregular treatment and chronic use of cosmetics are identified to be one of the many causative factors for melasma in men with patients being affected as early as 21 years of age.

Type I leucocyte adhesion deficiency (LAD I): a familiar case

Objectives: Backgroung: Primary immunodeficiencies(PIDS)are rare,inherited diseases,characterized by altered function or absence of immune cells.Among them is leukocyte adhesion deficiency type I (LAD-I),a rare autosomal recessive disorder characterized by primary immunodeficiency,caused by mutations in the ITGB2 gene and characterized by inability of leucocytes to migrate from the circulation towards the area of inflammation and associated recurrent and life-threatening bacterial infections. Introduction: Observation: Here we present a yemenian family with LAD-I from consanguineous relatives.All patients had history of chronic recurrent skin ulcerations, without bleeding tendency, requiring steroids and antibiotics, associated with persistent neutrophilia, without history of delayed cord separation, initially diagnosed as epidermolysis bullosa, successively as PG. LAD-I should be kept in mind while evaluating patients with PG especially in children with persistent neutrophilia in the absence of other rheumatological disorders.

Materials / method: Keynote: Diagnosis of LAD-I is extremely important for management, as treating these patients without adequate antibiotic cover may be fatal, as happened to one of them, and these patients often require hematopoietic stem cell transplantation for permanent cure, therefore genetic counseling especially on consanguineous parents is mandatory.

Results: Key words: adhesion defects- bacterial infections- exome sequencing- leucocyte adhesion deficiency- primary immunodeficiencies.

Effective removal of a permanent tattoo with platelet rich plasma (PRP): a rare case report

Objectives: Permanent makeup is a rage amongst all age groups with varying degrees of success. Common modalities for permanent make up removal are Lasers, Dermabrasion, Chemical peeling and surgical excision.

The objective of this case report is to emphasize the efficacy of PRP combined with counselling for rectification of a permanent tattoo gone wrong.

Introduction: Case:
26 Year Old aspiring actress presented with hyperpigmentation, swelling, indurations, pain and ulcerations arising post Q-Switch laser sessions to remove excessive pigment from permanent blush.

Materials / method: Treatment Approach:
Oral and topical antibiotic therapy to allow natural wound healing process for a month followed by PRP sessions undertaken once every 3 weeks for 11 consecutive months resulting in >80% improvement in skin, tone and texture. Significant improvement in pigment density and lesion size observed. Patient has been on PRP maintenance therapy once every 2 months since the last one year. Further improvement noted.

Results: -Improvement in facial lesions observed from third session onwards
-Improvement with reduction in induration, Superficial Scarring, PIH
followed by erythema after third session
- Outcome Post 6th Session: Significant Improvement in Skin Thickness, Reduced Scarring. Concurrently Significant Improvement in Skin Pigmentation

Conclusion: PRP is indeed a high efficacy procedure with sustained effect and minimal adverse reactions for permanent tattoo removal. It can nullify the need for a surgical procedure. More studies are required to establish the same.

Melasma: a dermoscopic guide to treatment by energy-based devices in Asian morphology

Objectives: 1. To acknowledge dermoscopy as a more accurate tool to diagnose melasma than Wood’s lamp.
2. To learn how dermoscopy can categorize different types of melasma.
3. To help physicians to select appropriate energy-based devices for melasma therapy based on dermoscopic findings logically.

Introduction: Melasma is an notorious difficult condition to treat. Aesthetic physicians are using energy-based device to treat melasma despite the fact that it is an second-line treatment option for melasma. Melasma is divided into three main types which their clinical categorization always depends on Wood’s lamp. However, mixed melasma may be treated as epidermal melasma because Wood’s lamp may not pick up the dermal component of that mixed melasma lesion. Dermoscopy can be a more accurate tool for diagnosis and differentiation of types of melasma and so assist the choice of energy-based device therapy.

Materials / method: This is a review of 2 -years experience of how dermoscopy can assist diagnosis and treatment of melasma by energy-based devices for Fitzpatrick skin type III and IV (FST III and IV) patients in a single laser dermatological practice in Asia.

Results: Dermoscopy is more favorable than Wood’s lamp to differentiate different types of melasma (epidermal, dermal, mixed, vascular) in Asian by colour and pattern analysis so as to determine the best choice of energy-based device treatment. Dermoscopy is able to pick up the mimic of melasma such as freckle, solar lentigo or Hori’s nevus which then guides aesthetic physician to treat the particular lesions appropriately. Dermoscopy can also delineate the distribution of vascularity underneath the melasma lesion so to avoid high laser fluence resulting in post-inflammatory hyperpigmentation.

Conclusion: Dermoscopy can help aesthetic physicians to diagnose different types of melasma and guides them to select the appropriate energy-based device therapy.

Depigmentants update: tranexamic and cysteamine

Objectives: To describe and compare the newest depigment topic drugs used for melasma.

Introduction: Most of the villains in movies show dermatologic lesions. And out from all this lesions, the most common one is dark spots. So we may say the most common and first complain in consultation is melisma.

Melasma and hyperpigmentation are one of the principal negative reported causes of low self steem and affects in patients life quality, furthermore its treatment and evolution over the years has become hard as at the moment we don’t have a definitive treatment to clear melasma.

Materials / method: Clinical field experience with two of the newest therapies besides published papers.
At the moment the most trendy and common treatments count as, Lasers, peelings, intradermotherapy and topic use medication, but neither of these can achieve total results alone, its been always the combined procedures the approach needed in every case.

Results: Gold standard for melasma treatment has been Hydroquinone and Kligman Formula.

Tranexamic Acid.

Since the 70’s decades Tranexamic acid has been studied not only for its efficiency over melanine suppression but melanine degradation as well, besides acting on vascular changes in hyperpigmentations, and it became popular for treating melasma .Second, Cysteamine Chlorhydrate
First research on 1966 showed a powerfull depigmentant. in 2010, Cysteamine has been used all over Europe since 2010 and since 2017 In Latin America with good results.

Conclusion: Conclusions

We can conclude that Tranexamic acid used for treating melasma is nowdays one of the most effective therapies. And it could be used both, on clinics and by domiciliary treatments. Cysteamine Chlorhydrate as a novel treatment, is one of the safest, fastest and easies in home treatments that can be used by patients. Patients can notice excellent improve on skin quality by the fourth week and could maintain this results thru the year if good sun protection and home use products follow up is achieved.
Furthermore physicians could combine in clinic tranexamic with im home cysteamine.
