Objectives: This talk updates the management of difficult-to-treat scar, keloid.

Introduction: Hypertrophic scar and keloid are abnormal scarrings after wound healing. Hypertrophic scar tends to resolve but a keloid usually continues to grow. Present treatments include intralesional steroid, bleomycin, 5-fluorouracil, cryosurgery, surgical removal with/without post-operational radiation.

Materials / method: We emphasize a concept called “no inflammation, no keloid”. The average time for an asymptomatic (non-inflamed) keloid to resolve is 5 years. Endeavors to reduce inflammation in a keloid are the key to success for managing keloid, needless to say, in treating hypertrophic and normal scar as well.

Results: The algorithm in both medications and surgical approach, especially to control recurred abscess in a keloid will be introduced.

Conclusion: Treat the inflammation, either with medications and/or surgery, are critical for success in managing a keloid.

Objectives: Deep depressed scar, especially as boxcar shaped acne scar, is difficult to be well treared, either surgical excision or Laser dermabrasion can’t completely seal off the scar, without residual deformities.

Introduction: We developed a novel strategy for deep depressed scar management, to effectively solve the problem.

The strategy including two stages :
First Stage :
Deep depressed scar shallowing procedure. It is subdivided into 3 sugical procedures :
a) Scar Subcision.
b) Inner purse string suture.
c) Dermal grafting.

Second Stage :
Laser Dermabrasion, 3~6 month later

Results: During the past 5 years, 56 patients were treated following the strategy in our clinic, the satisfactory rate is high, over 80%, there are no any significant complications, such as scar hypertrophy, hyperpigmentation, cyst formation or infection noted.

Conclusion: Strategy of deep depressed scar management is practically a novel and effective management, for all deep acne and other depressed scars.
