Objectives: The objective of the study is to analyze aesthetic outcome and downtime of full face armouring procedure with absorbable threads in copolymer of PLLA/PCL compared to the new version of the same threads enriched in HA.

Introduction: Non surgical tissue reposition with absorbable threads is safe, effective and minimally invasive procedure. However, compared to other aesthetic treatments, like filler or botox injections, sometimes there is a little longer downtime. Recently a new version of absorbable threads in copolymer of PLLA/PCL enriched in HA was created in order to provide less traumatic treatment with shorter downtime.

Materials / method: 8 patients (7 F, 1 M) 42-60 y. o. have undergone full face treatment with absorbable threads in copolymer PLLA/PCL with multidirectional barbs. One side of the face was treated with traditional threads in PLLA/PCL, another side was treated in the same way but with threads enriched in HA. Follow up at 2 weeks, 1 month and 3 months with subjective evaluation by patients and objective evaluation by means of 3D LifeViz system for skin parameters and volume analysis.

Results: The totality of the patients reported shorter downtime and less discomfort at the face side treated by threads enriched in HA. The parameters evaluated by means of 3D reconstruction showed volume redistribution with statistically significant volume reduction in the lower third of the face and volume increase in the medial third without significant differences between two hemifaces. The skin parameters evaluated by means of SkinCare software showed more significant improvement at the side treated by threads enriched in HA.

Conclusion: The absorbable threads in copolymer of PLLA/PCL are an excellent device in skin ptosis treatment and volume replacement. The new version of these threads enriched in HA has an additional advantage to reduce downtime and accelerate the repairing and regenerating processes.

Objectives: To know various ways of Double Chin and Jaw Line Contouring .

Introduction: Background
Double chin is found in 77% of patients visiting Aesthetic doctors and 61% of them are interested to treat it.

Materials / method: From September 2016 till November 2018 596 cases of jaw line contouring ( of which 181 by silhouette threads alone, 259 by filler alone and 156 by threads and filler). All works were on jaw line without any intervention in the neck. Before and after photos were taken.

Results: 50 % improvement in the double chin in those cases of threads alone and 30% in cases of filler alone and 70% in cases of thread and filler together.

Conclusion: For satisfactory esthetics outcome mandibular jaw-line contouring
is a must to obtain a sufficient esthetics outcome for double chin
treatment and alone without working in the neck could give
satisfactory result.

Objectives: We already have enough experience in our clinics with the use of surgical sutures, applied in order to reposition and lift the middle and lower area of the face, with very satisfactory results, but with the disadvantage that it was a pseudo-surgical technique difficult to perform, due to the fact that the fixation area had to be periosteal. With this new technique, we may obtain very similar results in terms of effectiveness, as positive as those obtained with tensioning with Serdev threads, but in a much easier, less problematic and less traumatic way, since periosteal anchoring is not needed

Introduction: We already have enough experience in our clinics with the use of surgical sutures, applied in order to reposition and lift the middle and lower area of the face, with very satisfactory results, but with the disadvantage that it was a pseudo-surgical technique difficult to perform, due to the fact that the fixation area had to be periosteal.

Materials / method: On the basis of such backgroud, we have developed the BQ technique, which by definition is based on the use of bidirectional threads, with spicules, cones and other means of fixation, with double needle, being introduced in at least 2 entry points, and directing them to a single exit point, or 2 very close points, located in a more elevated area.
The part of the thread that joins the 2 entry points will tighten the area.
The ends of the thread will knot if the exit point is common.

Results: This way, we obtained very similar results in terms of effectiveness, as positive as those obtained with tensioning with our surgical threads, but in a much easier, less problematic and less traumatic way (since periosteal anchoring is not needed).
In order to prove the effectiveness of the technique, we performed it on a portion of pork skin, it was kept refrigerated hanging only by the points of the elevation, with satisfactory results after more than 2 weeks after its application.

Objectives: To develop a technique for nonsurgical lip augmentation using fillers to reliably and consistently produce subtle, beautiful natural looking results

Introduction: The lips along with the eyes are arguably the most important aesthetic features when it comes to judging beauty. It is no surprise then that lip procedures involving dermal fillers rank as one of the most sought after nonsurgical treatments. However, the lips are probably the most challenging procedure to perform as they require an artistic acumen with great technical skills. As a result, there seems to be a shortage of subtle and natural looking results with over-reliance on injecting the vermillion border and the unsightly 'duck look' or 'sausage lips' all too common.

Materials / method: The classic NLL involves ten steps with the typical amount of filler used between 0.05-0.1ml per step (total 0.5-1ml). The technique involves subtly augmenting the naturally occurring upper and lower tubercles of the lips along with injections under the vermilion border (a new curved threading technique) to increase vertical height rather than over-project outward. All injections are performed safely; anteriorly and superficially.

Results: The NLL reliably produced beautiful and subtle natural looking results, augmenting and balancing the natural anatomy of the lips with emphasis on vertical height rather than outward projection. An audit on the NLL found a significant reduction in incidence of bruising using this technique.

Conclusion: In conclusion, the NLL is a relatively safe and effective technique which makes use of the natural anatomy of the lips to gently and subtly augment them. It enhances definition, shape, symmetry and proportions without compromising the vermillion border. When performed correctly the NLL leads to consistently subtle and beautiful natural looking results with a very high patient satisfaction rate.

Objectives: Since 2015 we tuned a new technique under local anesthesia extremely safe, fast and effective. Our objective is to verify the medium term result of this innovative technique.

Introduction: The mandibular area is very often the first showing the aging process of face contour.
It’s a common request by females after 40 years to improve such profile to give to face a younger aspect. Furthermore most patients request minimal invasiveness procedure.
The answer offered by the insertion of PDO and similar threads has shown enormous limits in exerting an effective and stable traction.
I perform this sort of mini lifting using minimal incision and permanent threads.

Materials / method: Through a 1,5 cm incision and a small undermining we operate a traction directly on sagging SMAS using a single permanent thread. The whole procedure request 20 min and is performed under local anesthesia. A careful application of anesthesia allow not to create any hematoma.

Results: We checked 29 of our 48 patients treated by this technique after 2 and 3 years. 85% of them referred to be still satisfied of the results. We had negligible complications.

Conclusion: In selected patients this technique of face mini-lift is an efficient approach as a minimal invasive procedure. It permits, with one surgical time, to obtain satisfactory results with a soft natural appearance, respecting the natural tension lines of the facial tissues. Patients’ selection is important to obtain the requested result, and the use of a permanent nylon thread resulted well accepted by tissues.

Introduction: Hands ageing process is very similar to the face one, resulting mainly from fat atrophy skin atrophy and thinning, with more visibility of the underlying structures like the vines and tendons

Materials / method: The significant difference between the two anatomical areas is function: the hand is a highly movable structure, function overcoming the aesthetic appearance. Consequently that the anatomical knowledge is fundamental and the respect for the more movable stutters fundamental. For this reason, the volume replacement with bio-stimulatory fillers becomes an advantage: increases the longevity of the result, reduces the frequency of the treatment sessions, reduces the complications related to migration, improves the skin quality offering an overall better result.

Conclusion: The author will share experience using a Calcium hydroxylapatite (CaHa), and a polycaprolactone (PCL) based filler, comparing the results concerning longevity and volume, and the guidelines for safeguarding the safety and avoiding complications
