Objectives: The purpose of this study is to clarify which plane actually moves the most during facial lifting. Based on this, the authors attempt to find out how to effectively move and fix the soft tissues of the face.
Clinicians can tell which parts of the face have moved when they do the face lifting. and can also understand which method to use for effective face lifting.

Introduction: Recently, however, non-surgical lifting has become popular due to changes in beauty trends. There are several nonsurgical lifting methods. One of them is thread lifting. It is also true that there are many debates about whether thread lifting has a lifting effect. We want to understand the mechanism and see if thread lifting can actually implement it. If the mechanism can be implemented through thread lifting procedures, thread lifting will have a lifting effect. Based on this understanding of the moving plane, we will learn how to move and fix the soft tissues of the face

Materials / method: While moving various parts of the face, check the moving plane using ultrasound at the same time. Set up the experiment site, taking into account the ligament system on the face. During the lifting procedure, check the moving plane by using ultrasonic waves for each part such as the temples, the eyes, and the cheeks. Investigate how to design and insert threads to create and fix moving plane movements.

Results: Experimental results show that when moving each part of the face, the most moving part is the deep fat layer (layer4) or loose connective tissue (layer 4). The deep fat layer (layer4) or loose connective tissue (layer4) area was most active in the temples, under the eyes, and in the lateral cheeks. Next, the superficial fat layer (layer 2) can also be observed to move slightly when lifting the face.

Conclusion: For effective face lifting, the movement of deep fat layer (layer 4) or loose connective tissue (layer 4) must be produced. The authors believe that the procedure of simply injecting a thread into the superficial fat layer should be understood as soft tissue redistribution rather than true lifting.

Objectives: The aim of this presentation is to locate an effective and safe layer for PDO thread lifting and to identify it in real time using ultrasound.

Introduction: Nowadays, minimal invasive procedures such as injectables have been gaining huge popularity. Because they have advantages of simple and minimal downtime and complication. In the meantime, simple PDO thread lifting using short length and cannula inserted type is popular. However, a right layer is crucial to reduce downtime and complication and to increase efficacy. Mostly thread lifting is performed blindly, so it is hard to identify where to be inserted.

Materials / method: During the thread procedures, ultrasound machine was used to identify which layer thread is inserted. Thread is inserted in various vector. Each directions, normal anatomy was checked and ultrasound guided thread insertion was performed.

Results: Cannula inserted thread was identified to enter subcutaneous layer or SMAS layer through ultrasound images.

Conclusion: In order to achieve optimal result and to minimize complications, thread insertion to the right layer is important. Real time ultrasound is useful to identify a right layer for PDO thread lifting.

Objectives: 1. Understand the concept of reposition of the buccal fat pad
2. To learn the safest and the most practical way to perform the deep plane thread lift for buccal fat pad reposition

Introduction: Management of facial rejuvenation by the thread lift has evolved over past few years. The role of the deep plane thread lift for buccal fat pad reposition was advocated. However, the risks and the feasibility of the deep plane thread lift are concerned. This study was designed to determine whether the deep plane thread lift could achieve effective aesthetic result and to investigate the possible risks of critical tissue injury through cadaveric study.

Materials / method: Twelve fresh frozen cephalic specimens of 8 male and 4 female Asian body donors (mean age, 63.3 ± 8.0 years) were investigated. The deep plane thread lifts for reposition of the buccal fat pads were performed for all the left hemifaces. Cadaveric dissections were performed to investigate the moving distance of the buccal fat pad and to examine surrounding tissue of the passage of the deep plane thread lift.

Results: The average moving distance of the buccal fat pads after the deep plane thread lift is 3.73cm. The difference in moving distance of buccal fat pads between bilateral sides was statistically significant (p < 0.001). There are no injuries of the critical vessels or nerves found after cadaveric dissection. The passage of the deep plane thread lift was evaluated.

Conclusion: The deep plane thread lift for reposition of the buccal fat pad is a safe, effective and practical method.
