The emerging world of bioabsorbable threads: anatomy, techniques and results

Objectives: Thread lifting for face rejuvenation using bioresorbable threads are considered satisfactory and has significant long-lasting effect that include skin lifting. The mechanical action produced by the threads give an immediate lifting effect of the tissue, which contrasts the falling of the area treated. The type, form, length of thread and the technique used during the procedure are key factors for optimum facial rejuvenation results. The 3 types of material in threads induced comparable, length of time for degradation levels of collagen. The extent of collagen formation appeared to be dependent

Introduction: Demand in minimally invasive method for the improvement of age-related soft tissue changes of the face and body is gradually increasing. Thread lifting is a cosmetic procedure that lifts and realigns sagging tissue to enhance the facial contour, tighten the tissue and volumize the area of application. These threads are classified according to its absorbability, form (barbed or non-barbed) and length (long or short). Commercially available threads are composed of biodegradable, bioresorbable, and synthetic polymers such as Polydioxanone (PDO), Poly-Lactic Acid (PLA) and Poly ε-caprolactone(PCL)

Materials / method: Through systematic research and actual clinical experience, references in thread lifting using biodegradable threads for facial rejuvenation were undertaken. Evaluated review articles include the function, formation, composition, length, efficacy, durability, the biocompatibility of absorbable threads, and the basic techniques of thread lifting for facial rejuvenation.

Results: Thread lifting for face rejuvenation using bioresorbable threads are considered satisfactory and has significant long-lasting effect that include skin lifting. The mechanical action produced by the threads give an immediate lifting effect of the tissue, which contrasts the falling of the area treated. Conclusion: Thread lift using the biodegradable and bioresorbable threads has proven to be effective for lifting and long term facial rejuvenation and could be of great alternative for invasive procedures. Different threads and specific techniques differ per case. The type, form, length and composition of threads together with an in-depth assessment should be considered necessary before conducting any thread lifting procedure.

M.U.S.T. upper blepharoplasty approach with sub-eyebrow skin incision and OM plication technique in cosmetic oculoplasty

Objectives: As with any procedure, upper eyelid blepharoplasty requires a thorough preoperative evaluation of the patient. Dermatochalasis may result in functional visual field obstruction or may create an aesthetically displeasing redundancy and excessive skin protrusion to the upper eyelids. In some cases, the traditional incision of the upper eyelid can not be hidden, and during the time the postoperative white scars may be obviously visualized. According to my surgical experience, the sub eyebrow incision could be a better solution for patients with excessive protruding upper eyelid and high brows position.

Introduction: Having obtained good results with the M.U.S.T. FACELIFT approach (Minimally invasive Undermining with Suspension using silicone elastic Threads) in rejuvenating face surgeries, I decided to adapt this technique into eyelid surgery practice for using in a less invasive way and more natural look achievement. The M.U.S.T. Upper/Lower Eyelid Blepharoplasty benefits significantly from traditional blepharoplasty in terms of protocol simplification and invasiveness reduction.

Materials / method: In contrast to traditional Upper Eyelid Blepharoplasty, the skin undermining makes in a less invasive way by using a special plane-tip blunt cannula. Moreover, the area of subcutaneous undermining might be more extensive than in a routine Upper Eyelid Blepharoplasty, in a case when it is required. It is absolutely safe and easy to perform. The M.U.S.T. Upper Lid Blepharoplasty incision makes direct beneath the lower border of eyebrows along the line going from the head to tail of brows on each side. In benefit, it subsequently allows achieving inconspicuous scar direct under the eyebrow and no any scars on eyelids at all. In a month, postoperatively patients successfully use brow make-up or permanent eyebrow tattoo for scars concealing.
After subdermal skin undermining pinch-test easy demonstrates how much skin should excise.
OM plication with small amount lateral part resection performed routinely by many cases to create a deep upper eyelid hollow with a high crease and slightly elevated tail of eyebrows.
In my practice, nasal and preaponeurotic partial fat pad removal also could be made by transconjunctival upper lid approach.

Conclusion: So the M.U.S.T. Upper Eyelid Blepharoplasty technique becomes much safer, more straightforward to perform and substantially less invasive; the procedure requires only and local anaesthesia or light sedation, the rehabilitation period is shorter and less morbid for patients

Outcomes in thread lift for face and neck: innovative and classic techniques

Objectives: We conducted two separate studies with the objective of evaluating the effectiveness and outcomes of thread lifting surgery and to evaluate our innovative techniques for thread lifting from January 2016 to May 2017.

Introduction: Thread lift is a new mild invasive developing procedure that can be done to attenuate facial and neck deformity secondary to aging process ,trauma and congenital malformation .It seems to be going to replace other more invasive surgical methods due to its numerous benefits such as short down time ,few complications and no need for hospitalization.

Materials / method: In the first study we performed the thread lifting on 193 volunteers between 25 -84 years old based on different techniques of thread lift with silhouette soft or promo double needle happy lift(12 and 23).In second study,a total of 151 patients(27‐78 years),underwent thread lifting using our innovative thread‐lift techniques for head and neck with PDO threads.The patients were followed up first week, first, third, and sixth month after lifting surgery. Outcomes were assessed by two surgeon and patients based on the GAIS score. All data were analyzed by SPSS‐17 software with chi‐square test.

Results: IN both of study the most common sites for the lifting surgery were the jawline , mid face , eyebrows, and neck respectively. The level of satisfaction increased from first week after surgery to sixth month after surgery for patients, surgeon and independent surgeon . Based on the findings, the highest level of satisfaction of the patients, surgeon 1, and surgeon 2 was for the jawline site at all times studied. The highest rate of potential complications after the lifting surgery was related to ecchymosis , followed by tumefaction , pain and dimples.Complications reported more in females

Conclusion: The results showed that the level satisfaction of patients and surgeons increased over time. In addition, the complications developed after surgery, which were carefully assessed by the surgeons, were for the most part minor, self-limiting, and short-lived. These results demonstrated that facial thread lifting could be cosmetically more acceptance, safe and effectiveness procedures associated with minor complications,also It seems that thread lifting with our innovative techniques is effective, safe, and cosmetically more acceptance with minor complications than other face‐lifting techniques.

Threads rejuvenation and hormonal imbalance

Objectives: Women threads treatment with hormonal imbalance

Introduction: From the age of 35, many women experience hormonal imbalance in their body that affect the early decrease in collagen synthesis, it also lead to increasing of facial ligament ptosis, SMAS, reduces thickness of the subcutaneous fat layer, wrinkles and signs of aging in general.

Materials / method: The effectiveness of the face thread rejuvenation and the duration of the results obtained depend primarily on the reparative potential of tissues which is controlled by the level of hormones and the activity of steroid receptors. Depending on the level of hormonal imbalance and methods of thread rejuvenation, women results of thread lifting are different.

Results: By mixing the right technique with the most functional materials, I developed the Luxeface concept that showed the world’s best long term results of skin lifting & its regeneration. In this case, the criteria for the selection of patients and their hormonal correction allows to achieve an optimal and long-lasting result of thread rejuvenation.

Conclusion: Our 10 years of experience with the LUXEFCE facial and body threads rejuvenation method has allowed us to accumulate sufficient experience to appreciate the importance of preparing facial tissues and to gather best results.

PDO threads: avoidable complications

Objectives: The objective is to establish the frequency and repetition of adverse effects, complications, inherent to the product, to the tissue reaction or to the technique and to establish which complications are avoidable. The results conclude that most of the complications are due to defects of the technique used and, therefore, are in most cases, avoidable.

Introduction: Polydioxanone sutures, known as PDO threads, have been used for 7 years, in Aesthetic Medicine to improve skin quality as well as non-surgical facelift. The first sutures used were smooth and sutures with spicules, screw, needle/cannula guide, special nose or dark circles were incorporated. Several studies have been published about the safety and effectiveness of the application. This study collects cases that have reported complications over the years, from the author herself, as well as reports from other clinics, or other countries that have shared their cases with the author.

Materials / method: It is a retrospective descriptive study performed on 100 cases. The information processed are cases reported by the author and other colleagues i that describe complications after the use of PDO threads. The cases whose information was incomplete were discarded. The inclusion criteria were description of the material used, description of the technique, if there were associated treatments, relevant patient data, and the resolution.

Results: 20% related smooth threads, the most frequent thread visibility, thread in the oral cavity and the migration of the thread.
The 80% reported spiculated threads, the most frequent a superficial point where the thread was touched and, in most cases, the need was abcesified. Retraction, skin pucker. Only in one case in one case nasal infection.
The resolution of the problem was spontaneous in half of of the cases and the extraction of the thread in the rest was appreciated. The treatments applied were enzymatic, orally and topic, corticosteroids and antibiotics.

Conclusion: In all cases happened an inadequate insertion into the skin layer. The visibility of the thread, retraction, migration and abcesification were associated with a very superficial application, in the epidermis or superficial dermis.
The insertion layer of PDO threads is hypodermis.
The evolution went to the resolution within 15-30 days but exceeded in a case in which the thread was not removed after retraction and took three months.
In conclusion, all complications would have been avoidable if an appropriate technique had been used, so training in the use of PDO threads is essential.

Absorbable threads: focus on complications and their management

Objectives: This Lecture aims to demonstrate the presenter’s own experience with different types of threads. It discusses possible side effects and educates about prevention and management of these side effects.

Introduction: The procedure of Non surgical thread face lifting using absorbable suture has risen during the last decade. The main reason is that patients are looking for less invasive and less painful procedures. The concept of global face sculpturing approach using different aesthetic modalities played a role as well. The threads are inserted into the skin, then they are “hooked“ to the skin through barbs or cones.

Results: There are different types of suspension threads that are available in the market. The choice of which thread to be used depends on the doctor experience and preference and on specific patients- related needs.

Conclusion: Common side effects include mild pain, edema, erythema and bruising. Avoiding serious side effects depends highly on proper training and following the guidelines set by the suture manufacturer.

Complications from absorbable threads following thread lift for facial rejuvenation

Introduction: Nowadays, thread lifting is favoured as a minimally invasive alternative to surgical lifting.

Materials / method: Absorbable suspension sutures are the first minimally invasive, entirely absorbable treatment option for tissue repositioning and recontouring that can be performed under local anaesthesia and which demands very little patient downtime.

Conclusion: But some complications are reported and include thread disruption, thread migration, cutaneous exposure, and skin dimpling. Thread removal and reimplantation of additional threads was occasionally required.
