Comparison of efficacy of 40% mandelic acid with 30% salicylic acid peels in mild to moderate acne vulgaris: a randomized study

Objectives: Comparison of efficacy of 40% MA with 30% SA peels in patients suffering from mild‐to‐moderate facial Acne vulgaris.

Introduction: Chemical Peel is a cosmetic procedure that is becoming a popular modality in treating acne vulgaris (AV). Mandelic acid (MA) is an upcoming peeling agent for AV due to its anti‐inflammatory and antibacterial traits. Hence, it is worthwhile to appraise this newer agent's effectiveness and safety profile and compare it with a more traditional and established peeling agent, salicylic acid (SA), in the treatment of Acne vulgaris.

Materials / method: One hundred patients suffering from mild‐to‐moderate facial Acne vulgaris were distributed randomly into two groups of 50 each, with group A receiving 40% MA peel and group B receiving 30% SA peel at an interval of two weeks for six sessions. The duration of the study was twelve weeks. Clinical pictures and Michaelsson acne scores (MAS) were used to evaluate the effectiveness of treatment objectively. Adverse effects of both the peeling agents were also noted.

Results: Overall, there was no significant difference in the efficacy between the two peels. However, adverse effects were slightly higher with SA peel.

Conclusion: The 40% MA peel was equally effective as 30% SA peel in mild‐to‐moderate facial AV. However, safety profile and tolerability were better in the MA peel group than the SA peel group.

Tranexamic acid: an old drug for new indications in dermatology

Objectives: There are more and more frequent publications about the use of tranexamic acid in rosacea. To date all published studies were on Asian skins.
We intend to disclose the results of a clinical trial conducted in Mexico with topical tranexamic acid in patients with rosacea grade1/2.

Introduction: Topical tranexamic acid (TTA) is mainly used in the treatment of melasma.
Fewer reports exist about its use in erythematotelangiectasic rosacea.
Further in this indication only exist reports in Asian skins.
For this reason we conducted a clinical trial on Latin American skins in Mexico.
The results will be disclosed here for first time.

Materials / method: 30 patients (males and females) with bilateral lesions of erythematotelangiectasic rosacea treated in a split-face protocol on one side with 3% TTA cream + photprotector SPF 50 and on the other side with photoprotector SPF 50 alone. 2 daily applications . Duration 1 month.
Photos with artificial light taken at D0 and D30.
Evaluation at D0 and D30 of erythema, telangiectasia, flushing, itching and burning.

Results: Main criterion
Percentage of positive responses at D30 vs. placebo, defined by an improvement in at least three of the five evaluated characteristics (erythema, telangiectasias, flushing, itching and burning) = 56.7% vs. 20.0% (p<0.0001)

Conclusion: Topical tranexamic acid constitutes an efficient and safe alternative in the treatment of erythematotelangiectasic rosacea.

Acne scars: combined treatment

In this combined therapy we can use all the benefits of several methods to achieve the best results

Introduction: This method is a kind of combined therapy that can use for acne scars treatment.
In this technique I have used subcision ، collagen stimulator filler،peeling ،non cross linked HA mesotherapy ،home used topical creams.
Due to this combined therapy،the treatment has been done in two layers : 1-deep dermis 2- superficial epidermis.
Subcision : minor surgical procedure that used for treating depressed cutaneous scars.
Collagen stimulator filler : this type of filler contain PLLA (poly-L-lactic acid) that is bio-stimulator & can rejuvenate & volumizing the skin.

Materials / method: At first we should exam the face carefully till determine the type & shape of scars so then planned the treatment.
The best strategy for this combined therapy is that at first we do subcision with collagen stimulator filler،then during 3 month we do peeling with TCA(20% -40%) every month.
During this 3 month we can use non cross linked HA for acceleration of repairing process.
As you know topical creams can help us to protect from treated skin.
After 3 month we evaluate our case & then decide for next 3 month period if she/he needed

Results: During 3 years I have used this combined therapy for about 300 cases & I have achieved wonderful results versus lasers & other techniques.
I have experienced that for acne scar treatment we need combined therapy & single therapy can not satisfy our cases.
During this therapy we need our patients accompany، so that will be happen if we explain the therapy process completely at first session.
in my experience this therapy period can be repeated for 4 times.

Conclusion: This method is the best method for acne scars treatment due to combined therapy & two layers therapy(deep dermis – superficial epidermis) simultaneously.
This is very important that we can heal the acne scars from deep to superficial & this raise our results.
As you know there are many cases that during single therapy the healing process has been stopped so the best solution is combined therapy & this strategy that I have experienced is the best due to using several methods & two layers therapy simultaneously

Acne and scarring: current knowledge and innovations

Objectives: This presentation discussed and analyzed the various minimally invasive approaches to the treatment of acne scarring, innovative modalities, and different protocols for early intervention to minimize scarring formation.

Introduction: Acne is a common disease affecting a high percentage of the younger population. Without appropriate and effective primary prevention of scarring, post-acne scars occur in about 80-95% of all patients. Acne scarring is the result of an alteration of the healing process, and it can have deep psychosocial implications for patients. Scars can involve textural change in the superficial and deep dermis, and it can also be associated with erythema or pigmentation.

Materials / method: The presentation explored and analyzed different recent studies regarding new treatment, combination, and early intervention to minimize scarring based on many publications and consensus

Results: The efficacy of various treatment modalities were highlighted with a focus on choosing the correct modalities for specific scar types.

Conclusion: Good assessment is very important to select the proper and specific treatment for each type of post acne scar. Combination treatment has shown better efficacy compared to single modalities.

The combination of niacinamide, vitamin C, and PDRN mitigates melanogenesis by modulating nicotinamide nucleotide transhydrogenase

Objectives: I evaluated whether a mixture of nicotinamide, vitamin C, and PDRN (NVP-mix) decreased melanogenesis by modulating mitochondrial oxidative stress and NNT expression in UV-B-irradiated animals and in an in vitro model of melanocytes treated with conditioned media (CM) from UVB-irradiated keratinocytes.
Thus, I planned this study to clinically apply anti-melanogenesis proven material to patients with melasma and PIH.Based on this study, I am clinically applying it to existing whitening treatments such as micro-needle RF and 1064nm Q-switch laser toning.

Introduction: Nicotinamide nucleotide transhydrogenase (NNT) is involved in decreasing melanogenesisnthrough tyrosinase degradation induced by cellular redox changes., Nicotinamide nucleotide transhydrogenase (NNT) was recently shown to be involved in melanin synthesis in the skin. ctive oxygen species (ROS)-induced injury. NNT induces cellular redox changes that lead to tyrosinase degradation and melanosome maturation modulation to decrease eumelanin levels.Vitamin C and polydeoxyribonucleotide (PDRN) are also known to decrease skin pigmentation.

Materials / method: We evaluated whether NVP-mix modulated NNT in UV-B-irradiated animal skin. The expression levels of NNT in the epidermal tissues of the control animals (controlgroup), UV-B-irradiated animals (UV group), and NVP-mix-applied UV-B-irradiated animals(UV/NVP group) were compared. Notably, the mice in the UV group and UV/NVP group were exposed to UV-B irradiation every 2 days until the end of the experiment. Fifteen days after the start of the experiment, the UV/NVP group was administered 50 µL of NVP-mix per square centimeter of skin at 7-day intervals via a microneedle therapy system (MTS).

Results: The expression of NNT in the epidermises of the animals in the UV group was
significantly lower than that in the control group, whereas the expression of NNT in the UV/NVP group was significantly higher than that in the UV group. The GSH/GSSG ratio in the mitochondria from the skins of the animals in the UV group was significantly lower than that in the control and UV/NVP groups. The mitochondrial NADPH/NADP+ ratio in the skins of the mice in the UV group was significantly lower than that found in the control and UV/NVP groups.

Conclusion: In conclusion, NVP-mix decreased mitochondrial oxidative stress by increasing NNT expression and decreased melanogenesis by decreasing MC1R/MITF, tyrosinase, TYRP1, and TYRP2. NVP-mix decreased melanogenesis signals, such as MC1R, MITF, TYRP1, and TYRP2, and decreased melanosome transfer-related signals, such as RAB32 and RAB27A, in UV-B-irradiated animal skin. NVP-mix also decreased MC1R, MITF, TYRP1, TYRP2, RAB32, and RAB27A in melanocytes treated with CM from UV-irradiated keratinocytes.

Melasma protocols and the latest updates: exosomes and hiperdiluted toxin, the new future for pigmentation?

Objectives: 1.- To stablish the efficacy of Exosomes and / or Hiperdiluted toxin as a new complement in the treatment protocols of melasma

Introduction: Treating Melasma is a challenge for dermatologists , therefore we are trying to find new techniques to fight agains pigmentation everyday . Lately many colleagues have described their positive experiences with Hiperdiluted Toxin and Exosomes in the treatment of pigmentation and skin texture , but , do we have enough cases to proof this ?
Should we add these 2 new components in our protocols ?
Is it really worth it ?

Materials / method: During the Lecture we will present a series of cases that have been treated with my Qswitched Laser protocol but also with Exosomes and Hiperdiluted Toxin so that we can analyze the results .

Results: Hiperdiluted Toxin has a positive effect on the vascular component of the Melasma .
Exosomes have a positive change in Skin texture .

Conclusion: Hiperdiluted Toxin has shown positive changes in the vascular component of the Melasma therefore it should be considered as a new complement in the therapy of Mix Type Melasma with Vascular Component .
Exosomes have shown to have positive results in the skin texture but not directly in the pigmentation , however , considering melasma has multiple causes like free radicals and aging , adding Exosomes into the therapy will give us a smoother and healthier looking skin that will be appreciated for our patients diagnosed with Melasma .

Treatment modalities for hyperhidrosis, wounds, and burns

Objectives: 1. Discuss treatments for hyperhidrosis with clinical trials to support the different theoretical backgrounds.
2. Evaluate different procedures for second and third degree burns.
3. Assess different therapies for skin lesions, especially diabetic neuropathic wounds which is one of the most difficult to treat, with high levels of reoccurrence and a substantial increase with aging.

Introduction: Diabetic wounds result in expensive hospitalizations, often resulting in an amputated lower limb. We explored a variety of methods such as RF, low-level laser, monochromatic near-infrared treatment, TENS, acupuncture and pulsed electromagnetic fields that demonstrated inconclusive, limited or temporary wound repair with minor or short-term improvements. We also explored low energy technologies that seemed to offer long lasing results with no re-occurrence.

Materials / method: Research conducted by ultra-low energy technologies also reports a fast improvement in hyperhidrosis, burns and pain relief. Medical research reports a reduction of inflammation as a result of anti-oxidant electron donation transforming free radicals into stable molecules. We report the results of clinical studies on burns and hyperhidrosis and a randomized double-blind one-year-long longitudinal clinical study on skin lesions. Hyperhidrosis patients experience fast relief from their symptomatology with no side effects.

Results: Burns patients reported significant improvement and no scarring. The statistically significant results on wound repair and pain analgesia and improvement in neuropathic symptomatology were not age contingent. This contradicted past research postulating that age-accumulated inflammation and endothelial dysfunction can further exacerbate diabetic neuropathy. Importantly, a method offering age-independent, cost-effective, long-term neuropathic pain relief and increased mobility, fast relief from hyperhidrosis and substantial skin healing after severe burns has major implications

Conclusion: by offering a solution that enhances quality of life.
1. In hyperhidrosis, getting rid of the sweat glands that eliminate the symptom expose the patients to possible health deterioration due to the inability to sweat during the course of physical illness. Safe modalities that take into consideration the patient’s health must be considered.
2. Evaluation of different methodologies for wound healing should consider the degree of relief and symptom reoccurrence after treatment completion
3. Trauma-based tech may not be indicated for lesions or burns due to additional inflammation + complications

Potential role of exosomes in wound healing

Objectives: Facial/lip injuries after a dog bite can cause major tissue loss resulting in disfiguring scars. Adjuvant topical exosomes therapy on the primary wound closure area will accelerate wound healing time, reduce early and delayed complications and achieve an aesthetically pleasing scar.

Introduction: Exosomes are extracellular microvesicles enclosed by a lipid membrane 30-150nM which carry an abundant amount of growth factors and signaling mediators that enhance all 4 phases of wound healing and scar formation.
Facial injury from dog bites may result in major tissue loss and is a frequent reason for ER visits. Every dog bite is unique and always contaminated, is challenging for surgical repair, and pose a high risk for aberrant scarring and disfigurement. Topical exosomes after primary wound closure is an adjuvant to achieving aesthetically pleasing scar.

Materials / method: We report the case of a 49-year-old female who suffered from a serious dog bite on her lower lip. She went to the ER for debridement and primary wound closure and antibiotics. The patient was seen in the clinic 14 hrs later and wound clot and debris were removed with hypochlorous spray. The MSC-derived exosomes of were topically dripped on the sutured wound using an insulin syringe for over 15 minutes. Daily evaluation of the wound site, signs of infection, pain, itching, and redness were noted by both the patient and the physician and Photographs taken.

Results: Exosome-assisted wound remodeling, rapid reepithelialization, and attenuation of inflammation result in less aberrant scarring, fibrosis, and almost perfect scarless wound. Our patient showed a significant improvement on Day 10 almost no scaring in 12 months. She has not had any side effects and did not need any scar revisions after 18 months.

Conclusion: Early application of topical exosomes on the primarily closed wounds after any animal bite-induced facial injury can be safe with no side effects and potentially result in scarless wounds. The exosomes may add the benefit of a reduced rate for scar revisions and aesthetically pleasing scars.

Successful treatment of corticosteroid-induced cutaneous atrophy and dyspigmentation with Heterologous Type I Collagen (HTIC)

Objectives: We sought to evaluate the efficacy, tolerance and safety of Heterologous Type I Collagen for the treatment of steroid-induced dyspigmentation and cutaneous atrophy due to intralesional triamcinolone treatment of a lateral epicondylitis.

Introduction: Steroid-induced atrophy is a well-known complication of intralesional steroid use and, although typically self-limited, can be distressing to patients. This case report describes the successful treatment of cutaneous atrophy and dyspigmentation on the elbow joint secondary to intralesional steroid injections for lateral epicondylitis using Heterologous Type I Collagen (HTIC).

Materials / method: A 26-year-old woman with Fitzpatrick skin type IV presented with a steroid-induced cutaneous atrophy and dyspigmentation was treated with Heterologous Type I Collagen (HTIC) intradermal injections. 100mg of HTIC sterile powder, dissolved in 5ml of normal saline was injected to the affected area. The patient received 4 treatments, once every two weeks.

Results: The area treated with HTIC demonstrated skin quality improvement after the first session and complete resolution of the lesion after the fourth session. The patient reported minimal to moderate pain and there were no side-effects.

Conclusion: HTIC is a potential new treatment option for steroid- induced cutaneous atrophy and dyspigmentation. The treatment is highly efficient and safe for use.
Further assessment with more extensive clinical trials are necessary.

Innovative revision of hypertrophic and keloid scars - A fusion of skin needling and drug delivery

Objectives: The objective is to deliver an innovative and comfortable combination therapy that targets and treats hypertrophic scars on the face and body. The non-thermolytic and non-ablative technique is suitable for all Fitzpatrick skin types and ethnicities, especially for patients with darker skins. The procedure offers fast and effective results with minimal downtime. The combination therapy furthermore offers an affordable alternative to traditional scar treatments with less complications and discomfort. Automated technology delivers expert precision and administration.

Introduction: As the third most common reason patients seek an aesthetic solution, the scar revision market is estimated to reach a value of USD$34.5 billion by 2025. Patients of Asian, Middle Eastern & African morphotypes serve as a high-risk demographics particularly for hypertrophic and keloid scar formation. Whilst dermal rolling and fractional laser have been preferred treatments for scar reduction since the 1990s, the resulting thermolysis and ablation can be problematic for PIH and the activation of pro-fibrotic growth factors.

Materials / method: Updates with skin-needling offer a mechanical scar revision where procedural depth and speed may be customised at any point or time. There is less practitioner dependence. A combination of gliding and imprint techniques not only allow for the physical breakdown of existing fibrous tissue but can also administer steroid and anti-mitotic drugs via infusion method. Such drugs include TA and 5-Fu. A pre-op intralesional injection however may be administered if required. Scars may be treated at multiple angles, without heat or ablation. The procedure may be performed in minutes and without TLA.

Results: Patients report an improvement in lesion size and softness within a few days. This effect continues and is maximised with follow-up treatments for at least 4 procedures in total. So, long as the treated area is not re-traumatised, results are long-lasting and provide an above satisfactory cosmetic result. Associated post-inflammatory hyper and hypopigmentation also appear to become rebalanced.

Conclusion: The modern patient demands comfortable and affordable procedures that deliver long lasting results. With the pressures of social media and the rise of the ‘selfie’ (face photos) and ‘belfie’ (body photos), patients affected by post-traumatic and post-surgical hypertrophic scars on their face and body now have a new option for scar revision.
