NEXTGEN SURGERY AWARD - Carbon-assisted Q-switched Nd: YAG laser and microneedling delivery of botulinum toxin: a new combined treatment

Objectives: Objectives
1. Diagnose irregular skin texture, dilated facial pores and acne lesions
2. Formulate a combined protocol to improve irregular skin texture, dilated facial pores and acne lesions
3. Manage a combined. protocol of Carbon-assisted Q-switched Nd: YAG laser and Microneedling Delivery of Botulinum Toxin

Introduction: Carbon-assisted Q-switched Nd:Yag laser treatment has been used in order to improve skin texture, superficial wrinkles, acne lesions and dilated pore. This technique consists of applying a topical carbon suspension all over the face followed by irradiation with a Q-switched 1064 Nd:YAG laser. The injection of multiple microdroplets of dilute Botulinum toxin A (BTX-A) into the dermis, has been investigated as a tool for facial rejuvenation. The aim of this study is to assess the effectiveness of the combined treatment with botulinum toxin and carbon peel laser.

Materials / method: All patients were subjected to 3 sessions of Carbon-assisted Q-switched Nd: YAG laser and Microneedling Delivery of Botulinum Toxin, 3 months apart from the other.
To assess the improvement we used Fitzpatrick Wrinkle Assessment Scale (FWAS), the Physician Global Aesthetic Improvement Scale (GAIS), a photographic scale for the pore assessment and the Investigator Global Assessment of Acne (IGA). Face Q was also administered (Rasch Measurement Theory analysis was used). The scores obtained were compared using a paired T test. A p value < 0.05 was considered as a a cut-off for significance.

Results: Twenty patients were recruited. Difference of pre and post-treatment scores of the FWAS were statistically significant (p-value =.000349) demonstrating an improvement of the of thin and deep lines on the face.
The GAIS showed a very good overall improvement of the aspect off the face. A significant difference between pre and post-treatment scores of the photographic scale for the pore assessment was observed. Conclusion: Carbon-assisted Q-switched Nd: YAG laser and Microneedling Delivery of Botulinum Toxin could be considered as an adjunctive tool in patients suffering from acne lesions, dilated pores and irregular skin texture. The high versatility of this combined protocol allows customized treatment with minimum discomfort and high tolerability in patients.

NEXTGEN SURGERY AWARD - 3D surface scanner in facial surgery - Indications, advantages and case presentation

Objectives: 1. Describe the use of a 3D surface scanner in facial surgery.

2. Discuss the indications of using 3D surface scanner in facial surgery and its advantages:
2.1. Facial analysis: assessment of dentofacial deformities.
2.2 Facial reconstruction: creation of customized anatomical models.
2.3 Scanning of preformed prosthetic elements.

3. Case presentation: Auricular reconstruction.

Introduction: Advances in 3D technology have enabled significant progress in diagnostic and therapeutic possibilities in surgery. This is especially noticeable in facial surgery where the precision required is very high in all cases. Until now we relied on radiological imaging: CT and MRI and digital photography. Advances in radiological imaging have allowed us to render 3D rendering of bone parts but not at the soft tissue level. The use of the surface scanner provides a solution to this deficit. Although current applications are limited, its results are already remarkable.

Materials / method: During the diagnostic stage: they allow advanced facial analysis, their integration with specific software allows anthropometric measurements to be taken and volumes to be measured.

During the treatment stage: in the pre-surgical preparation, with the possibility of scanning facial structures, especially the ears and nose, to allow the printing of 3D patient customized models. As a complementary use, it allows the scanning of prosthetic elements to check their adjustment and allow surgical navigation during surgery.

During the follow-up stage: allows pre- and post-treatment comparison.

Results: We present the use of this technology and its application in a case of auricular reconstruction due to microtia. The steps of the process are described step by step:

- Diagnostic: Scanning of the contralateral ear in the patient, data processing, creation of the mirror model and printing of the 3D model.
- Therapeutic: Use of the printed model during intra-operative carving of the autogenous cartilage framework.

The use of this technology allows to be more precise in the creation of the ear, with exact anatomical characteristics of the contralateral ear and reduces the surgical time.

Conclusion: The application of surface scanning in facial surgery is already providing important advantages in all phases of treatment: diagnostic, therapeutic and follow-up. The main advantages are to increase surgical precision and to offer patient customized results, bringing our clinical practice closer to the highest standards of personalized medicine.

NEXTGEN SURGERY AWARD - Soft-tissue correction method for a sunken eyelid

Objectives: This study aims to introduce our methods to surgeons and to give them the opportunity to adapt it in their practices.

Introduction: The periocular area is one of the initial parts of the face that show signs of aging, where patients usually worry about the visual manifestations of aging, such as the sunken lower eyelid. The condition is usually caused by iatrogenic factors or involutional changes in the periocular area. This study aimed to develop and improve surgical methods to fill the sunken lower eyelids and examine their efficacy and safety.

Materials / method: This study included 26 patients who had been treated with the musculofascial flap
transposition method from the upper to the lower eyelid, under the posterior lamella.
In the presented method, a deepithelized triangular musculofascial flap with a lateral feeding pedicle was transposed from the upper eyelid to the lower eyelid tear through depression

Results: In all patients, the method achieved either a complete or a partial elimination of the defect.

Conclusion: The proposed method to fill a defect of soft-tissues in the arcus marginalis can be considered useful if upper blepharoplasty has not been performed previously and the orbicular muscle has been preserved.

NEXTGEN SURGERY AWARD - Treatment of connective tissue disorder with micro and nanofat - Case report

Objectives: We present a case of 30 years old women who suffered from
connective tissue disorder diagnosed as Scleroderma by her
dermatologist. She had facial atrophy, more pronounced on right
side, “salt and pepper” skin appearance with hyperpigmentation. She
complained about difficulties with mouth opening and she also had
asymmetrically positioned earlobes and eyeballs. Her skin presented
decreased elasticity.

Introduction: Introduction: Scleroderma is a type of autoimmune disease that
affects connective tissue. It is more common in women, appears
between ages of 30 and 50, but affects all races. It is characterised by
increased synthesis of collagen, damage to small blood vessels,
activation of T lymphocytes and production of altered connective
tissue. Scleroderma can be classified as localised and systemic.
Treatment is symptomatic. The latest advancement in the treatment
is the application of the adipose tissue and regenerative cells that are
located within it. Subcutaneous adipose tissue is a source of

Materials / method: Microfat was injected in subcutaneous layer in withdrawal fashion in the following regions: zygomatic, nasolabial fold, jaw line, buccal and perioral region. During the first procedure we performed lipofilling with 58 ml, 3 months after and we injected 30 ml of microfat and 5 months after the second surgery, we injected 30 ml of microfat on the right side of the face and also 15 ml of previously prepared nanofat that we applied using nappage technique.
We followed up the patient between the procedures and during one year after the last surgery. For the evaluation of the result we used pictures

Results: Results: All procedures were uneventful and well tolerated by the
patient. Both, the patient and surgical team were pleased with the
final aesthetic outcome. The skin appearance and the symptoms
such as mouth opening were significantly improved as well. We present pictures before and after each procedure.

Conclusion: Conclusion: This procedure is very useful for mitigating symptoms of
tissue fibrosis, improving the quality of skin and quality of patients
life. More cases are needed before this procedure becomes standard
for connective tissue disorders treatment.
