Objectives: Medical rhinoplasty is playing an increasingly important role in our practice. All the publications explain that the injection must take place against the bone or against the cartilage under the Smas, i.e. in the deep fat. But, as dissections show and as Dr. Sabban, among others, writes, there is no deep fat under the nasal smas. This explains the large number of sometimes very serious complications encountered by this type of deep injection. On the other hand, very superficial injections using inter-pore or vacuum techniques, depending on the desired result, eliminate these major risk
