Introduction: In this document we are going to talk about hair transplant or hair restoration, the new techniques and also my own experience, during my 15 years of practice this kind of surgery. It’s a pleasure for me communicate to all of you some of my experience. I want you to know that, hair transplant as a procedure is successful only when hair loss is stabilised, as it will not stop further hair loss. We as surgeons can only relocate and transfer few strong follicles from donor area to recipient area without increasing any new follicle per se.

Results: 12 cases of Hair implant, Bread Implant and Eyebrow Implant

Conclusion: Hair transplant surgery demands good planning, great surgical skill and good aesthetic sense on the part of the clinician, as hairs grow in specific direction and angulations for the given area of the scalp, and grafts should be placed in a way which should look natural and cover the scalp gaps thus achieving good density.

Objectives: 5 alpha reductase is the gold standard molecule for treating androgenic alopecia, but in the same way a lot of side effects are related to it. Is it true in real practice? Are these side effects prevalent enough to consider instead of the good clinical results? In this lecture we will discuss based on recent evidence and real clinical cases the role of this molecule in non cicatricial alopecia.

Objectives: Minoxidil has been used as a topical treatment for androgenetic alopecia (AGA) for more than 30 years. Unfortunately, clinical trials have demonstrated that approximately 60 to 70% of patients do not achieve hair growth following six months of treatment. The objective of this session is to understand the mechanism of action of minoxidil on hair growth, limitation of the drug and possible mechanism to increase its efficacy.

Introduction: Minoxidil is a pro-drug requiring conversion to minoxidil sulfate by sulfotransferase enzymes (SULT1A1). We have reported extensively that SULT1A1 activity in the outer root sheath (ORS) of the hair follicle correlates directly with topical minoxidil response. Here we present the results of a topical minoxidil adjuvant therapy designed to increase the SULT1A1 activity of the ORS.

Materials / method: A novel topical formula was developed. Nineteen patients with clinical diagnosis of AGA were recruited for a fourteen day study. Twelve hairs from boarder of scalp with alopecia and normal scalp were plucked for sulfotransferase analysis at the first visit. Subsequently, the patients were provided the adjuvant therapy and instructed to apply 2 ml of the new minoxidil booster once daily in the same target area. After fourteen days, twelve hairs from the same location were collected again for sulfotransferase analysis.

Results: ORS sulfotransferase activity increased for 10 of 19 patients after 14 days of applying the novel formula. More importantly, subjects predicted to be non-responders to minoxidil from the assay had the most significant increase in their SUT1A1 activity. Pre-treatment, the average OD in the non-responder group was 0.2206 (95%CI: 0.1661 to 0.2750) compared to post-treatment 0.4946 (95%CI: 0.2036 to 0.7855) (p<0.03).

Conclusion: In the reported pilot study, we demonstrate the ability of a novel topical formula to increase the activity of SULT1A1 in the outer root sheath of the hair follicle. Increasing the activity of SULT1A1 will likely increase the efficacy of topical minoxidil for the treatment of AGA.

Objectives: to identify current studies on the use of tranexamic acid in surgical procedures.
-to evaluate the possibility of using tranexamic acid for hair transplantation
-to identify the evidence on the best route of use of the medication - topical, oral or venous.
-to evaluate the risks and possible complications of tranexamic acid in hair transplant surgery patients

Introduction: Tranexamic acid is an inexpensive medication, used for several years to control bleeding in different situations. This medication acts on the fibrinolytic system to control perioperative bleeding. despite the old use, there are several uncertainties about the best route of drug administration. There is also great concern related to risk of thrombotic complications (deep vein thrombosis [DVT], pulmonary embolism [PE], myocardial infarction [MI], and cerebrovascular accident [CVA]) and seizures using this medicaton.

Materials / method: Bibliography related to the use of tranexamic acid in orthopedic and plastic surgeries and directly in hair transplant surgeries was reviewed. its use was analyzed in its topical, oral and intravenous.

Results: New studies seem to demonstrate that the use of tranexamic acid in hair transplantation is safe. the reviewed studies demonstrate safety in relation to the use of the drug, most of which demonstrate that there is no great risk of ischemic complications. its use can reduce the amount or even suspend the use of adrenaline in hair transplants in patients with a higher risk of ischemia and necrosis in the scalp. Topical use should be avoided for long durations due to the risk of seizures with prolonged use in this route of administration

Conclusion: There are few studies regarding the use of this drug in hair transplantation. new studies related to the best route of administration in relation to hair transplantation may make tranexamic acid a key medication for this type of surgery, providing a cleaner surgical field and faster procedures with better results for patients.
