Objectives: To assess improvement of both tone and texture using a high power, single wavelength fractional non-ablative 1,064-nm laser with with 2 different pulse durations; targeting both epidermis and dermis with a novel focused depth controlled technolo

Introduction: As patients are seeking for non-invasive procedures, these continue to rise in popularity. The market is flooded with new devices- for wide range of indications addressing both epidermis and dermis. Usually few devices are needed to target different depths and chromophores and to achieve desired result.

Materials / method: 20 patients with sun damage and reduced skin quality underwent 4- 6 treatments on the face and neck. Patients were treated with 2 different modes: Quasi Long Pulse (QPL) 1064nm immediately followed by high power fractional Q-switched 1,064-nm laser. The platform is a stand-alone system comprised of 4 distinct modes of operation-two were used in the study. The second applicator which was used operates with high repetition rate, large spot size allowing high coverage rate, where a variable depth control technology allowed targeting epidermis or dermis. Both treatment modes are non-ablative with

Results: 82% of patients had shown significant improvement in both dermal and epidermal indications, including: tone, wrinkles, pores etc'. Side effects were minimal and included mild and transient erythema and petechia.

Conclusion: The stand-alone fractional non-ablative QS & QLP 1,064-nm laser and it's focused depth controlled technology is clinically safe and effective in addressing epidermis and dermis resulting in clinical multilayer improvement of skin tone and texture. Since the treatment is non-ablative it serves as an optional tool for patients wishing for treatment without downtime.

Dermoscopy facilitates laser treatment of pigmented lesions in patients with darker skin types

Objectives: 1. To learn dermoscopy can help the diagnoses of non-melanocytic pigmented lesions which can help treatment planning.
2. To realise dermoscopy can identify dermal vessels and pigmentation not seen by naked eyes which otherwise when stimulated by high laser fluence may result in post-inflammatory hyperpigmentation.
3. To know dermoscopy can visualise clearly the appropriate laser treatment endpoints such as frosting and dermal vascular coagulation and to monitor treatment progress.

Introduction: Dermoscopy allows identification of over hundred features for skin lesions. It plays an important role in the diagnosis and management of various non-melanocytic pigmented lesions in laser dermatological practice especially when treating patients with darker skin types. Portable non-contact polarized dermoscope with connection kit for the camera of mobile phone can be a convenient and valuable tool for laser physician to increase diagnostic accuracy and avoid side effect from laser treatment.

Materials / method: This is a review of 5-years experience of how dermoscopy can assist laser treatment of non-melanocytic pigmented lesions for Fitzpatrick skin type III and IV (FST III and IV) patients in a single laser dermatological practice in Asia.

Results: Dermoscopy can differentiate superficial from deep pigmentation so as to facilitate selection of laser treatment. It can also identify dermal vessels and pigmentation not seen by naked eyes which otherwise when stimulated by high laser fluence may result in post-inflammatory hyperpigmentation. Dermoscopy helps us to visualize clearly the appropriate laser treatment endpoints such as frosting and dermal vascular coagulation and to monitor treatment progress.

Conclusion: Dermoscopy helps laser physicians to diagnose different types of non-melanocytic pigmented lesions and to achieve better treatment efficacy and at the same time avoid side effect especially for darker skin type patients.

The effect of cold therapy and transepidermal nerve stimulation on weight loss and inch loss

Objectives: 1. Learn how Cold Therapy works in producing Weight Loss
2. Discover how much Weight Loss can be produced by Cold Therapy

Introduction: Slimming Centers in Switzerland have been using Cold Therapy for Weight Loss since the 1960’s & Trans Epidermal Nerve Stimulation for Inch Loss since the 1980’s. A recently, published Scientific Study (41,098 slimming treatments of 1,859 patients from 11 slimming centers w/ Cold Machine slimming programs consisting of 13,638 Cold Machine treatments) showed an average Weight Loss of 5 pounds & Inch Loss of 1 ½ inches in 8-12 treatments (roughly 10 treatments in 5 weeks if done 2x/wk). These current multiple cases were picked to show that better results can be achieved higher than the average.

Materials / method: Cold Therapy stimulates Non-Shivering Thermogenesis to stimulate Brown Fat Activation for burning of calories to induce Weight (Fat) Loss. It uses a machine that cools water & pushes it into rubber tubings arranged in an array like a blanket. This is wrapped around the body. Trans Epidermal Nerve Stimulation simulates 225 Sit-ups w/ 2 second abdominal muscle contraction and 2 second relaxation alternating between flexor vs. extensor muscles in 15 minutes but uses Isometric (not Isotonic) contractions. It is used for toning of loose tummy after fat loss with Cold Therapy.

Results: Based on 70 multiple cases (54 females, 16 males), from 11 private Slimming Centers, with ages ranging from 13 y.o. to 67 y.o., w/ BMI from 19.6 to 68.5 kg/m2 (Normal Weight to Obese III), from Jan 22, 2014 to Dec 19, 2016, their average Weight Loss is 12.2 lbs (10 lbs – 26.4 lbs) and average Inch Loss is 2.6 inches (2 in – 6.5 in) in an average duration span of 5.66 wks (4 wks – 6.72 wks).

Conclusion: Based on the multiple cases presented in this report, a patient can lose 10 lbs in gross weight and 2 inches in waist/tummy circumference within 6 weeks. It should be noted though that the ones with the higher BMIs (Obese II and Obese III) had the best weight loss results while those with lower BMIs (Normal Weight and Overweight) were the ones with good inch loss results.

The comparison between Er:Yag and CO2 laser for male circumcision

Objectives: How to do Male Laser Circumcision using 2940 nm Er:YAG Laser and the benefit of Er: Yag laser for circumcision

Introduction: Male circumcision is the surgical removal of the foreskin (prepuce) from the penis. The word ‘circumcision’ comes from the Latin circumcidere, meaning ‘to cut around’. Infant male circumcision dates back more than 6000 years as indicated in pharaonic drawings and circumcised Egyptian mummies. Some historians even suggest that the procedure started some 15,000 years ago.

Materials / method: The method of circumcision used was similar to the manual guillotine technique, but instead of a scalpel the Er:YAG laser (2940 nm). Using the R08 handpiece with a pulse duration of MSP (100 us), 50 Hz and 140 mJ, we could remove the preputial skin with minimal surrounding tissue damage. For the more vascularized parts we used the R08 with a pulse duration of LP (600 us), 20 Hz and 180 to 200 mJ, which cuts and coagulates at the same time.

Results: The Er:YAG laser is an ideal device to remove epithelium of the glans penis without risk of scarring and faster healing than manual or CO laser

Conclusion: An alternative for circumcision: We can use Er:YAG laser as a low risk, replacing more thermally destructive CO2 laser.

Treating acne vulgaris patients who are resistant to oral isotretinoin with intense pulsed light (IPL)

Objectives: Treating acne vulgaris patients who are resistant to oral isotretinoin with intense pulsed light (IPL)

Introduction: Background Acne Vulgaris is a long-term skin disease that occurs when hair follicles are clogged with oil and dead skin cells from the skin. Severe cases were treated with oral isotretinoin. Nevertheless, around 15% of these cases were resistant to oral isotretinoin or recurrence occur once the physician stopped the medication. We are presenting four Chinese patients with acne vulgaris resistant to oral isotretinoin successfully managed with intense pulsed light treatment.

Materials / method: Methods 4 cases with oral isotretinoin treated acne vulgaris Chinese patients referred by other doctors were studied. All four of them received 12 sessions of IPL treatment in a three weeks interval. Clinical photos were taken in every visits. Two individual blinded investigators were asked to assess and compare with the pretreatment photos.

Results: There were significant clinical improvement in the number of comedones, redness and active lesions. The scarrings do not have significant improvement in appearance.

Conclusion: This study suggests IPL is effective in improving the number of comedones, redness and active lesions of oral isotretinoin resistant acne vulgaris patient. However, scarring cannot be improved with intense pulsed light treatment.
