Objectives: Although treatment with topical and systemic antibiotics, azelaic acid, isotretinoin, tranexamic acid, chemical peels and intense pulsed light therapy may significantly improve the papulopustular component of rosacea, in some patients the vascular symptoms of persistent erythema, mild edema, flushing and telangiectasias tend to persist.

Introduction: The therapeutic benefit of BTX in rosacea probably stems from acetylcholine blocking effects targeting arrector pili muscles (reducing pore size) and local muscarinic receptors in the sebaceous glands. Improvement in flushing, erythema and inflammation are attributable to potent blockade of acetylcholine release from autonomic peripheral nerves of the cutaneous vasodilatory system, and inhibition of the release of inflammatory mediators such as calcitonin gene-related peptide (CGRP) and substance P.

Materials / method: Botulinum toxin (BTX) diluted to 10 units/ml is administered intradermally in the hypervascular and telangiectatic centrofacial face as 0.05 ml microdroplet injections with 0.5 cm spacing under topical anaesthesia.. Significant reduction in erythema, edema, telangiectasias, and flushing is apparent within 1-2 weeks. Any remaining papulopustular lesions also show improvement;additiona Botulinum lly a reduction in pore-size is also noticeable within 2 weeks. The improvement lasts for 3-4 months but repeat sessions of BTX Mesotherapy are required once every 4-5 months to maintain the remission

Results: Clinical image after 3 weeks of treatment with intradermally injected diluted botulinum toxin displaying a significant reduction in the erythema, edema, inflammatory lesions as well as inconspicuousness of pores .Sustained clinical improvement appreciable after 3 month.

Conclusion: In conclusion, diluted BTX mesotherapy repeated every 4-5 months is an option to relieve the vascular signs and symptoms of rosacea, although the procedure is limited by the cost, and the need for repeated injections of BTX.

Objectives: 1: Diagnose facial flushing and understand the photobooster
2: Formulate a tailored approach to facial flushing with PDL
3: Understand how to enhance the therapeutic effect of PDL treatment in facial flushing

Introduction: Flushing is defined clinically as a transient reddening of the face and other areas. Due to the transient nature of flushing, a patient may not show signs of flushing during laser treatment. The aim of this study was to evaluate the efficacy and safety of 595-nm pulsed-dye laser treatment of flushing or erythema after provocation of flushing by topical niacin cream.

Materials / method: We retrospectively reviewed a total of 25 Korean patients with facial flushing who were treated with three sessions of 595-nm pulsed-dye laser after the application of topical niacin cream.

Results: Follow-up results revealed that 12 of the 25 patients demonstrated marked clinical improvement of baseline facial erythema. Eight patients had moderate improvement and three demonstrated near total improvement. Two patients showed minimal to no improvement. We observed that the reactivity to topical niacin cream was markedly reduced in 64% of our patients after 595-nm pulsed-dye laser treatments. Minimal post-therapy facial oedema was noted in most of the patients, which usually resolved spontaneously within 2 days. Pronounced facial swelling was observed in four patients.

Conclusion: We suggest that 595-nm pulsed-dye laser treatment after provocation of flushing by topical niacin cream may provide a new treatment algorithm for facial flushing in Asians.

Objectives: The purpose of this study was to evaluate the effectiveness of EBD on acne erythema treatment.

Introduction: Acne erythema is considered a low-grade scar, and it can be stressful for patients.
The persisting scars can develop from post-inflammatory acne lesions. Therefore, it is important to treat post-acne erythema as early as possible.

Materials / method: Acne erythema was evaluated after treatment with PDL and needle RF treatment.
Especially for needle RF treatment, a skin color analysis system was used to evaluate the acne erythema's improvement objectively.

Results: The number of lesions and summation of the area exceeding the erythema threshold on the forehead, left malar, and right malar was evaluated by the skin color analysis system. In RMANOVA, the number of lesions and area of acne erythema showed a statistically significant decrease over time except for the number of forehead lesions.

Conclusion: Pulsed dye laser and needle RF are efficient tools for acne erythema treatment, improving patients' DLQI and reducing the persisting scars of acne.
