Introduction: Background: in recent years an increasing interest in volumetric hand rejuvenation can be noticed. Structural fat grafts or injectable fillers are normally used to achieve a smooth surface with a good coverage oft he veins. As in any rejuvenation technique, safety is an issue to achieve a maximum of patient´s satisfaction. Cadaver dissection, literature review, research and personal experience lead to guidelines to be safe and effective.

Materials / method: Methods: 20 cadaveric hands preserved in a Thiel´s solution have been dissected. Ultrasound was used on 20 living hands to explain the different layers of the back oft he hand. Literature review was used to compare the best posttreatment outcomes

Results: Results: The author´s anatomical examinations correspond exaxtly to the literature showing three fatty laminas separated by three fascias. 10-12 perforating vessels transverse the three laminae raising from the dorsal metacarpal artery which run parallel to the metacarpal bones within the fascia of the dorsal interosseous muscles deep to the extensor tendons.5,9
Dorsal veins and dorsal sensory nerves resided can be found within the intermediate lamina. The extensor tendons are running within the deep lamina. Beneath the dorsal deep fascia, dorsal interosseus muscles can be found.

Conclusion: Conclusions: To perfome a perfect volumetric dorsal hand rejuvenation, the anatomy hast o be respected. The ideal layer for injection ist he superficial lamina. Care hast o be taken, not to destroy the perforating vessels, which are raising from the dorsal metacarpal artery, especially 1 cm proximally to the metacarpal heads. Similar effects can be achieved using autologous fat, hyaluronic acid fillers or calcium hydroxylapatit.

Objectives: Introduction – The use of dermal-filler has now come into its own also in the treatment of hypo-atrophy in the dorsal region of the hands.
Its particular the use of the use of Advanced Dermal Filler are able to produce a light inflammatory reaction with subsequent stimulation and production of neo-collagenesis, type I and type III.with better outcomes in term of results and long lasting results.

Introduction: Introduction – The use of bio-active dermal-filler has now come into its own also in the treatment of hypo-atrophy in the dorsal region of the hands.
Its particular the use of these substances able to produce a light inflammatory reaction with subsequent stimulation and production of neocollagenesis, type I and type III.

Materials / method: Methods and Materials– Calcium Hidroxylapatite or Polycaprolactone in an aqueous gel was injected in a suitable ambulatory environment, using multiple injections 4-6, were done, with the additional use of lidocaine 0,5 % in pomphus or bolus-like injection points: for creating appropriate layering and for pain management. Furthermore we are using 27 G cannula applications.
Post-treatment: dressing, white cotton glove for 3 h, home auto-massage by hand using cream on the injected site to ensure a uniform distribution.

Results: Results – The treatment reveals a reinforcement of the sub-dermal matrix of the dorsal region of the hands provoking an immediate improvement, with consequent stretching/thickening of the overlying cutaneous tissue and then a secondary increase, in the following 2-3 months, of the autologous synthesis of new collagen with the improvement of the texture and therefore the harmony of the entire hand.
The effect of the injection of the substance is nevertheless temporary: it has a variable duration of 12-18 months, the result depend from the substance used, the amount, the age of the patient.

Conclusion: Conclusions – These procedures are simple and safe, it do not interrupt the patient’s normal social life or working activity, and the results are highly satisfying, even with a single application, plus it’s repeatable.
